흡연

직업 및 환경적 발암물질

(1) 석면증(asbestosis) 환자: 4-5배 발병

특히 흡연시 14%까지 발병 (80-90배)

(2) Radon 가스, Arsenic, Chromiun, Beryllium,

Chloromethyl ether, Polycyclic hydrocarbons,

Vinyl chloride

(3) 대기 오염

기존의 폐질환 : 만성 폐쇄성 폐질환, ILD

수술

방사선치료

항암화학요법(항암제치료)

완화요법 (supportive care)

비소세포암의 치료원칙

IA : 수술

IB, IIA, IIB : 수술, 수술 + adjuvant chemotherapy

IIIA : chemotherapy + 방사선치료 (concurrent > sequential)

수술 + adjuvant chemotherapy

neoadjuvant chemotherapy + 수술 (방사선치료)

IIIB : chemotherapy + 방사선치료 (concurrent > sequential)

VI : chemotherapy

best supportive care

방사선치료 :

대개 수술을 하지 못하는 3기

증상완화목적: 골전이, 뇌전이

operability가 없는 조기 폐암환자

항암화학요법 :

3기와 4기 폐암환자에게 시행

수술전 (neoadjuvant) 또는 수술후 (adjuvant)

최근 표적항암제가 많이 사용됨

레이저치료 : 일부 환자의 증상을 완화

면역요법, 유전자치료 : 효과 확실치 않고 연구단계

식이요법 및 여러 종류의 대체의학 : 근거 없음

소세포암의 치료

전신전이가 빠른 암으로 전신 암으로 간주하여

수술은 원칙적으로 불가능

대부분 항암제 치료, 추가치료로 방사선치료를

병행

항암화학요법과 방사선치료에 매우 잘 듣고

완치되는 경우도 있음

항암제 치료에 반응을 잘 하지만 재발과 전이가

흔함

소세포암의 치료원칙

Limited disease :

chemotherapy + 방사선치료 (동시)

PCI (prophylactic cranial irradiation)

Extensive disease :

chemotherapy +/- PCI

폐암치료에 사용되는 항암제

Cisplatin, Carboplatin, Paclitaxel,

Docetaxel, Vinorelbine, Vinblastine,

Gemcitabine, Etopocide, Irinotecan,

Mitomycin, Ifosfamide, Pemetrexed

Targeted therapy :

EGFR TKI : Gefitinib, Erlotinib

VEGF inhibitor : Bevacizumab

ALK inhibitor : Crizotinib
