• Check the effect of the treatment yourself byarranging to see the patient again after anappropriate interval of time. Only conclude thetreatment when it can be established that alltraces of the disorder have disappeared.

Follow-up treatment• Keep the treated area of skin clean.• Swimming or showering are permitted.• Do not pick or scratch the treated area.• Use a tape to protect any blisters that mayform.

• Do not lance any blisters that may form.

Undesirable effects• A stinging or painful sensation during and afterfreezing, which will rapidly fade away after thethawing phase.

• Changes in the intensity of pigmentation mayoccur. This will generally take the form of hypopigmentation; however, post-inflammatory hyperpigmentation due to melanin or haemosiderin can also occur.

Recommended Freezing Time• The doctor can, according to the type,spreading of the lesion and skin thickness,adapt the duration of the treatment.

Remarks• Dispensing additional cryogen causes morewater vapour to condense onto the applicator,thereby making it so damp as to impair itsfunction as a reservoir. Visible ice crystals thenform. If this should occur, replace theapplicator with a new one.

• Histofreezer® Portable Cryosurgical Systemshould only be used in combination with thespecial applicators.

• Imprudent use can lead to excessively deepfreezing, producing damage to the dermis andconsequent scar formation and nerve damage.

• The gas used by this equipment is slightlyflammable! Do not use in combination with, ornear, diathermy.

• Histofreezer® Portable Cryosurgical System hasa shelf-life of 3 years under normal storageconditions (see section on storage andtransport).

Patient informationIt is important that patients be precisely and fullyinformed concerning treatment withHistofreezer® Portable Cryosurgical System.Histofreezer® Portable Cryosurgical System is asafe, effective and controlled form ofcryotherapy. The skin is treated by freezing. Theapplicator, which is held in contact with the skin,reaches a temperature of -55 ˚C. The uppermostlayer of skin, together with the diseased tissue,will disappear. It will be replaced by a new,healthy layer of skin in 10 to 14 days. Freezingcommences once the applicator is placed incontact with the skin. The affected skin will turnwhite. From this point on you may experience astinging or burning sensation. This sensation willrapidly fade away after the thawing phase.Temporary, visible changes in the intensity ofpigmentation may occur following treatment.Cryotherapy sometimes gives rise to blisters.Under no circumstances should you lance theblister, instead protect it with a tape. Keep thetreated area clean and do not pick or scratch it.Swimming or showering are permitted. Somedisorders may require a series of treatments.

Principe d’actionL’évaporation du mélange gazeux liquéfié

L’action repose sur les différentessensibilités au froid des différents types decellules de la peau. Ainsi, les kératinocytes del’épiderme sont beaucoup plus sensibles aufroid que le réseau de fibres de collagène et de fibroblastes du derme inférieur. Lesmélanocytes aussi sont très sensibles au froid.Une cloque peut se former, due à la nécrosedes kératinocytes. La régénération suit dans les10 à 14 jours, depuis l’épiderme environnant etles annexes plus profondes. Si le derme resteintact pendant l’intervention, la guérison sedéroulera sans cicatrice. Toutes les formes decryothérapie reposent sur ce principe.

Contre-indicationsContre-indications absoluesLa cryothérapie est contre-indiquée chez lespatients atteints de cryoglobulinémie.

Précautions• Incertitude dans le diagnostic de l’affection(attention au carcinome cutané).

• La dépigmentation, comme effet secondaire,peut ne pas être esthétique dans le casd’une peau foncée. Sur une peau claire ladépigmentation est rarement visible, maiselle a tendance à se colorer aprèsexposition au soleil.

• Une congélation (trop profonde) dans levoisinage des vaisseaux du bout des doigtset des orteils peut, théoriquement,provoquer une nécrose distale des lésionscongelées. Ce phénomène n’a pas encoreété observé à ce jour avec Histofreezer®Système Cryochirurgical Portatif.

The Histofreezer® PortableCryosurgical System Kit consists of:1. Aerosol canister. Filled with liquified gas,consisting of a mixture of dimethyl ether,propane and isobutane. This gas mixturedoes not damage the ozone layer.

2. Applicators. The package includes:24–2mm Small applicators and 36–5mm Medium applicators

3. Directions for use. This contains fulldetails concerning the principle andoperation of Histofreezer® PortableCryosurgical System, and its use in thetreatment of warts.

ImportantHistofreezer® Portable Cryosurgical Systemshould only be supplied to (para) medicallytrained healthcare professionals. It should onlybe used by and be available to (para) medicallytrained healthcare professionals. Imprudent usecan lead to unwanted damage to the skin andunderlying tissues. It is prohibited to sell or giveHistofreezer® Portable Cryosurgical System topatients. Use the aerosol canister only incombination with the special Histofreezer®Portable Cryosurgical System applicators.

Storage and transportThe canister is pressurized. Protect it fromdirect sunlight and do not expose it totemperatures in excess of 50 ˚C. Even afteruse, do not pierce or burn. Do not spray in thedirection of a flame or a glowing object.COMBUSTIBLE. Does not damage the ozone layer.

