Contents
Bishan Home was declared open by then Deputy Prime Minister Lee Hsien Loong on 25th September 1999 and started operations the following month. The Home is a joint effort between the Ministry of Community Development, Youth and Sports (MCYS) and the Bible-Presbyterian Welfare Services.
When Bishan Home first started, the Home was registered under the Bible Presbyterian Welfare Services umbrella. In 2007 the Home was registered as a separate Society and obtained Charity and Institution of Public Character (IPC) status in its own right. Bishan Home is also a member of the National Council of Social Service.
From an initial population of slightly over 20 residents, the Home is now near full capacity of 132 residents. In addition to it’s residential program, the Home also runs a Day Activity Centre (DAC).
The Home introduced its DAC program in 1999 as a social service in response to appeals from desperate parents in the surrounding neighbourhood for such a Program. Initially, the program did not receive any government funding. It was only in 2005 that the Home applied to MCYS to fund both the DAC Program and its expansion. Expansion works to the DAC were completed in 2007 and as of 2011 the DAC has 29 clients.

Chairman’s Message
During the Year under review, we increased the number of Day Activity Center Clients to 29. This is the result of the extension of our transport coverage to areas such as Kallang, and Hougang. We also maintained our residential home at near full capacity. Even with the additional work-load, we continued to maintain the high quality of care as seen through the achievement of our EPES goals.
We included new programs for our residents such as cooking classes where our residents learn to cook simple but delicious meals. Other programs of note include our computer classes where the residents learn to use the computer, and our PT department where our therapy aides improve the fitness and flexibility of our clients.
Improvement to our facilitiesWe continued to improve the facilities around the home, in particular the support structure for our IT systems. The electrical wiring was upgraded in key areas of the home, ensuring that essential services will not be affected in case of power surges. Our CCTV camera system was also upgraded to improve the security around the home.
Bishan Home set about revamping its website in 2011. In collaboration with Nanyang Polytechnic and using a grant from IDA, Bishan Home transformed a static website into a dynamic, interactive site that includes video links, updated photos of volunteer activities, outings and Bishan Home events and classes. The new website also has a link to Bishan Home’s Facebook page, and to SG Gives, the local donation portal. We feel the new website will be essential in our future efforts in reaching out, informing and educating the public on intellectual disability and the good works we do in Bishan Home.

Improvements to servicesBishan Home began a collaborative program with the occupational therapy and physiotherapy departments of Allied Health Services, Tan Tock Seng Hospital (TTSH) to improve the quality of the care provided to our residents. With an aging population, it is essential we take steps to help the residents to prolong their ambulating activities, keeping them active and fit. The collaboration with TTSH will enable us to upgrade our therapy services, as well as plan a therapy program that targets the needs of our residents and clients better. An environmental scan done by the TTSH staff also highlighted possible areas of concern for an aging population; Bishan Home is now looking at addressing these areas. Already we are starting to see the results of the collaboration, with less accidents occurring.
We have begun a program for providing medical consultations for our more needy residents with SATA. Aptly named Doctors on Wheels, this program brings SATA doctors to Bishan Home once every three months where they will hold consultations for our residents.
With the aging population in Bishan Home, bringing the doctors to our residents will allow more residents, especially those who face difficulties in traveling to the doctors, more access to health care.
Looking forward to the future - Standards of careBishan Home is well positioned to improve the standards of care in its services to our residents. Bishan Home already has a comprehensive, ISO certified system (re-certified in 2011) in place, which we can build on to establish Standards of Care. For a start, we will be putting a team into place to look at areas we can improve so that we can deliver ever better services and care to our residents and DAC clients. This is in keeping with our ISO quality policy of continual improvement: to provide holistic care to develop life skills of our residents and DAC clients in order for them to lead meaningful lives.
The environmental scan has also highlighted areas where Bishan Home can improve its safety, especially in the light of Bishan Home’s aging population. Bishan Home will look at improving the safety especially in our toilets as it is often the location of most accidents in the home.
In the light of several high profile internal control failures at other organizations, Bishan Home has also begun a review of its processes, looking at tightening our internal controls, as well as looking the choice of vendors and suppliers.
We look forward to another year of continuing review and improvements in standards of care and to extend our care ministry to more in the community who can benefit from our work at Bishan Home.

