Wound Care Wound Cleansing Goal : To get rid of• Bacteria• Necrotic slough

Wound Cleansing Solutions/AgentsCleanse wounds with low-toxicity solutions such as: • normal saline

• Tap water • Topical antiseptic solutions should be reserved for wounds that are nonhealable or those in which the local bacterial burden is of greater concern than the stimulation of healing. (Level of Evidence: III)

• วัสดุปิดแผลที่รักษาความชุ่มชืน้ (Moisture Retentive Dressings )

• ความชุ่มชืน้ที่เหมาะสมจะช่วยให้กระบวนการหายของแผลดาํเนินไปอย่างรวดเร็ว ดงันัน้ วัสดุปิดแผลที่ด ีจงึต้องสามารถควบคุมความชุ่มชืน้ให้แผลได้ ความชืน้จากแผลจะระเหยออกได้ช้าลง (lower moisture vapor transmission rate  ) จะทาํให้สิ่งขับหลั่งจากแผลที่มี enzymes ต่าง ๆ , growth factors รวมทัง้เมด็เลือดขาวชนิด neutrophils และ macrophages คงอยู่บริเวณพืน้ผิวของแผลได้นานขึน้ จะช่วยกระตุ้นกระบวนการหายของแผลในระยะงอกขยายได้ดขีึน้ ในกรณีที่พืน้ผิวของแผลที่มีเนือ้ตาย (slough tissue หรือ necrotic tissue)

• ถ้าวัสดุปิดแผลนัน้สามารถตดิอยู่ได้นาน 72-96 ชั่วโมง จะช่วยให้กระบวนการ autolytic debridement มีประสิทธิภาพด ีนอกจากนี ้Moisture  Retentive Dressings ยังช่วยกระตุ้นการเกดิ granulation tissue การสังเคราะห์ collagen และการงอกของEpithelialization

อีกด้วย

Modern Wound Dressing 

ตวัอย่างแผลใช้ Modern Dressing เปลี่ยนทกุ3-5วัน

• จากรายงานของ Hutchinson และ Bolton พบว่า ในรายงาน 70 เรื่อง การใช้วัสดุปิดแผลแบบปิดกัน้ไม่ให้สารผ่าน Occlusive Dressing มีอัตราการตดิเชือ้ร้อยละ 2.6 เมื่อเทยีบกับการปิดแผลโดยใช้ Gauze Dressing ซึ่งพบอัตราการตดิเชือ้ร้อยละ 7.1 สรุปได้ว่า อาจเกดิจากปัจจัย ดงัต่อไปนีค้ือ

• 1.วัสดุปิดแผลแบบ Occlusive Dressing ช่วยป้องกันแบคทเีรียเข้าสู่แผล โดยเฉพาะปกป้องไม่ให้ปัสสาวะและอุจจาระสัมผัสกับแผล ในกรณีที่มีแผลกดทบับริเวณก้น

• 2. จาํนวนครัง้ของการเปลี่ยนแผลลดลง ช่วยลดการเกดิ Cross‐contamination

• 3. ควบคุมเมด็เลือดขาวชนิด neutrophils และ macrophages คงไว้บริเวณพืน้ผิวของแผล ช่วยให้เกดิการแบ่งตวั และการทาํงานของเมด็เลือดขาวได้ดขีึน้

• 4. ช่วยรักษา Endogenous enzymes ไว้ในแผล ช่วยกระตุ้นให้เกดิกระบวนการ autolyticdebridement ทาํให้พวกเนือ้ตายต่าง ๆ หลุดออกได้ง่ายขึน้ ไม่เป็นแหล่งอาหารของแบคทเีรีย ซึ่งอาจนํามาซึ่งการตดิเชือ้ได้

Modern Wound Dressing 

• การแบ่งประเภทของวัสดุปิดแผล

• Primary Dressing คือ วัสดุปิดแผลที่สัมผัสกับพืน้ผิวของแผล บางครัง้อาจจะเรียกว่า Contact layer

