AASM: American Academy of Sleep Medicine
Definitions (AASM)Apnea/hypopnea
index (AHI)
1. A clear decrease (>50%) from baseline in the amplitude of a validated
measure of breathing during sleep.
2. A clear amplitude reduction of a validated measure of breathing during sleep
that does not reach the above criterion but is associated with either an oxygen
desaturation of >3% or an arousal.
3. The event lasts 10 seconds or longer (appendix 1).
Respiratory effort-
related arousal
(RERA)
A sequence of breaths characterized by increasing respiratory effort leading to
an arousal from sleep, but which does not meet criteria for an apnea or
hypopnea. These events must fulfill both of the following criteria:
1. Pattern of progressively more negative esophageal pressure, terminated by a
sudden change in pressure to a less negative level and an arousal
2. The event lasts 10 seconds or longer ( appendix 1).

• Case-finding strategie eenvoudig te implementeren in de huisartspraktijk
• Aanpassing inclusiecriteria mogelijk? inclusie vanaf > 1 i.p.v. > 2 positieve vragen? verder onderzoek nodig

Krachtige parameter om ‘gasping’ en snurkpatroon van patiënt te bepalen
Toekomst: enkel JAWAC sensoren en O2 saturatie meting voldoende??
LIMITATIES (1)
• Screeningsvragenlijst vervangt anamnese niet
o Vb. Rouw als oorzaak van slechte slaap en moeheid
o Vb. ‘Moeder zijn van 3 kinderen’ als oorzaak voor moeheid
Extra vraag in screeningsvragenlijst:“ Heeft u zelf een idee wat uw klachten van moeheid zou kunnen verklaren?”
LIMITATIES (2)
• Brochures en screeningsvragenlijsten nietsystematisch uitgedeeld in 3/5 praktijken selectiebias mogelijk studie heeft geen epidemiologische waarde
• Onvolledig/fout ingevulde screeningsvragenlijsten
• Slechts beperkt aantal patiënten die voldeden aan inclusiecriteria konden effectief getest worden met het Somnolter®-toestel
CONCLUSIE
• Voorgestelde case-finding strategie nuttig voor het opsporen van patiënten met hoog risico voor OSAHS: 14 OSAHS / 18 geteste patiëntenMAAR optimalisatie van vragenlijst nodig.
• OSAHS zeer prevalent en frequent niet gediagnostiseerd.
• Somnolter® is gebruiksvriendelijk.
