Generic Drug Products
• Generic drug product / Multi-source pharmaceutical product / Abbreviated new drug / Subsequent entry drug product / Copy drug / Local made drug
• Multi-source pharmaceutical product need to conform to the same appropriate standards of quality, safety, and efficacy as those required for innovator (comparator) product
• Reasonable assurance must be provide must be provided that the multisource product is therapeutically equivalent and interchangeable (prescribability and swichability) with the comparator product

Drug product performance - a release of the drug substance from the drug product leading to bioavailability of the drug substance
Factors affecting drug product performance
• Pharmaceutical factor
• Physicochemical properties of drugs – solubility, particle size, polymorphism, salt form, hydrophobicity, pKa, stability
• Dosage form characteristics – disintegration time, dissolution time, excipients
• Patient-related factor
• Age, gastric emptying time, intestinal transit time, food content, pre-systemic metabolism
• Route of administration 6

Bioequivalence (USFDA)
• The absence of a significant difference in the rate and extent to which the active ingredient or active moiety in pharmaceutical equivalents or pharmaceutical alternatives become available at the site of drug active when administered at the same molar dose under similar conditions in an appropriately designed study
16
Therapeutic equivalence (USFDA)
Drug products are considered to be therapeutic equivalents only if they are pharmaceutical equivalents and if they can be expected to have the same clinical effect and safety profile when administered to patients under the conditions specified in the labeling

Dissolution testing
• Testing product quality
• Bioequivalence surrogate inference
21
Dissolution testing
• Accordance with pharmacopoeial requirement
• Medium pH : 1.2, 4.6, and 6.8
• 12 individual value for every time point
22http://www.pharmamirror.com/pharmaceutical-articles/bio-equivalence-study-of-
generic-pharmaceutical-oral-solid-products/
Dissolution testing
• > 85% within 15 min – no need similarity
factor (f2)
23
• n = No. of time points
• Rt = %API dissolved of reference product at time point x
• Tt = %API dissolved of test product at time point x
• F2 50 = similar and no difference > 10%
Dissolution testing
• Minimum of 3 time points
• Not more than one mean value of >
85% dissolved for each formulation
• %CV not > 10% over the second to the
last points 24

• Don’t use t = 0
• Use more point at >85%

Types of Bioequivalence study
Types of Bioequivalence study
• Comparative pharmacokinetics studies
31
Types of Bioequivalence study
• Comparative pharmacodynamics studies Cmax and AUC are unable to be the
representative its level at site of action
Limited by within-subject variability
• Comparative clinical studies
Alternative for limitation of comparative PK or PD studies
Less sensitive
• Comparative in vitro studies Less expensive
Popular 32
Study design• Non-replicated designs – Two-formulation, two-period, two-
sequence cross-over design
• Parallel design – long half-life drug
33
Study design
• Replicated crossover designs
34
Study design
• Replicated crossover designs
35
Highly variable drug
36
Subject, sample size and dropout
• Healthy volunteer or patients
• Number of subjects
Within-subject %CV
Study power; not less than 80%
Acceptance range for the test-reference product difference 20%
Minimum numbers are 12
Dropout: the number entered to the study should be sufficient for dropout
37
Drug administration
• Single dose is preferred
• Multiple dose Non-linear PK, modified release drug, analytical problem, highly
variable drugs, and patients
• Test and reference product
Reference product should be the originator
38
Fast-Fed condition
• Fast condition is preferred
• If the Summary of Product Characteristics (SmPC) mentions about food-drug interaction Fed condition
39
Dose selection
• Depend upon the label claims, assay sensitivity, and relevance to the practical use condition
• Highest strength is preferred (most sensitive)
• Highest strength = highest dose in therapeutic range
40
Dose selection
• For drug with nonlinear PK (EMEA, 2010)
Increase dose increase AUC or Cmax : study at highest dose
Increase dose decrease AUC or Cmax : study at both highest and lowest dose
41
Dose selection
42
Dose selection
43
Dose selection
44
Sample collection
• 1 point immediately prior dose, 1-2 points before Cmax, 2 points cover Cmax, and 3-4 point at elimination phase
45
Parent compound or metabolites
• Parent compound is the best surrogate to demonstrate BE
• Metabolite are needed when Be unable to determine parent compounds, probably due to lower t1/2 or parent
as prodrug
Metabolites play major roles of the activity
Non-Linear PK drug
46
Chemical analysis
• Good laboratory practice: EMEA/OECD GLP หรอ WHO GLP หรอ ISO/IEC 17025:2005
• Guidance for Industry: Bioanalytical Method Validation
Pharmacokinetic and statistical analysis
• For single dose; AUCt, AUC, Cmax, tmax
• For multiple dose; AUC, Cmin, Cmax, Cav, Fluctuation and swing
• Non-compartmental model
• Acceptance criteria: 80-125% at 90% CI
• ANOVA (90%CI)