• Secondary Dressing คือ วัสดุปิดแผลที่ปิดทบั Primary Dressing อีกชัน้หนึ่ง ป้องกันสิ่งปนเปื้อนต่าง ๆ และช่วยในการดดูซับปริมาณสิ่งขับหลั่ง

• ในวัสดุปิดแผลบางชนิดที่เป็น Primary Dressing สามารถตดิกับผิวหนังรอบแผลได้เลย กไ็ม่จาํเป็นจะต้องใช้ Secondary Dressing เช่น แผ่นปิดแผล Hydrocolliod

Principle of Wound Management and Wound Bed Preparation

การเตรียมพืน้ของแผล (wound bed preparation)

• การจดัการผู้ป่วยที่มีบาดแผลเรือ้รังนัน้ แนวคดิเกี่ยวกับการเตรียม

พืน้ของแผล (wound bed preparation) เป็นแนวคดิที่มีความสาํคัญในการเตรียมความพร้อมของแผลให้หายอย่างมี

ประสิทธิภาพ (Falanga, 2003; Hess & Kirsner, 2003) การเตรียมพืน้ของแผล หมายถงึ การจดัการแผลอย่างครอบคลุม

โดยเน้นการเตรียมสภาพพืน้ของแผลเป็นอย่างด ีเพื่อส่งเสริมให้เกดิ

กระบวนการหายของแผลตามปกต ิ(Schultz, Mozingo, Romanelli, & Claxton, 2005) ถ้าไม่มีการเตรียมพืน้แผลที่ดไีม่ว่าการใช้ผลิตภณัฑ์ใดในการดแูลแผลกไ็ม่สามารถช่วยให้แผล

เรือ้รังหายได้อย่างเหมาะสม (Falanga, 2003)

Wound bed preparation• หมายถงึ การทาํให้บาดแผลของผู้ป่วย มกัเป็นบาดแผลเรื้อรัง มสีภาพของ

บาดแผลและสภาพแวดล้อมของบาดแผลเหมาะสมสําหรับการหายของแผล คอื

บาดแผลทีพ่ืน้ผวิ(wound bed)ทีแ่ขง็แรง เลอืดและออกซิเจนมาเลีย้งมากพอ

สําหรับการหายของแผล ไม่มหีรือมนีํา้เหลอืง(exudate)เพยีงเลก็น้อย และไม่มี

เชื้อแบคทเีรียในปริมาณและความรุนแรงมากพอในการทาํลายหรือขดัขวางการ

หายของแผล (Wound bed preparation is the management of the wound to

accelerate endogenous healing or to facilitate the effectiveness of other

therapeutic measures)

Wound bed preparation (WBP)

Component of WBP ประกอบด้วย

1.Management of necrosis (T)

• 2.Restoration of bacterial balance (I)

• 3. Management of exudate (M)

• 4.Correction of cellular dysfunction (E)

• 5. Restoration of biochemical

balance(E)(Ruth A.Bryant, 2000)

Wound bed preparation( WBP)• การเตรียมพืน้ของแผล (wound bed preparation)• การจดัการกับแผลมีแนวทางจากลักษณะพยาธิสภาพของแผลที่สาํคัญ 4 อย่าง เรียกย่อ

ว่า “TIME” (Templeton, 2004; Schultz, Mozingo, Romanelli, & Claxton, 2005)

• “T” หมายถงึ เนือ้เยื่อ (Tissue) ที่ใช้การไม่ได้หรือไม่เพยีงพอต่อการหายของแผล การจดัการกับเนือ้เยื่อ ที่เป็นเนือ้ตายทาํให้แผลหายช้าลง (Beitz, 2005) โดยการตดัเนือ้ตาย (Debridement technique)

• “I” หมายถงึ การตดิเชือ้และการอักเสบ (Infection and Inflammation) การจดัการ คือ

การควบคุมการตดิเชือ้และการอักเสบ (Bacterial Management)

• “M” หมายถงึ ความชุ่มชืน้ของแผลที่ไม่สมดุล (Moisture imbalance) การจดัการ คือ

การสร้างความสมดุลความชุ่มชืน้ของแผล(Exudate management)

