¨ CASO CLÍNICO –ESTEATOHEPATITIS NO ALCOHOLICA INDUCIDA POR RALOXIFENO¨

Una mujer posmenopáusica de 53 años de edad,con antecedentes familiares de la cirrosis hepática, se lediagnosticó osteoporosis, y se inicia en el moduladorselectivo del receptor de estrógeno (SERM) raloxifeno 60mg / día por vía oral. Ella desarrolló marcada disfunciónhepática. Su índice de masa corporal (IMC) fue de 26,5.Suquímica sanguínea indicó AST 342 UI / L, ALT 356 U/ L. marcada
con resistencia a la insulina.Histológicamente, se diagnostica este caso como conesteatohepatitis no alcohólica pre cirróticos (EHNA). Estees el primer caso confirmado histológicamente de EHNA que se vioagravada por el raloxifeno.

desarrollaron cirrosis hepática .Hermana y hermano con hipertensión.Antecedentes Patológicos o personales:1998: 47 años leve disfunción hepática-higado
graso.
HABITOS NOCIVOSAlcohol: No consume.Cigarro: No hay datos.Café: No hay datos.Te: No hay datos.Drogas de consumo: No hay referencias.HÁBITOS ALIMENTICIOS Y/O DIETÉTICOSEjercicios físicos : No refiere.
IMPRESIÓN DIAGNOSTICA :
1. Artritis reumatoide. 2. Osteoporosis.
Problema de Salud que tiene el paciente:
.La paciente presenta una disfunción hepática,
las transaminasas se encuentran elevadas elevadas: AST 342 UI/L , ALT 356 UI /L .

CONCLUSIONES
• Se debe de realizar antes de utilizar el raloxifeno ,un seguimiento de las pruebas de función hepatica antes y durante el tratamiento con raloxifeno , para poder evitar una ESTEATOHEPATITIS NO ALCOHOLICA .
• Se desconoce el posible mecanismo de la ESTEATOHEPATITIS NO ALCOHOLICA inducida por raloxifeno.