Year in Retrospect
TTSH therapy programWe began an audit review of our therapy department as part of the
ongoing upgrading of our services in anticipation of an increasingly
ageing population in Bishan Home. The review was conducted by
Tan Tock Seng Hospital Allied Health Services Department in both a
consultant as well as an auditor role. The scope of review also included
an environmental safety scan. The review started in late October and is
expected to be completed in the second half of 2012.
SATA Doctors on Wheels & Biennal health screening programsWe have an MOU with SATA CommHealth under their Doctor on Wheels
program to 3-monthly visits by their Doctor to Bishan Home to provide
medical consultation for our residents and clients. The program ensures
that our residents have access to regular medical screening and regular
follow ups where required. Urgent cases are referred to SATA’s polyclinic
at AMK. Also under a separate program with SATA our residents undergo
a biennial x-ray and blood test screening which is undertaken by SATA’s
health screening unit.
Both programs are one day events and form an integral part of our
healthcare service to our residents by providing affordable access to
medical consultation, laboratory investigations. It also helps the Home
chronic disease management.
13th AnniversaryThis year, Bishan Home celebrated its 13th anniversary in-house, with
our guest-of-honour and patron, Associate Professor Ho Peng Kee. It
was a chance for us to showcase the various programs and activities
held in the Home. Visitors and volunteers alike were taken around the
home, interacting with our residents and clients and participating in the
programs themselves with the help of our staff.
Glaucoma screeningBishan Home enlisted the help of the Glaucoma Society to screen about
90 of our residents for Glaucoma and other related eye conditions. The
Society arranged for a team of volunteer ophthalmologists and nurses
from Tan Tock Seng Hospital to test our residents and staff for eye
conditions

New programs for residents and DAC clientsAs part of a new series of programs for our residents, we introduced
cooking classes for some of our higher functioning groups where they are
made of aware of the ingredients used and the way the food is cooked.
These classes are very popular because the residents get to eat the food
they cooked.
Also our residents with the help of volunteers are encouraged to prepare
simple, easy- to-make dishes such as sandwiches, French toast and bee
hoon in soup.
In this program, our residents are taken shopping at nearby supermarkets
to buy the necessary food ingredients for the cooking classes. This helps
to reinforce the cooking lessons as well to give the residents the social
experience of shopping not only for the right ingredients but also for the
right quantity of ingredients needed and make payment for the purchases.
Horse ridingThis year Bishan Home was invited by the Riding for the Disabled
Association (RDA) to send some residents for their learning to ride
program. The program was conducted over a series of sessions where our
residents learnt how to ride a horse under the guidance of RDA volunteers.
Five of our residents took part in the program and performed well for the
different phases of the horse riding program and were given certificates.
Two of them were given special commendations.
Microsoft training sessionsMicrosoft very generously donated its 2010 Microsoft Office Pro Plus and
Server software to Bishan Home. Microsoft also very kindly sent its staff
on a voluntary basis to teach our staff how they could maximize on the
greater productivity and presentation features provided by Office 2010.
The training was conducted over a series of sessions to ensure that all
Bishan Home staff was trained in the effective use of Office 2010
Christmas carolingBishan Home continued its annual practice of spreading the Christmas
cheer and goodwill by caroling at three Homes, Ling Kwang Home for
Senior Citizens, Singapore Christian Home for the Aged and the Lentor
Residence (Nursing Home). Some 40 residents and staff were involved
in the carolling and were rewarded by smiling and joyful faces of the
residents and their efforts to clap and sing-along with our choir whenever
a familiar carol was sung.

Training and Development
Bishan Home promotes lifelong learning opportunities for adults
with intellectual disability. Our objective is to encourage, enable
and enrich the lives of our residents and clients through programs
where residents and clients are taught through lessons and constant
practice to achieve a quality of life that maximizes and/or stretches
their functioning capabilities.
As part of the training and development programme, all residents
and clients will undergo training in Activities in Daily Living Skills
(ADL) and Community Living Skills (CLS). Our training programs are
outcome based taking into consideration the needs, learning pace,
physical, intellectual and functional abilities of each client.
Our Daily Living Skills program is activity based and focused in the following areas:1. Personal grooming
2. Feeding
3. Mobility
4. Toileting
In our Community Living Skills program we cover:1. Interpersonal Relations – Meaningful social interactions with
others; appropriate social behaviours in public settings.
2. Community Mobility - Using traffic lights to cross (with staff
supervision) the road; able to go to nearby amenities with some
assistance.
3. Community Activities – Participate in social and recreation
activities with volunteers. Can be organized both within and outside
Bishan Home.
4. Home Living – Engagement in simple household chores or food
preparation.
Bishan Home also provides fun based learning activities which
include Hydro-pool therapy sessions, dancing, singing, gardening
and interaction with volunteers through games, art & craft activities,
outings and computer classes.

