Sykehistorie

• 38 år gammel kvinne

• Symptomfri, engstelig for kreft

• Henvist til en rutine undersøkelse ved

gynekologisk poliklinikk

• UL (2012): myom i uterus + på høyre side

en glattvegget simpel ovarialcyste på 3,5 cm

Sykehistorie

• UL i 2013: uendret myom + en 7x6 cm stor,

enkamret og simpel ovarialcyste,

tilsynelatende relatert til venstre ovarium.

• Laparoskopi: normale ovaria, en stor

peritonealcyste relatert til omentum majus.

Cysten og en del av oment ble fjernet.

Under dette ble cysten punktert, og det kom

ut sjokoladefarget væske.

Makroskopisk undersøkelse

• Cyste:

– 7,0 x 6,5 x 6,5 cm

– Enkamret

– 2-3 mm tykk cystevegg

– Overflaten litt ujevn og kvitlig

– Innsiden er grålig og jevn

Diagnose

Oment-resektat med cystisk, muskulært

hulorgan, innvendig kledd av endometrioid

vev uten malignitetssuspekte forandringer,

forenlig med såkalt "uterus-like mass" i

omentum majus.

Diskusjon

Uterus-like mass (ULM):

– Sjelden og godartet ekstrauterin tumor

– Et sentralt hulrom kledd av endometriode

kjertler og stroma som er omgitt av et tykt lag

med glatt muskulatur

– Morfologi som ligner uterus

either to the liver or the spleen, and it likewise is subject to prolapsus
