THE HEALTH ISSUE• Over 50% of Tohono O’odham Nation has

been diagnosed with Type 2 Diabetes.• This is the highest rate in the world. • Precursors are lack of physical activity, obesity, poor diet, and tobacco use.

APPLICATION OF PRECEDE-PROCEEDOur desired outcome for the N.A.T.I.V. is to:• Reduce the rate of type II diabetes in the

Tohono O’odham adult male population with the help of the Tohono people

The N.A.T.I.V. program will improve:• The current poor diet and the sedentary

lifestyle that are the main causes of type II diabetes in the Tohono male population

APPLICATION OF PRECEDE-PROCEEDWith the implementation of our 5-year

program we will:• Create better access to healthier, traditional

food through community gardens.• Create an exercise program that will allow

individuals to live a more active life.

Goal: To reduce the prevalence of Type II Diabetes found in Tohono O’odham males living in Pima County, Arizona 5 years from the date of the program implementation.

N.A.T.I.V. OBJECTIVES1)Outcome Objective:

a) By the end of the 5 year program, there will be a 40% reduction in the rate of Type II Diabetes among Adult Tohono O’odham males.

N.A.T.I.V. OBJECTIVES 2) Behavioral Objectives:

a) By the end of the program, 70% of the Tohono O’odham participants will be able to demonstrate basic nutritional skills learned from the program.

b) By the end of the program, 70% of the Tohono O’odham participants will demonstrate their knowledge of physical activity and its importance.

N.A.T.I.V. OBJECTIVES 3) Administrative Objectives:

a) By the third month of the program, 100 Tohono O’odham adult males will be enrolled into the N.A.T.I.V program

b) Receive consent from community leaders and members by explaining the program and its goals.

c) Administrative Objectives for staff:i) Recruit two professional (fitness educator and

nutritionist) for each of the 10 groups, for a total of 10 fitness educators and 10 nutritionists.

steps:• Step 1: Information Workshop• Step 2: Dietary/Exercise Planning• Step 3: Community Gardening• Step 4: Utilizing the plans• Step 5: Reporting to groups on a weekly

basis
