1.0 EMERGENCY PLAN1.1 AIM AND AUTHORITYThe aim of the plan is to improve in detail the control, co-ordination and response to any emergency occurrence within the prescribed area which is, or could be, a threat to life and property or the environment. Because every threat / emergency is different this plan should only be used as a guide. Consideration of most elements of a threat / emergency needs to be made before any action is put in place. 1.2 SITE DESCRIPTION 1.2.1 General Description The Van Oord Thai Limited.site comprises a LNG terminal, workshops and warehouse; also contractor facilities and administration facilities. The government centre of Rayong province is located five (7) kilometres north-east of the construction site

1.4 EMERGENCY RESPONSE ANALYSISCompiling the List of Emergency EventsWhen considering the emergency events that could possibly impact the Van Oord Thai Limited. construction and necessitate an emergency response the following list of identified loss exposures was put together. Because Van Oord thai Limited.is the only construction of its type and has limited emergency response resources it has been decided to utilise the government emergency services. It was also decided to include Van Oord Thai Limited.emergency responses where possible to aid the local government in community emergency events.1. Accident/ injury to employees,

The Van Oord thai Limited.Emergency Response Management Team then considered and evaluated the risks to highlight the emergency events that should be formatted into the response manual.

Risk Assessment of Emergency EventsThe risk assessment used in this case to prioritise the most severe type of emergency event is called the Potential Problem Analysis (PPA) system. It is based on looking at Personnel, Production Infrastructure or Equipment, Regulatory Approval, Environment, Corporate Image or Utility and Likelihood of Occurrence using a lettering system of 1, 2, 3, and 4 with 1 being the extreme end of the scale and N being the negligible end of the scale.

The list of 6 emergency events was prioritised by considering the PERSONNEL factor and the LIKELIHOOD of OCCURRENCE factor in the Potential Problem Analysis (PPA) system. If one or both of these areas showed an EXTREME (E), then that emergency event was given more priority over other emergency events. After a (E) rating, a rating of (H) was considered the next priority.It was also decided to put certain emergency events together to form one section within the manual. For example all fires were collated into one section; all types of equipment and vehicle accidents into one section and Parts of civil unrest covers arm do not have a section in the manual but are handled as an emergency if and when they should occur. On distribution to manual holders a memorandum was issued with each manual explaining the manual and for training to begin with the work groups to understand the sections within the manual.This was to present the manual at monthly safety meetings using sections as topics for those meetings. It is envisaged that work group Supervisors carry out practice drills/ training eg evacuations regularly, the Emergency Response Team conduct emergency scenarios/ training in and about the construction site at their training sessions and management carry out an annual mine site scenario.

1.5 IDENTIFICATION OF REQUIREMENTSIdentification of requirements

The following functions are requested to counter effects of a threatening event:

ControlDecisions for actions are made by the Emergency Response Management Team or by individual team members if and when a quick decision / action is required.The HSE Manager carries the responsibility for co-ordinating actions including other organisations in accordance with the needs of the situation.The Project Manager will assist and advise the HSE Manager as necessary.

1.7 ALLOCATION OF RESPONSIBILITIES1.7.1 IntroductionThis section details the roles and responsibilities of certain key members of the Team, other staff and key agencies available to assist in an emergency.

1.7.2 Project ManagerHas overall control of personnel and resources.

1.7.3 HSE Manager / Safety Officer Works with Project Manager. Calls for assistance from outside

organisations. Disseminates warnings and information

as required. Initiates appropriate action to minimise

the effects of injury at the accident scene and damage to property.

1.7.4 Emergency Response Team Leader Responds to initial emergency call. Organises the remainder of the team. Liases with the safety/medical

coordinator for additional services. Provides first aid and rescue services in

consultation with the qualify First-aider. Ensures that emergency rescue and

support equipment is maintained in readiness.

All emergency equipment as listed in Appendix 2, will be located in the HSE Office. The Safety officer will maintain emergency equipment.

1.7.6 Department ManagersIn an emergency each Department Manager or his alternate will check his/her own area of responsibility. If the emergency has arisen in the area they will remain there and advise on the following:

1.7.7 Emergency Response TeamThe Emergency Response Team will report to the Emergency Response Team Leader. The Safety officer will report to the scene of the emergency and stand by to treat the injured and assist as necessary when advised by the Emergency Response Team Leader or Safety / Medical Coordinator.

1.7.8 Other Supervisory Staff and EmployeesAll other supervisory staff and employees will remain at their workstations unless directed otherwise.