Training Outcome for residential clients and DAC clients From the period of April 2011 to March 2012
Activities in Daily Living Skills (ADL) training outcomes for both residential clients and DAC clients
(see figure 1) showed that 77% of residential clients and 68% of DAC clients gain independence in
performing at least one new activity in an ADL domain respectively.
Community Living Skills (CLS) training outcomes for both residential clients and DAC clients (see figure
1) showed that 72% of residential clients and 64% of DAC clients gain independence in performing at
least one new activity in a CLS domain respectively.
ConclusionBesides teaching new skills to clients who have achieved their training goals; clients also require
constant practice for learnt skills through application, regular rehearsal and review to ensure that skills
are maintained over time. For clients from lower functioning groups, we provide active support to
enable clients to participate in everyday activities.

Volunteer Activities
NUS Community Service Club (NUSCSC) This is a weekly programme involving 25 students from the NUS Community Service Club. The objectives
include interacting with our Bishan Home residents, helping them develop adaptive skills and social
capabilities through activities between them and volunteers. This program also seeks to promote the
volunteers’ acceptance of intellectually disabled persons by providing a platform for interaction.
Hydroponics GardeningThis programme is a joint effort with Kuo Chuan Presbyterian Primary School to
cultivate vegetables hydroponically. The students get a chance to interact and
work with our residents, whilst learning the process of farming without soil and
learning about the life cycle of plants and:
from planting of the seeds to harvesting
and selling the vegetables
CIP- Interaction Bishan Home’s policy is to co-operate with schools on their CIP program. We actively support this
programme as it will expose students to the need to be socially responsible and expose them to
leadership roles in planning and executing programmes for our Residents. Also by their interaction
with our residents, they will become more aware of Intellectual Disability and hopefully to gain a better
understanding and knowledge of the disability. Our hope is that they will become our ambassadors to
the community at large.

YMCA : Y Dance Outreach This programme started in 2007, working in conjunction with YMCA Singapore, it involves volunteers
who possess a love for dance and a passion to enrich the lives of our residents. Volunteers perform and
teach simple dance movements during this monthly programme.
Therapy Dogs, SingaporeThis programme started in December 2002. Volunteers of Therapy Dogs Singapore (TDS) are also
strong supporters of GSDC’s PAT (pet-assisted therapy) work. It aims to achieve comfort and to provide
physical, social and emotional therapy to patients, through the comforting presence of, and activities
with, therapy dogs.

Our department provides physiotherapy/exercise treatment to 128 residents and 29 DAC clients in the home. Since 2010, we have been concerned with the ageing profile of our residents. With the help of one of our strategic volunteer partners, UPS, we were able to implement our plan to prepare the Home in providing the required therapy/exercise services and facilities/equipment to cater for an ageing resident population.
Under the plan we contracted Tan Tock Seng Allied Health Services to do an audit of our therapy/exercise capability and based on that audit to upgrade our capacity and programs. The audit began in the first half of 2011 and we are currently in the training program phase which is expected to be completed in May 2012.
The program also included training on how to better understand and manage behaviors and to run more effective therapy/exercise activity which are specifically targeted for the different functioning groups.
Also under the plan, we purchased geriatric chairs and 3 cranked hospital beds to make it more comfortable for the residents who have use the equipment for long periods of time as well as to ensure their safety. In addition we also have a close collaboration with the Singapore Polytechnic Center for Applications in Rehabilitative Engineering (SP Care). Through this collaboration, they have developed and donated exercise equipment to help strengthen and tone major muscle groups eg a Universal Leg Exerciser and a Neck Exerciser for our residents. These exercisers are wheelchair-based, enabling our PT aides to move them to the residents easily.
We strengthened our PT aide team by recruiting an additional aide in July 2011 to provide more personalized therapy and exercises to our residents. The individualized therapy hours dropped in August and October due to a flu outbreak in the home and preparation for our 13th anniversary respectively- see bar chart Figure 1. Besides individual training sessions, we also provide group therapy sessions for each functioning group which comes up to 44 hours a month.
These groups are not as active as the others in lessons and activities; they face more problems as they age. Our objective is to maintain their physical condition, or at least to slow down the decline in their functionality during their stay here.
A Success Story Fanny was referred for physiotherapy exercises due to developing a frozen right shoulder which caused her a lot of discomfort. With that condition, she refused to move her right arm because of the severe pain, depending on her left arm for all her upper body activities. Activities of daily living such as dressing became difficult as her injury and pain limited the movement of her arm.
Our physiotherapists assessed Fanny’s condition, and came up with a therapy program for her. With continuous therapy such as shoulder mobilization exercises that concentrated on the shoulders and upper limbs, she was eventually able to move her shoulders to a functional level. Activities of daily living became easier and she is now able to perform many tasks. The improvement has not only just been physical, Fanny is much happier: smiling more.

