SPrednisolone
Psychiatric disturbances in a child: case reportAn 8-year-old boy developed psychiatric disturbances
following treatment with prednisolone. The boy presented to a hospital with pallor, malaise and a
sore throat. He also had splenomegaly. Investigations revealedanaemia and reticulocytosis. His haemoglobin stabilisedduring his hospital stay, but he continued to have haemolysis.Years later, he experienced decompensated haemolysis withmalaise, lethargy, marked splenomegaly and a fall inhaemoglobin. He was started on prednisolone 1 mg/kg [routeand frequency not stated]. His blood count stabilised, buttreatment was complicated by psychiatric disturbances[duration of treatment to reaction onset not stated].
At a routine follow-up appointment, the boy reportedkeeping over 100 racing pigeons in his home. He had a case ofa pigeon allergy causing severe haemolytic anaemia.Observation and immunosuppressant therapy had beenineffective in controlling the disease, and attempts at reducingantigen exposure did not work as well. Once the birds wereculled, his haemoglobin level started to increase. Thereticulocytosis continued to fall, and steroids werediscontinued. [Outcome of ADR not stated.]
Author comment: "He was commenced on prednisolone(1 mg/kg) which stabilised his blood count but wascomplicated by psychiatric disturbances."David T, et al. Severe haemolytic anaemia secondary to avian allergy. Blood 122:No. 21, 21 Oct 2013 [abstract] - United Kingdom 803098306
