Predictors of relapse among pulmonary tuberculosis patients treated in a DOTS
programme in South India (Thomas A, et al. Int J Tuberc Lung Dis 2005; 9: 556-61.)
- Objective :
Identify risk factors associated with relapse
among cure TB patients in DOTS programme in
South India
- Cohort Study : between April 2000 and
December 2001 fluorescence microscopy for
AFB and by culture M.TB at 6, 12 and 18 months
after treatment completion
Results : 534 cured patients, 503 (94%) F/U 18 months after
treatment completion
: 62 (12%) relapsed; 48 of 62 (77%) relapses occurred during the first 6 months of F/U
: Patients who took treatment irregularly were twice more
likely to have relapse than adherent patients(OR 2.5; 95% CI 1.4 to 4.6)
: Other independent predictors of relapse were initial drug resistance to INH and/or RMP (OR 4.8; 95% CI 2.0 to 11.6)
and smoking (OR 3.1; 95% CI 1.6-6.0)
Results
: Relapse rate among non-smoking,
treatment
adherent patients with drug-sensitive
organisms was 4.8%
