Dilema Seorang PekerjaPak Kimo, 45 tahun adalah seorang
karyawan sebuah pabrik kimia yang bergerak dalam penyediaan bahan baku kimia untuk berbagai industri dan kesehatan. Pak Kimo yang berlatar belakang sekolah tehnik kimia, ditempatkan di bagian produksi sejak 20 tahun yang lalu. Akhir-akhir ini dia sering mengeluh badan terasa meriang dan mudah pilek. Sedikit saja bekerja lembur, esok harinya dia akan merasa demam dan sakit kepala.
Setelah dianjurkan oleh istrinya, pak Kimo datang ke poliklinik perusahaan. Pada pemeriksaan fisik dokter menemukan konjungtiva subanemis, sklera tidak ikterik, suhu 37,80C, tekanan darah 140/80 mmHg. Dokter juga menemukan limfadenopati, single, diameter 1 cm, konsistensi kenyal-padat dan mobile di regio cervical sinistra. Dokter menyarankan untuk melakukan pemeriksaan lengkap dan membuatkan surat pengantar untuk pemeriksaan rontgen foto torak, laboratorium darah dan FNAB (fine needle aspiration biopsy). Rontgen foto torak menunjukkan adanya bagian infark lama di jantung, disertai hipertrofi ventrikel kiri, sedangkan dari pemeriksaaan FNAB sesuai dengan mononukleosis infeksiosa. Pemeriksaan darah Hb 9,5 mg/dL, limfositosis, kolesterol dan trigliserida meningkat. Akhirnya dokter memberikan resep dan menyarankan pada pak Kimo untuk memperhatikan gaya hidupnya, termasuk masalah pekerjaannya, agar tidak terus sakit sakitan karena bisa berakibat buruk. Pak Kimo jadi bingung, apakah dia harus berhenti bekerja? Bagaimana anda menjelaskan apa yang terjadi pada pak Kimo?
Terminology
1. Konjungtiva Subanemis : Keadaan pada selaput pembungkus mata ( konjungtiva ) yang tidak terlalu pucat dan menandakan seseorang mengalami hampir anemia.
2. Limfadenopati : Pembesaran limfe secara abnormal yang belum diketahui penyebabnya. Limfadenopati merujuk kepada ketidaknormalan kelenjar getah bening dalam ukuran, konsistensi ataupun jumlahnya.
3. FNAB : suatu metode pemeriksaan dengan sebagian jaringan tubuh dengan menggunakan aspirator ( jarum yang lebih kecil dari jarum suntik ) untuk pemeriksaan tumor
4. Mononukleosis infeksiosa : penyakit yang disebabkan virus Epstein Bar ( EBV ) ditanda dengan gejala demam, limfadenopati,
5. Limfositosis : merupakan salah satu mekanisme pertahanan tubuh terhadap patogen dengan cara meningkatnya jumlah sel limfosit dalam darah.

Analisis Masalah
1. -lokasi kerja - Durasi paparan radiasi - Sistem imun yang berkurang
2. Karena pak Kimo terpapar zat kimia, mungkin saja respon imun terus bekerja, sampai respon imun pak Kimo tak sanggup lagi untuk bertahan, sehingga apabila ada patogen masuk pak kimo mudah mengalami meriang dan pilek.
3. - kurang istirahat - tempat kerja yang tidak sehat - S. Imun berkurang Akibatnya dengan sedikit patogen mudah terserang penyakit
4. - Konjungtiva menandakan bahwa pak Kimo hampir anemis.
• - Sklera tidak ikterik menandakan tidak ada kelebihan bilirubin dalam darah yang berarti fungsi hati normal.
• - Suhu badan menunjukkan lebih dari 37,5°c, menandakan hipertermi.
• - Tekanan darah 140/80 mmhg, menunjukkan bahwa pak Kimo mengalami hipertensi sistolik terisolasi.

The virus has a specific surface proteins (ligands) that bind to host proteins (receptors).
MECHANISM
1. Virus (ligand) binds to host cell surface receptors
2. After attaching the virus to enter the cytoplasm by 3 pathways:
a. Translocation through the plasma membrane of intact virus
b. Endocytosis mediated by a receptor in the membrane fusion and endosome
c. Envelope protein diffusion through the cell membrane
3. In the cell, separating the genome of the virus structural components
4. Viruses reproduce themselves by using a special enzim.
The virus will kill the host cell and cause damage
in several ways: The virus may inhibit the synthesis of DNA,
RNA, host cell protein Plasma membrane proteins make up the virus
and destroy its integrity Virus replication and host cell lysis lymphocytes and the immune system attacks
the virus-infected cells The virus causes secondary infections.
