ผปวยทสมผสโรคสกใส ไดแก อยในหอผปวยเดยวกน หรออยในหองเดยวกนในกรณทผปวย index case ไมไดลงจากเตยง

เปน immunocompromised host ไมเปน immunocompromised host

อยในสภาวะทปวยหนก วกฤต ไขสง vital signs ไม stable หรอม skin lesions ทรนแรง

อยในสภาวะ ทคอนขางด

พจารณาให IVIG หรอ VZIG(1)

• แยกไวในหองเฝาระวง(2) ในชวงวนท 8-21 หลงสมผส

• หากมผนขนรบใหacyclovir(3) รกษา และยายไป อน. 2(4)

• แยกไวในหองเฝา ระวง(2) ในชวงวนท 8-21 หลงสมผส

• หากมผนขนรบให acyclovir(3) และยาย ไป อน. 2(4)

• พจารณา oral acyclovir 80 mg/kg/day เรมวนท 7 หลงสมผส x 5 วน

ไมมภมตานทาน ไดแก - ไมเคยเปนสกใส - ไมเคยฉดวคซน - อาย < 1 ปและมารดาไมเคยเปนสกใส

มภมตานทาน ไดแก - เคยเปนสกใส - เคยไดวคซน - อาย < 1 ปและมารดา เคยเปนสกใสแลว

ไมตองเฝาระวงสามารถอยหอผปวยหองใดๆ กได สมผสโรคมาเกน

5 วน หรอมอาย <1 ป

สมผสโรคไมเกน 5 วนและ อาย

>1 ป

- แนะน าใหวคซนเรวทสด (ถาอาย >13 ปใหนดมาใหเขมท 2 อก 4 สปดาหตอมา) - แยกไวในหองเฝาระวง(2) (ไมวาจะไดรบวคซนหรอไม) ในชวงวนท 8-21 หลงสมผส หากมผนขน รบให acyclovir(3) และยายไป อน.2(4)

- หากผปวยสามารถกลบบานได ขอใหสงกลบบานพรอมค าแนะน าในการเฝาระวง การเกดโรคและการดแลรกษา

หมายเหต: 1. การปองกนดวย VZIG หรอ IVIG นน มราคาแพง อาจมปญหาแทรกซอนจากการให และไม สามารถปองกนโรค

ได 100% นอกจากนอาจท าให incubation period ยาวขน ดงนน ควรเลอกให VZIG หรอ IVIG ส าหรบผปวยทเปนผปวยวกฤตจรงๆ แตไมสามารถรบประกนประสทธภาพได โดยใหเรวทสดหรอภายใน 10 วนหลงสมผสสกใส

2. ผปวยในหองเฝาระวง สามารถรวมผปวย immunocompromised และ immunocompetent ดวยกนได ยกเวนมปญหาอนทตองแยก เชน เปน TB รวมดวย 3. การให acyclovir i.v. ควรใหในผปวยทเปน immunocompromised, ผปวยวกฤต และผปวยทคาดวาการดดซมยาจะไมด เชน มปญหาของระบบทางเดนอาหาร มฉะนนสามารถใหเปนยากนได 4. ผปวยทเปนโรคสกใสตองยายลงตก อน.2 เสมอ หากเตยงเตมใหปรกษาสาขาโรคตดเชอ

• เจาะเลอดตรวจ varicella IgG โดยเรวทสด ใหไดผลภายใน 3 วนหลงสมผส • แจงสาขาโรคตดเชอ • ระหวางรอผลเลอด สามารถปฏบตงานไปไดตามปกต

ไมตองท าอะไร ไมสามารถ เจาะเลอดให ทราบผลได ภายใน 3 วน

- ฉดวคซนทนทภายใน 3-5 วน และฉวคซนเขมท 2 อก 4 สปดาหตอมา

Infection Preventionist (IP) Competency Model

Management styles and characteristics of IPs

1. Autocratic The IP independently solves problems, without input or advice from others. This style may be necessary when patient and/or employee safety is at risk and decisions must be made quickly.

2. Consultative IPs will share problems with peers and solicit their ideas before making a decision. IPs use this approach most often.
