この制度は、独立行政法人日本スポーツ振興センター法に基づき、国・学校の設置者(名古屋市)・
保護者の三者が経費負担する互助共済制度で、お子様が学校でけが等をした場合、災害共済給付を
受けることができます。この制度の概要は以下のとおりです。名古屋市立学校では、児童生徒等総
数の 99.7%の方が加入しています。ご理解の上、ご加入いただきますようお願いいたします。
<日本スポーツ振興センター災害共済給付制度の概要について>

National Agency for the Advancement of Sports and Health (NAASH) Mutual Aid Accident Compensation System Membership
The NAASH Mutual Aid Accident Compensation System, based on the National Agency for the Advancement of Sports and Health Law (NAASH Law), is a mutual aid system wherein you may receive accident compensation benefits in the event that your child sustains injury at school. Expenses are shared by the three parties of the national government, the school’s administrator (the City of Nagoya), and the parent or guardian. We ask for your understanding of the following brief overview of the system before enrollment.

Overview
Eligibility for compensation
Injury, etc. sustained while under care of school management during year enrolled in system (transferring schools is permitted) * Under care of school management refers to circumstances where your child is in class, on break, participating in club activities, studying off-campus, and/or commuting to or from school, etc.
Types of compensation
Medical expenses * If total medical expenses where national health insurance is used are equal to or greater than ¥5,000 by the time treatment ends, you are eligible to receive compensation for the co-payment (normally 30% of the total) + 10% of the total amount. Disability consolation payments * Compensation will be paid according to the extent of damage remaining (including death). * Please refer to accompanying document entitled “Mutual Aid Accident Compensation System Notice.”
Compensation period A maximum of 10 years from initial examination (for the same accident)
Premium Annual amount: borne by parent/guardian: ¥___________ borne by the city: ¥__________ (per child)

Scenario 1 - While students are AT school: We will immediately cease all school lessons/activities and students will proceed home using the methods/routes which have already been agreed upon by parents and teachers. The school will remain closed until further notice. The school will notify parents as to when it is safe to resume classes.
Scenario 2 - Traveling TO or FROM school: Students will continue directly to school or home. In the case of traveling TO school, we will follow the measure in scenario 1. The students will then proceed home using the methods/routes which have already been agreed upon by parents and teachers.
Scenario 3 - Students are currently AT home: The school will remain closed until further notice. The school will notify parents as to when it is safe to resume classes.
