Praktijk plasmatransfusie
• 74 aanvragen pakket bloeding na chirurgie (44 patiënten)
• Registratie APTT, APTT na heparinase, PT, fibrinogeen, aantal FFP en type ingreep
• Conclusies:
– FFP-transfusie bij 30% van patiënten met aantoonbare heparine
– FFP-transfusie bij 35% van patiënten met waarschijnlijke chirurgische bloeding of trombocytendefect of -tekort.
– FFP-transfusie 41% van patiënten met bloeding door stollingsprobleem.
– 7 patiënten (16% van totaal) had een APTT>1.5xURL (60 sec). FFP-transfusie bij slechts 2 (29%). Beiden vielen in categorie stollingsprobleem (APTT verlengd, heparinase verlengd).

Management of bleeding following major trauma: an updates European guideline. Crit Care 2010; 14: R52:
– Rec. 12: “We suggest that thromboelastometry also be performed to assist in characterising the coagulopathy and in guiding haemostatic therapy (Grade 2C).”
– Rec. 22: “We recommend that monitoring and measures to support coagulation be initiated as early as possible (Grade 1C).”(Hb, trombo, APTT, PT, fibrinogeen, geïoniseerd Ca)
– Rec 24: “We recommend early treatment with thawed FFP in patients with massive bleeding (Grade 1B).”
– Rec. 24: “The initial recommended dose is 10 to 15 mL/kg. Further doses will depend on coagulation monitoring and the amount of other blood products administered (Grade 1C).”
– Rec. 27: “We recommend monitoring of fibrinolysis in all patients and administration of antifibrinolytic agents in patients with established hyperfibrinolysis (Grade 1B).”

• Belangrijk: TEG heeft een lage specificiteit
• TEG wordt aanbevolen bij begeleiding van massale bloeding protocollen
• Mogelijk verbeterd de TEG de overleving bij massale bloeding