• “E” หมายถงึ ขอบแผลที่ไม่เจริญ (Edge of wound) การจดัการ กับขอบแผล คือกระตุ้น

ให้ขอบแผลเจริญเตบิโตได้อย่างมีประสิทธิภาพ ได้แก่ การใช้แรงดดูสุญญากาศ (NPWT) หรือ การใช้ ผลิตภนัฑ์เช่น growth factor/ Honey- ต่างๆ

Autolytic debridement• วธิีการย่อยทาํลายตัวเองโดยสิ่งคัดหลั่งจากแผล (autolytic) ได้แก่

การใช้เอนไซม์จากสิ่งคัดหลั่งจากแผลเป็นตัวทาํให้เนือ้ตายอ่อนตัว อาจใช้ Hydrocolloids เพื่อช่วยให้ความชุ่มชืน้กับแผล Hydrogel ช่วยให้นํา้แทรกเข้าไปในเนือ้ตายให้อ่อนตัวลง Alginates, Foam, Hydrofiber หรือ moisture vapor permeable ต่าง ๆ ใช้ในการดูดซมึสิ่งคัดหลั่งจากแผลที่มากเกนิไปช่วยให้เกดิความชุ่มชืน้ที่เหมาะสมกับแผล ผู้ป่วยจะรู้สึกสบายขึน้ ไม่เจบ็ปวดแผลเมื่อทาํแผล

• ข้อเสียคือการทาํงานค่อนข้างใช้เวลา ควรระวังเรื่องการตดิเชือ้ ต้องประเมนิการไหลเวียนเลือดสู่ส่วนปลาย ห้ามใช้ในแผลตดิเชือ้ แผล cellulitis แผลที่ลึกมากเกนิไป ผู้ที่มีภาวะเมด็เลือดขาวตํ่าอย่างรุนแรง กรณีที่ใช้ผลิตภณัฑ์ที่มีคุณสมบัตเิป็นตัวดูดซับต้องระวังไม่ให้พืน้แผลแห้งเกนิไปจะทาํให้แผลหายช้าได้

Vacuum-Assisted Closure

• การใช้ความดนัตํ่ากว่าบรรยากาศทาํให้เกดิการกระตุ้นของการขับของinterstitial fluid จากบริเวณเนือ้เยื่อรอบๆแผลทนัที

• เป็นการลดแรงกดของmicrovascular และมีการไหลเวียนได้เพิ่มขึน้

Exudate บางส่วนยังคงหลงเหลืออยู่ทาํให้เกดิความชุ่มชืน้กับพืน้ผิว การที่มีเลือดมาเลีย้งและนําพาออกซเิจนมาด้วยส่งผลให้เกดิการเพิ่มของ granulation tissue

• การหายของแผล การจดัการ คอืกระตุ้นให้ขอบแผลเจริญเตบิโตได้อย่างมปีระสิทธิภาพ

เช่น การใช้แรงดูดสุญญากาศ(NPWT)หรือการใช้ ผลติภนัฑ์ growth factor/ Honey- ต่างๆ

เจลนํา้ผึง้เมลลาเดมิ(Melladerm Plus ):ทาํให้แผลหายเร็วขึน้และทาํหน้าทีย่บัยั้งต่อต้านและ

ทาํลายเชื้อโรคได้ ตวัอย่าง แผลBurn ทีข่าทาํแผลมา 1 เดอืนไม่มคีวามก้าวหน้าการหาย ใช้

Melladerm Plusทาแผลเปลีย่นได้ทุก1-2 วนั แผลหายใน2อาทติย์

• Charcot’s Foot เป็นลักษณะหนึ่งที่พบได้ในผู้ป่วยที่เป็นโรคเบาหวาน สาเหตุเกติ

จากการที่ กล้ามเนือ้ Gastrocnemies ดงึกระดกูเท้า Calcaneous ทาํให้รูปเท้าบดิเสียรูปไป ถูกพบโดย นายแพทย์ชาวฝรั่งเศษที่ชื่อ Jean-Martin Charcot