Each supervisor will ensure that all members of his work group are accounted for.

1.8 ACTIVATION OF THE PLAN AND EBRIEFING

1.8.1 Stages of ActivationAfter the emergency response has been initiated, the Safety officer will advise on:

A limited response only is necessary in the area concerned such as in the case of fire or injured persons.

OR A complete mobilisation of resources.

1.8.3 BriefingThe Project Manager or his alternate will ensure that personnel are adequately briefed both during and after the emergency.

1.8.4 DebriefingThe Project Manager or his alternate is responsible for calling together the Emergency Response Management Team, plus other key people involved, as soon as possible after the event for the purpose of investigating what happened and what modifications, if any, should be made to the plan for the future.Statutory reports to the Mab taphut Industrial Estate Authority of Thailand (IEAT)and other Government bodies are the responsibility of the Project Manager or his alternate.

1.9 SPECIFIC EMERGENCY CIRCUMSTANCESWhere ever a person (litle) is designated in these procedures to carry out duties is offsite; then those duites become the responsibirity of the person acting in that position or otherwise given to a person nominated by a department manager.

There may be times in an emergency where people will be asked to carry out emergency duites by others who are not their regular supervisor. For example The HSE Manager / Safety officer, these people should be cooperated with, however the right to refuse work without reprisal is still valid.There may be in some cases of an emergency response where time is extreamly important and Van Oord Thai Limited.Safety Rules may not adhered with. This will become a judgement made by those involved in the emergency response with all due care and consideration in making a safe decision for the circumstances at the time to save life and property.

2.2.2 Action by Safety Officer On receipt of a call advising of an injured person gather as much information as possible depending on the severity of the injuries and notify the following personnel: Emergency Response Team Leader or

Alternate Safety ManagerProceed to the accident scene with the Ambulance. On arrival assess the situation and if

further assistance is required mobilise the Emergency Response Team if not already done.

Commence treatment of the injured person / persons.

The First Aid Centre should be manned at all times during an emergency situation by competent trained staff. (if possible)

2.3 MEDICAL EVACUATION2.3.1 First-aider After assessing if the Medical Evacuation is necessary, the First-aider will:

1. Ensure the receiving hospital is notified of the details for the Medical Evacuation.

2. Arrange for the transportation of the patient from the First Aid Centre to the hospital.

3. Arrange for medical staff, trained in life support and critical care to accompany patient, if possible.

4. Ensure all relevant documents are dispatched with the patient.

5. Ensure appropriate medical monitoring equipment, medication and therapy equipment is available to accompany the patient, if possible.

6. If possible, speak directly to the receiving facility’s Medical Officer.

3.0 FIRES - GENERAL PROCEDURES3.0.1 SCOPE & PURPOSEFires have the potential to harm personnel and to cause significant damage and disruption to Van Oord thai Limited. The impact of such an occurrence can be dramatically reduced if personnel on site have a comprehensive knowledge of the procedures that must be followed in the event of a fire outbreak. This section of the Emergency Response Management Plan pertaining to fires is divided into two parts, they are;

3.0 -- 3.2 GENERAL PROCEDURES; This part lists in specific details the roles and responsibilities of all personnel who may be affected by the outbreak of fire in their work area. The information outlined within this part of the plan should be studied and disseminated prior to the occurrence of an incident.

3.2 -- 3.11 IMMEDIATE ACTION SECTION; This part provides a ready reference that is to be used by personnel that may be involved in the control and containment of fire within the work place. A copy of this section should be kept as a “Stand Alone” document in all work areas, and all personnel should receive instruction on how it is to be used.

3.1 ACTION TO BE TAKEN3.1.1 ACTION BY FIRST PERSON ON SCENEIn the event of a fire, the first person on the scene

should take the following action:a) Immediately advise all other workers in the area that there is a fire.b) Attempt to extinguish the fire using the appropriate extinguishing agent.c) Raise the alarm as per section 3.0.3. d) If the fire becomes too large or the evacuation warning sounds, go to your nearest Emergency Assembly Point. Close all doors and windows when leaving rooms.e) Do not enter or re-enter a burning building or structure.f) Remain at the Emergency Assembly Point until the all clear is given.

3.1.2 ACTION BY SAFETY OFFICER / FIRST AIDER

a) On receipt of a fire call, the Safety Officer / First aider shall gather as much information as possible. then notify the following personnel: Emergency Response Team Leader of alternate Work Manager HSE Manager

b) The Safety Officer / will then proceed to the affected area with the Emergency Response Team. Any response shall include the Water Truck and referral vehicle.

