Reactions 1484, p28 - 18 Jan 2014 S Prednisolone Psychiatric disturbances in a child: case report An 8-year-old boy developed psychiatric disturbances following treatment with prednisolone. The boy presented to a hospital with pallor, malaise and a sore throat. He also had splenomegaly. Investigations revealed anaemia and reticulocytosis. His haemoglobin stabilised during his hospital stay, but he continued to have haemolysis. Years later, he experienced decompensated haemolysis with malaise, lethargy, marked splenomegaly and a fall in haemoglobin. He was started on prednisolone 1 mg/kg [route and frequency not stated]. His blood count stabilised, but treatment was complicated by psychiatric disturbances [duration of treatment to reaction onset not stated]. At a routine follow-up appointment, the boy reported keeping over 100 racing pigeons in his home. He had a case of a pigeon allergy causing severe haemolytic anaemia. Observation and immunosuppressant therapy had been ineffective in controlling the disease, and attempts at reducing antigen exposure did not work as well. Once the birds were culled, his haemoglobin level started to increase. The reticulocytosis continued to fall, and steroids were discontinued.