เป็นผู้ให้คาํจาํกัดความไว้

• นอนพกั เลี่ยงการกดทบัแผล Moisture skin Off loading: felted foam, total contact cast, proper footwear

• Debridement : สิ่งแปลกปลอมสิ่งสกปรก และ พงัผืด (callosity removal)

• Dressing :Non‐adherent,NSS,Calaium alginate,Hydrocolliods, Foams,

• แก้ไขจุดกดทบัจากปุ่มกระดกูที่ยื่นออกมาผิดปกตจิากเท้าผิดรูป

• มาตรการสาํคัญ : ป้องกันการเกดิแผลซํา้ใหม่(recurrent ulcer) และตดิเชือ้

‐ Minor Leg Amputation : เช่น

(การตดัเท้าโดยเหลือเฉพาะส่วนของส้นเท้า)

Ischaemic Foot Ulcer :ใช้ Modern Dressing ถ้ามภีาวะตดิเชื้อใช้ silver coated

dressing เปลีย่นทุก3-5 วนั ประเมนิ Granulation Tissue

ตวัอย่างแผลใช้ Modern Dressing: Hydrogel, Hydrocolloid,Foam,Transorbant

เปลีย่นทุก3-5วนั ประเมนิ Granulation Tissue ห้ามพนัแผลแน่น

Infective UlcerPathology:  Superficial abscess

อักเสบบริเวณผิวหนังและชัน้ไขมัน

: Cellulitis > 2 cm should admit

อักเสบถงึทางเดนินํา้เหลือง บวมแดงรอบแผล

: Fasciitisอักเสบถงึชัน้พงัผืดที่หุ้มกล้ามเนือ้

: Necrotizing Fasciitisชัน้พงัผืดถูกหนองกัดเซาะทาํลาย

:Deep Abscess     Septicemiaชัน้กล้ามเนือ้ถูกหนองกัดเซาะทาํลาย เกดิโพรงหนอง

:Chronic osteomyelitisอักเสบถงึชัน้กระดกู

เชื้อโรคทีพ่บบ่อยใน Infective Ulcer• Gram positive bacteria:Staphyllococcus,Streptococcus• Gram negative bacteria  E. Coli, 

Klebsiella,Proteus• Anaerobic bacteriaBacteroides,ClostridiumPeptostreptococcus

Empiric ABO should cover all gram + cocci, gram – bacilli and anarobe: clindamycin + ciprofloxacin or ertapenamDuration: skin/subcut 14 Day, deeper 3-4 wks

Biofilm  Management in Infective Foot Ulcer• กรณีแผลไม่มีความก้าวหน้าการหาย อาจตดิเชือ้Biofilm  ซึ่งส่งผล

เกดิการสร้าง fibrin slough และ exudate มากขึน้ จงึทาํให้แผลนัน้หายช้า และการขจดัการตดิเชือ้จงึต้องเพิ่มปริมาณการใช้

Antimicrobial Drug มากขึน้ จงึเป็นเหตุให้เชือ้นัน้ดือ้ยา

ดงันัน้การขจดั Biofilm ปัจจุบัน ที่นิยม ใช้ เช่น

: Pontosan solution  ซึ่งใช้ในแผลตดิเชือ้เรือ้รัง หายช้าและ แผลNecrotizing  Fascitis ที่ขาได้ผลด ี