Care-Giving for the Special Needs Adults in Bishan Home
1. Bishan Home provides quality care by ensuring balance diet, a range of diversified training programs, including leisure activities, outings to places of special interest in Singapore and sporting sessions and events – for example bowling.
2. Our residential program is effectively full. However in our Day Activity Centre (DAC) program our client enrolment is just under 30 and we have plans to actively increase our client numbers to 40 in the next year. In both our programs all training and activities are based on our residents’ or clients’ functional abilities needs.
3. Promote better general health by collaborating with SATA in the Doctor-On- Wheels services:
Our clients are generally more at risk because of their intellectual disability. They are significantly more at risk than the general population for a wide range of health problems, including obesity, hypertension, hyperlipidaemia, epilepsy, thyroid dysfunction, skin diseases, sensory impairment, visual and hearing impairment. Others with associated physical disability, eg cerebral palsy, are more at risk of postural deformity, pulmonary infections, gastro-intestinal issues etc.

4. Safety of Clients, Prevention of Falls Introduction of fall-prevention beds, geriatric chairs, grip mats, railings at fish pond and sourcing for non-slippery yet comfortable footwear for the residents We are planning to introduce a lecture cum training program on falls – risk assessment, prevention and response. The program will be mounted by our therapy/exercise dept.

5. Staffing – Manpower and Capabilites
A team of ophthalmologists from TTSH came in to conduct eye tests for about 90 clients and some staff. They were tested for common eye conditions, including eye checks to exclude glaucoma.
A mass health screening for all of the residents was conducted in Nov 2011. This was smoothly conducted with the assistance of SATA Mass Health Screening medical team. Basic tests were done for individual clients – CXR in a mobile vehicle, Full Blood Count

In the year under review, we increased our Nursing Aides staff strength and also recruited an additional Therapy Aide. This increase in staffing was accompanied by a greater emphasis on skills training and upgrading. The objective is to increase our service delivery capabilities so as to provide holistic care to our residents and clients which enable them to develop life skills and lead meaningful lives.
During the year our therapy aides, nursing aides and other direct caregivers underwent an exhaustive training program with Tan Tock Seng Hospital’s allied health professionals from the PT and OT departments. The whole training program lasted about 9 months. It involved competency assessments of the PT aides, review of the PT and OT programmes, managing the challenging behaviours, and an environmental scan assessment.
For the financial year ended 31 March 2012 (“FY2012”), Bishan Home for the Intellectually Disabled (“the Home”)
reported income of S$2.87m and expenditure of S$2.44m from its operation, resulting in a net operating surplus of
$0.43m.
The net operating surplus for FY2012 was 75.7% higher than that of the previous financial year. The increase in net
operating surplus was the result of a greater increase in operating income compared to the increase in operating
expenses.
The increase in operating income was derived substantially from an increase in 3 income categories: Government
Subvention, Cash Donations and Deferred Capital Donations.
1. The amount of Government Subvention received was boosted by an increase in the enrolment in the
Home’s Day Activity Centre (“DAC”) as well as an increase in the number of Residents and DAC Clients
qualifying for higher rates of Government Subvention due to a change in the Government’s guidelines for
Government Subvention eligibility.
2. Cash Donations received more than doubled during FY2012 on the back of the Home’s registration on the
SG Gives donation portal in early 2011 in tandem with its improvement of its website and facebook links.
3. Deferred capital donations rose due to substantial Government funding received for cyclical maintenance
projects carried out in 2011 to maintain the 13 year old leasehold property of the Home.
With regard to the operating expenditure of the Home, the fastest rising expenditure was staff costs for both
operations staff as well as staff involved in governance activities, followed by governance costs.
1. Staff costs rose as the Home sought to improve its service delivery through the hiring of more qualified
therapy and nursing staff to help the Home’s Resident population cope with premature ageing issues (due
to their intellectual disability). The average age of the Home’s Residents in FY 2012 was 45, with 35% of the
Home’s Resident population aged 50 and above. Consequently, foreign levy increased by S$0.02m in
FY2012. Staff training cost similarly increased as the Home sought continual improvement in its service
delivery effectiveness.
2. Governance costs (including governance staff costs) also rose to enable the Home to meet the increasingly
stringent governance standards set by the Government. In addition, depreciation of assets acquired
from the cyclical maintenance projects of the Home (classified as governance costs) contributed
significantly to the increase in governance costs.
On the other hand, the Home managed to generate several cost savings through the tight control of other cost items
such as transport and food (with the aid of donations-in-kind) and prudent management of its debtors, resulting in a
reduction in doubtful debt provisions.
