Vertical ocular misalignment due to supranuclear lesions involving projections from utricle to interstitial nucleus of Cajal, cerebellum, or different areas of brainstem

Skew Deviation: lệch nghiêng

An acquired vertical* strabismus resulting from imbalance in

vestibular(balance system) inputs to the oculomotor system

(lác dọc mắc phải do mất thăng bằng xung thần kinh đi vào

nhân vận nhãn)

• Skew deviation is a vertical misalignment of the visual

axes caused by a disturbance of prenuclear inputs, with or

without abnormal torsion

• (lệch nghiêng là sự đặt sai vị trí theo chiều dọc của

trục mắt gây ra bởi rối loạn trước nhân có hay không

xoắn nhãn cầu bất thường)

• Skew deviation is associated with other central neurologic

signs and may be part of the ocular tilt reaction (OTR).

• (lệch nghiêng kết hợp với dấu tk trung ương khác và

có thể một phần của phản ứng nghiêng nhãn cầu)

Có 3 loại lệch nghiêng (skew deviations):

Type 1: Upward deviation of both eyes; sound-induced vestibular symptoms; both eyes show counterclockwise rotary upward rotation (lệch lên trên cả 2 mắt, xoay lên ngược chiều kim đồng hồ) Type 2: Hypertropia in one eye; dorsolateral medullary infarctions; excyclotropia in the ipsilateral eye; hypertropia in the contralateral eye ( lác trên 1 mắt, nhồi máu hành tủy lưng bên; lác vòng ngoài cùng bên, lác trên đối bên) Type 3: Simultaneous hypertropia one eye and hypotropia in the other eye; upper brainstem lesion (lác trên 1 mắt, lác dưới 1 mắt; sang thương phần trên thân não)

vestibular periphery, brainstem, or the cerebellum

Thực hành lâm sàng

Test lệch nghiêng (skew deviation) Yếu tố tiên đoán đột quỵ thứ ba là test lệch nghiêng, mất sự thẳng hàng nhãn cầu

theo chiều dọc do sự mất cân bằng phóng lực tiền đình bên phải và bên trái (neural

firing). Lệch nghiêng phát hiên bằng test che mắt thay đổi 2 bên (alternate cover

testing). Đây là dấu hiệu xác định bệnh lý hố sau, thường gặp nhất trong đột quỵ thân não và chỉ điểm tắc động mạch thân nền.

Eye Movement Abnormalities in Ocular Tilt Reaction

• Skew deviation(e.g., hypotropia of the right eye in right OTR)

• Ocular torsion with upper poles of the eyes rotated toward the lower ear (e.g., in right OTR, excyclotorsion of the right eye and incyclotorsion of the left eye)

• This is in contrast to physiologic counterrolling, in which the upper poles of the eyes rotate toward the higher ear

• May be dissociated between the two eyes

• Head tilt (e.g., right head tilt in right OTR)

• Acutely, may have associated torsional nystagmus

• Deviation of the subjective visual vertical (e.g., tilting of subjective visual vertical to the right in right OTR)

Ocular tilt reaction (OTR)

Skew deviation, head tilt và ocular torsion

+ Liên hệ cấu trúc tai giữa đáp ứng duy trì thăng bằng cơ thể:

semi-circular canals (SCC), utricle and saccule.

+ Nghiêng đầu gây kích thích. cả anterior SCC và posterior SCC kết

quả kích thích ipsilateral intorters (SO and SR) và contralateral

extorters (IO and IR) trong khi antagonists thì simultaneously

inhibited.

+ Otoliths (utricle and saccule) có thể theo đường tương tự

plane – there is a compensatory right head tilt with downward rotation and

excyclotorsion of the right eye and upward rotation and incyclotorsion of the left eye

Pathologic

Rightward OTR

OTR will have the same changes in head posture, eye

position and rotation as the physiologic OTR in the

absence of a change in body position in the roll plane to

stimulate it.

1) an ocular tilt reaction in the absence of any true body tilt in the roll

plane - this can be tonic or paroxysmal and can be a complete

ocular tilt reaction or partial with only certain components becoming

manifest (i.e., only a skew deviation or only synkinetic ocular

torsion),

2) a tilt in the subjective visual vertical – a perception by the patient

that vertical orientation is different from what is true vertical.

+ ipsilateral ocular tilt reaction can occur with lesions of the

utricle/labyrinth, vestibular nerve, or lateral medulla (i.e., in Wallenberg

syndrome) that are all caudal to the decussation of the graviceptive

pathway fibers in the pons

(OTR cùng bên: soan nang, mê đạo, tk tđ hay hành tủy bên)

+ Contralateral ocular tilt reaction can occur with lesions that affect

the rostral pons, MLF (i.e., in conjunction with internuclear

ophthalmoplegia), the midbrain or INC (i.e., in dorsal midbrain

syndrome) that are all rostral to the decussation of the graviceptive

pathway fibers in the pons.

+ There are a variety of etiologies that can affect these

areas, but, as stated previously, stroke, demyelination,

trauma, iatrogenic/post-surgical, and tumor are common

etiologies

+ Furthermore, there are multiple otolithic projections to

the cerebellum. Lesions of the cerebellum are also

known to cause complete or partial ocular tilt

reactions.

+ Pons – medial longitudinal fasciculus [MLF] (usually

contralesional head tilts)

+ Labyrinth/8th Nerve – ipsilesional head tilts

Symptoms

Dizziness or vertigo

Oscillopsia

Nausea/vomiting

Dysarthria/dysphagia

Clumsiness or poor coordination

Balance or gait problems (except due to loss of stereopsis)

Signs

Impaired dynamic visual acuity

Head thrust sign

Breakdown of smooth pursuit tracking or VOR suppression

Saccadic dysmetria

Nystagmus

Limb or gait ataxia

Differential Diagnosis of Skew Deviation and

Ocular Tilt Reaction

• Acute peripheral vestibulopathy

• Lesions in the vestibular nuclei

• Lesions in the cerebellum

• Lesions in the medial longitudinal fasciculus

• Lesions in the midbrain and interstitial nucleus of Cajal

• Raised intracranial pressure

1. A skew deviation is a vertical misalignment of the eyes that

is caused by damage to the otolithic input to the ocular

motor nuclei

2. When accompanied by binocular torsion, torticollis, and a

tilt in the subjective visual vertical, it is termed the ocular tilt

reaction.

Lesions causing this are classically localized to the

posterior fossa and can be from a variety of causes, but

are many times a result of an acute lesion, such as

ischemic stroke, demyelination, trauma, iatrogenic/post-

surgical, hemorrhage, or tumor affecting the brainstem,

cerebellum, or vestibular structures or pathways.

1. “Skew” is more than strabismus that doesn’t fit SO palsy

by 3-step test

2. Eye movement pattern in skew/OTR - towards ear is ‘on’;

keeps horizon stable; ‘derivable’

3. Some skews look like oblique palsy except fundus torted

the ‘wrong’ way

4. Ask about balance symptoms and look for signs in

patients with vertical strabismus