NOTE: The First Aid Centre should be manned at all times (where possible) during any emergency situation by trained staff.

5.1.5 ACTION BY DEPARTMENT SUPERVISORThe actions required by the Department Supervisor will vary, depending on the nature of the incident. If the incident is within the constructing area or areas directly under the responsibility of the contractor, the Department Supervisor will act as the Senior company officer until relieved by a person of higher authority. Elements of the response that involve the commitment of the Emergency Response Team shall at all times

3. Don’t expose personnel: all unnecessary personnel should be kept away from the area. Persons combating the fire should not be exposed directly to any possible blast.

4. The situation is unpredictable. Often it will not be known when a tyre is likely to explode and whether other tyres are likely to follow it.

5. Deflated tyres may explode. It cannot be assumed that tyres from which the pressure has been released are incapable of exploding. If the pressure is being released (e.g. due to a fire having melted the valve extensions) during the pyrolysis process then some of the vapours generated may be expelled and this will slow the buildup of vapours to the explosive concentration and possibly prevent an explosion. However, such a mechanism has not been proven to exist and should not be relied upon. Also, it would be extremely dangerous to have anyone attempt to deflate the tyres if an explosion was thought imminent and this should not be done.

6. Isolate the vehicle for 24 hours after removal of the source of heat thought likely to lead to an explosion. An emergency crew should remain in attendance during this period. From experience to date, it appears safe to approach the vehicle after this period. Operators must be trained to park a suspect vehicle only in designated areas.

4.0 EVACUATION GENERAL PROCEDURES4.1 SCOPE AND PURPOSEThere are several possible conditions that may lead to the need to evacuate an area or an entire site. These could include, but are not restricted to:

Fire Explosion Major Chemical spill

It is imperative that employees, contractors, and visitors have an understanding of the evacuation procedures for areas in which they are working or visiting. These procedures should be discussed during site general induction, visitor induction and at regular safety meetings.

IMMEDIATE ACTIONS These sections explain the immediate steps to be taken by employees, contractors and visitors as to the Evacuation Procedures for their designated work. Each section can be summarised as a step by step guideline and should be displayed in work areas to guide employees, contractors and visitors during an evacuation. All personnel should receive regular instruction concerning these guidelines.

4.2 PRIORITIESThe first priority is to be directed towards saving lives, company property, the environment and the community. It is important that when evacuating an area, it should be made without delay, giving occupants the best chance of safely clearing the area.

After all personnel have been accounted for efforts should be directed towards preserving company property.At the discretion of Management, tasking may be directed towards restoring the property to a normal working condition after all risks to personnel have been negated and an investigation commenced.

4.3 EVACUATION GUIDELINESWhen evacuating work the following points will assist in getting people to designated emergency assembly point safely.

Be familiar with the emergency assembly points associated with the work. When working in an unfamiliar area, find out location of the relevant emergency assembly point, and the evacuation procedures for the area.

Know the location of the fire fighting equipment in the work; this includes fire extinguishers, fire hydrants and hoses. Learn how to manually activate suppression systems.

Conditions within a fire effected building can deteriorate rapidly, be decisive when there is a need to evacuate; don’t hesitate even if conditions still appear to be good.

When evacuating work, try to close doors and windows, this will help to prevent fire from spreading quickly. Where possible shut down any electrical appliances.

When evacuating move at a quick pace. Fire can produce thick smoke; try to move through smoke by staying low where the freshest air is close to the floor.

To pass through doors, feel the door to see if it is hot, open doors slowly and look around to see if there is fire behind it. Remember to close all doors after passing through.

Never re-enter an area that has been evacuated until the all clear has been given.

At the emergency assembly point report to Supervisor, Fire Warden, or Department Head. Advise if any persons are missing.

6.2 MINE EVACUATIONGENERAL CONSIDERATIONSIt is not likely the mine workings would need to be evacuated however, it may be necessary to evacuate some areas of the pit and restrict movement for a number of reasons. These include;

Heavy rains or flooding Potential for an uncontrolled blast

Major slip or rock fall occurring or warnings that significant movement has been detected.

Fire on mobile equipment which may create hazardous situation.

Immediate actions would rest with the people at the scene. Restricted movement in areas of the mine would be the Mine Supervisor in consultation with Mine Engineering staff and the Mine Manager who will decide on the appropriate action.

6.3 PROCESS EVACUATION PROCEDURESGENERAL CONSIDERATIONSIt may be necessary to evacuate the Process Area for a number of reasons;

Fire or Explosion Gas Leak Chemical Spill

Fires, gas leaks, and chemical spills have the potential to create life threatening conditions. All evacuations should be expedient to the nominated emergency assembly point.

