An intensive “pilot program” was developed to support homeless individuals in moving forward and out of homelessness (July, 2014 to October 2014).

Address co-occurring conditions through a comprehensive “triage/wrap services” model that will include strong community programing and peer engagement.

• Six (6) part-time contractual Outreach Peer Specialists.

• The Outreach Peer Specialists are formerly homeless individuals encouraging service engagement and providing community support services.

The Outreach Peer Specialists work flexible hours (early mornings, late nights, and weekends) partnering with VBPD/2nd Precinct, HSD/Projects for Assistance in Transition from Homelessness (PATH), and HNP to “triage” individuals moving them into services.

• Outreach Peer Specialists report to the PATH Supervisor daily on the progress and activities on the boardwalk

• Weekly cadence calls held to review data and coordinate services

• Increased coordination between the PATH Supervisor and the Second Precinct Crime Prevention Unit Supervisor has enhanced communication with the business community