Principle of actionEvaporation of the liquified gas mixture drawsheat from the surroundings. The applicator,

which serves as a reservoir for the cryogen,reaches a working temperature of -55 ˚C. Its action is based on the fact that differenttypes of skin cells vary in their sensitivity tobeing frozen. Accordingly, epidermalkeratinocytes are many times more sensitive tobeing frozen than the network of collagen fibresand fibroblasts in the underlying dermis.Melanocytes are also highly sensitive to beingfrozen. Necrosis of the keratinocytes can resultin the development of a blister. Full recoverytakes about 10 to 14 days, with new tissuegrowing inwards from the surrounding epidermisand the more deeply situated adnexa. If thedermis is undamaged by the treatment thenhealing will take place without scar formation.All forms of cryotherapy are based on thisprinciple.

Contra-indicationsAbsolute contra-indicationsCryotherapy is contra-indicated in patientswith cryoglobulinaemia.

Precautions• Uncertainty concerning the diagnosis of thedisorder (possibility of skin cancers).

• Depigmentation, as an undesirable effect,can be cosmetically somewhat unattractivein more highly pigmented skin types. Inlight-coloured skin, depigmentation isbarely noticeable, but it does tend to colourdifferently after exposure to the sun.

• Freezing (to excessive depth) in the regionof peripheral arteries in fingers and toes cantheoretically produce necrosis distal to thefrozen lesions. However, this has never beenreported in conjunction with the use ofHistofreezer® Portable Cryosurgical System.

Depending on the nature and extent of the lesion, and the thickness of the skin, the treatment time canbe adapted appropriately.

2. Donner doucement un mouvement de

NE PAS plier deforce le tube parun mouvementde va-et-vientou de haut enbas à partir dela valve de labombe aérosol,ce qui pourraitcasserl'applicateur, enen laissant unepetite partiedans la valve.

• A blister, sometimes filled with blood, maydevelop after a few days. In areas with a thicklayer of callus, such blisters will not necessarilybe visible to the unaided eye. Do not lance the blister; instead, protect it by covering itwith a tape.

• Never treat two different patients withthe same applicator (possibility of cross-infection).

Once the Histofreezer® application has beenadministered to the lesion, remove the usedcustom applicator from the dispensing valve usingthe following steps:

1. Place your forefinger and thumbsecurely on either side of thehollow tube end close to thedispensing valve.

2. Gently rock the applicator back

and forth whilepulling away fromthe canister untilthe hollow tubereleases itselffrom the valve. DO NOTforcibly bendthe hollow tubeback and forthor up and downfrom thecanister valve as this mayinadvertentlysnap theapplicatorleaving a smallportionremaining in thedispensingvalve.

Programme detraitement1. Demander au patientd’exposer la surfaceà traiter vers le haut.

2. Placez l'applicateursur l'aérosol.

3. Ôter la capsule deprotection du bouton poussoiret injecter le gazdans l’applicateurjusqu’à ce que desgouttes s’enéchappent. Tenir l’aérosolbien droit.

4. Tenirl’applicateur 15 secondesdirigé vers lebas pour qu’ilatteigne latempératured’actionnécessaire.

5. Placer ensuitel’applicateur avecune légèrepression sur la lésion. Il est important demaintenir l’applicateur verticalementvers le bas.

• Tenir le doigt éloigné de la valve, sans ré-appuyer pendant le traitement.

Methods of treatmentGeneralCryotherapy can produce a painful, burningsensation on the skin. Acceptance of thetreatment can be enhanced substantially byinforming patients about the degree of painthat can be expected, the anticipated numberof treatments, any preparatory treatment thatmight be required, possible undesirable effectsand the follow-up treatment.

Preparatory treatmentKeratin tends to act as a thermal insulator.With highly elevated warts (in excess of a fewmm) or warts located at pressure points in thepalm of the hand or on the sole of the foot, itcan be extremely useful to remove theuppermost layer of keratin with a curette, fileor pumice stone, possibly after applying akeratolytic agent. Preparatory treatment canenhance the efficacy of Histofreezer® PortableCryosurgical System and reduce the number ofapplications required.

Treatment schedule1. Have patientspositionthemselves suchthat the surface to be treated isexposed and facing upwards.

2. Attach theapplicator to the canister.

3. Remove theprotective cap fromthe push buttonand spray gas intothe applicator untildroplets emergefrom it. Keep theaerosol canisterupright.

4.Hold theapplicatorverticallydownwards andwait 15 secondsfor it to reachits effectiveworkingtemperature.

5. Next, place theapplicator on thediseased tissue tobe frozen and exerta slight pressure. When doing so, it isimportant that the applicator ispointing directly downwards!

• Remove finger from the dispensing valve; do not spray again during treatment.

• Freezing starts within a few seconds, asshown by the white discolouration of theskin. From this point on, the patient mayexperience stinging, burning or, occasionally,painful sensations.

• During the period of freezing, a narrow stripof healthy tissue should be frozen along withthe diseased tissue. If this only disappearsslowly during the period of freezing, itindicates that the freezing process is notproceeding as well as it should. In this event,re-fill the applicator and repeat thetreatment.

• Once the applicator has been removed, the white discolouration of the skin will fadeaway after a few minutes. An erythema