Ensure all personnel have evacuated the building by doing a quick search of offices and toilets if possible. (Do not place yourself at risk trying to search the building).

Ensure a head count is carried out of all personnel at the emergency assembly point and advise the Emergency Response Team Leader of any missing personnel.

It is unlikely that an incident in either of these areas will impact on the other. However, it may be necessary to evacuate fire and or explosion.The emergency assembly point is:

6.7.1 ASSAY / METALLURGY LABORATORY BUILDING EVACUATION PROCEDURE

The relevant Fire Warden or any person finding an emergency situation will activate the evacuation alarm.

Raise the alarm via phone 0-5661-4141 or 0-5661-4500 ext. 1021 or radio channel 70, and advise the Security/First Aid that the Assay / Metallurgy Laboratory building are to be evacuated.

All personnel working within either the Assay / Metallurgy Laboratory building are to evacuate the building.

If possible switch of all electrical equipment when leaving the work area.

Close all doors and windows if possible. Follow the instructions of the Fire Wardens.

Proceed to the designated emergency assembly point. All personnel are to remain at the emergency assembly

point until advised of the all clear. The Fire Warden will arrange for a head count of

personnel at the emergency assembly point and advise the Emergency Response Team Leader of any missing

Proceed to the emergency assembly point for Assay / Metallurgy Laboratory.

Response to include mine water truck and ambulance. Liase with the relevant Fire Warden and Emergency

The First Aid Centre is to be manned at all times (if possible) during an Emergency situation by trained medical personnel.

6.8.1 EXPLOSIVES FACILITIES EVACUATION PROCEDURES The person discovering an emergency situation will raise

the alarm via phone 0-5661-4141 or 0-56614500 ext. 1021 or radio channel 70, and advise Safety Officer / Nurse of the Explosives Facilities evacuation.

Advise employees in the area as quickly as possible to evacuate. ONLY TRY TO EXTINGUISH A SMALL FIRE IF THERE IS ABSOLUTELY NO IMMEDIATE DANGER.

All personnel working within the Explosives Facilities are to evacuate to the designated emergency assembly point.

All personnel are to remain at the emergency assembly point until advised of all clear by the Mine Supervisor.

The Mine Supervisor will arrange for a head count of personnel at the emergency assembly point and advise the Emergency Response Team Leader of any missing personnel.

Ensure that the Mine Manager have been advised. When Emergency Response Team arrives, advise

Emergency Response Team Leader of situation. Work with the Emergency Response Team Leader to

safely evacuate all personnel from the Explosives Facilities.

Arrange for a head count of personnel at assembly point, advise Emergency Response Team Leader of missing personnel.

7.3 CYANIDE OR CHEMICAL SPILLS – SEE SECTION 5

7.4 FUEL AND OIL SPILLAGETo minimise the possibility and effects of any potential spillages of oil or diesel in which contamination of fresh water occurs on site.

7.4.1 USE AND LOCATION Primarily areas associated with Warehouse and Mining Contractor, however accidental spillage could occur on any area of operations.

7.4.2 DANGER 7.4.2.1 Health Hazard Data All spillages have fire/explosion/health/environmental risks. Prompt controls are required to prevent damage to life and equipment.

7.4.2.2 Specific Data/Precautions Reference must be made to the

chemical information manual and/or manufacturers guidelines for extra data and precautions.

7.4.3 PROCEDURES 7.4.3.1 Notification Large oil and diesel spillage requires immediate notification. Security Control will be notified and Security will contact the Environmental Coordinator.

7.4.3.2 Personnel Responsible Environmental Coordinator with the assistance of Safety / Medical Coordinator and operations personnel involved will be responsible for the control, removal and clean-up of any accidental spillage.

8.0 C0MMUNITY EMERGENCIESCompany Aim, Objective and Protocol

Because Company A Limited.has emergency response type resources, the company will endeavour to extend these resources to the wider community when there are real circumstances to save life and property off the mining lease.

No resources are to be moved off site without permission of the General Manager or his alternate.

Each community emergency will be dealt with on the information available at the time and will be handled in conjunction with the District Administrator and or local Police. Again a call for assistance from the District Administrator and or Local Police requires the General Manager or his alternate’s permission prior to any Company A Limited.response.

Where Company A Limited.employees are involved in a non-work related emergency the company will provide resources with the permission of the General Manager or his alternate.
