Our goal is to leverage mHealth Apps to drive better lifestylechanges for patients and greater communication between

Problems Addressed
Current Patient-Centric System focuses solely on self-diagnosis/self-treatment
Disconnect between primary care physicians and theadherence to prescribed advice to their patients.
The mobile generation accesses a multitude of mHealthApps and Sites whereby self diagnosis and results of suchadvice remains within the realm of the Patient. Primary CarePhysician remains a 3rd party, at best to this Currentecosystem

Initial target market will be the adolescent and adultpopulation ailing from Diabetes (There are 22 other chronic micro-