: Aquqcell AG Plus/  Extra   ปิดแผล หรือใส่ในโพรงแผล เปลี่ยนแผลได้ทุก3วัน

: Iodoflex dressing 

DM Infective Ulcer : แผลตดิเชื้อ• ลักษณะแผล

: แผลมีการอักเสบทัง้เฉียบพลันและเรือ้รัง : แผลมีบวม แดงร้อน กดเจบ็และอาจมีหนอง

• อาการเจบ็ป่วย

:ปวดและมีไข้ พบในแผลที่อักเสบเฉียบพลัน

• การตรวจร่างกาย

:ซมึลง/Vital signเ ปลี่ยน เมื่อตดิเชือ้รุนแรง

: บริเวณเท้าและน่องบวมตงึ กดเจบ็ เมื่อการ

ตดิเชือ้ลุกลาม

• การตรวจพเิศษ

: ตรวจหาชนิดของเชือ้โรคเพื่อให้ยาปฏชิีวนะ: เพาะเชือ้แบคทเีรียทัง้ที่แผลและในเลือด: ถ่ายภาพรังสีที่เท้าเพื่อตรวจ Osteomyelitis

Management  in Infective Foot Ulcerการรักษาขึน้กับความรุนแรงของการตดิเชือ้

การตดิเชือ้ไม่รุนแรง : อักเสบบริเวณผิวหนัง

และชัน้ไขมัน(Superficial abscess)

• Debridementเนือ้ที่ตดิเชือ้และระบายหนอง

• Swab culture

• ให้ยาปฏชิีวนะตามเชือ้

• ควบคุมเบาหวานระวังนํา้ตาลสูง

• Wet Dressing :Non-adherent, NSS

• silver coated dressing or silver sulfadiazine

(not povidine, hydrogen peroxide, acetic acid)

• ใช้ Ideal Dressing ต้องเลือก silver coated dressing ที่ใช้กับแผลตดิเชือ้ได้ เช่น Acticoat,Aquacel Ag,Calgitrol Ag AskinaAg,Tegaderm Ag, Allevyn Ag

Septicemia,Chronic osteomyelitis

- Pus culture,Haemoculture

-ให้ยาปฏชิีวนะชนิดฉีดทัง้ Grampositive/

negative/Anaerobic

- ควบคุมเบาหวาน ใช้ Insulin ฉีด

- Debridement/ระบายหนอง

-Wet Dressing :Non-adherent,NSS

AskinaAg,Tegaderm Ag, Allevyn Ag

-ประเมนิก้นแผลและการอุดตนัของหลอดเลือด

เพื่อผ่าตดัหลอดเลือดหรือการตดัขา

Osteomyelitis

• Dx: visible bone and palpable by probing positive predictive value 89%

• Plain film: destructive lesion: 30-50%

• Bone scan, WBC scan and MRI

• Rx: resection infected bone and ABO 2 week

• ABO alone 6 weeks: high rate of remission

แผลตดิเชื้อเรื้อรังใช้ Pontosan solution ชุบ gauze pack ทัว่แผล 15 นาท ี

ปิดแผล silver coated dressing (Calgitrol Ag)เปลีย่นแผลทุก3วนั และ ใส่ Pontosan solution

ด้วยวธิีการเดมิ ทุกครั้งทีเ่ปลีย่นแผล

แผลNecrotizing Fascitis ใช้ Pontosan solution ชุบ gauze pack ทัว่แผล 15 นาท ีแล้วนําทิง้

ปิดด้วย gauzeใส่ Pontosan solution ด้วยวธิีการเดมิ ทุกครั้งทีท่าํแผล ด้วย NSS sol

แผลใช้ Modern Dressing เปลี่ยนทกุ3-5 วัน

Escharotomy debridement :

• คอื การกรีดเนือ้ตาย(Necrotic Tissue ) ให้แยกออก ทา ด้วย Silver sulfadiazine (Ag SD) cream ปิดทบัด้วย Gauze ชุบ normal saline ทิง้ไว้24-48 ชม. เปิดแผลออก พบเนือ้ตายแผลเปื่อยยุ่ย ทาํการคบีเนือ้ตายทีเ่ปื่อยยุ่ย ออกเพือ่ให้เห็นก้นแผล

• ต้องประเมนิก้นแผล(wound bwd)ทุก

ครั้งเพือ่ระวงัการเลอืดและออกซิเจนมาเลีย้งบาดแผลเพราะ Silver sulfadiazine (Ag SD) cream อาจปิดกั้นการเข้าออกของออกซิเจน
