Give examples from your daily life surroundings or social sector. A1) Poka Yoke is useful everywhere and anywhere where there is a chance of something going wrong. For example, a process operation is an area where there is chance of missing a step. It helps eliminate human errors and even mechanical errors to certain extent which in term improves quality of product. The idea is to produce zero defects by utilizing quality tools which prevent human errors. Combining Poka Yoke with Six Sigma helps to achieve precisely that. The approach to mistake proofing can be procedural, mechanical, electrical, human or any other form of prevention. Having a checklist to ensure all steps are undertaken in a sequence can help. You should test the mistake-proofing device and ensure that the user is also aware and trained to u se and review it to achieve zero defects. Error elimination need not be a co mplicated process. There are simple solutions to small mistakes which can otherwise mean disaster for the product. Combines with Six Sigma, mistake- proofing ensures elimination of defects in the first place. Example of Poka Yoke in daily life could be: • One example is the small hole in almost every bathroom sink. It is a poka yoke to help prevent sink overflow. • Another example is in our cars that have automatic transmissions. You can not remove the car key while the transmission is in an unsafe position and the wheels are unlocked. The key interlock in this case is a Safety poka yoke.
