This procedure is applied for incidents/accidents that occurred or could have occurred at VJGs facilities and employees (BHF, MXF, PMF, HNB, called VJG)

This procedure is applied for all VJGs employees at all level position.

This procedure can be applied for all contractors also whom work at VJGs site. However, Factory Director shall have a responsibility for decision on accident/ incident investigation.

L tai nn lin quan n xe c xy ra trong lc ti x ang kim sot xe m dn n chn thng, cht ngi hoc h hi hoc va chm c th gy h hi cho xe. / Any accident involving vehicle occurring while drivers is being control of the vehicle that result in either peoples injury, fatality or vehicle damages or collision may cause vehicle damages.

Tai nn mt thi gian lm vic (LTI) Lost Time Injury

L tai nn lao ng lm cho ngi b thng khng c kh nng i lm vo ca/ngy tip theo (theo k hoch) v khng tnh thi gian ngh khm bnh. Danh sch tai nn mt thi gian lm vic c th tham kho Thng t lin tch s 12/2012/TTLT/BLTBXH-BYT Hng dn vic khai bo, iu tra, lp bin bn, thng k v bo co nh k tai nn lao ng / Any accident in which the victims absent from the next day after the day that accident occurred, its not including the time for getting medical services. The list of LTI can be refer to the annex no. 01 of Circular No. 12/2012/TTLT/BLTBXH-BYT on guiding declaration, investigation, minutes preparation, statistic and periodically report for labor accident.

Tai nn hn ch cng vic (RWC) Restricted Work Case

L tai nn nhng khng dn n vic phi ngh lm ca/ngy k tip m ch phi hn ch lm vic t nht l cho ca/ngy k tip tu theo mc tai nn. Nu ngi b thng khng th thc hin nhng cng vic thng ngy ca h hoc b hn ch v thi gian lm vic trong mi ca/ngy th c xp vo loi RWC. Sau , nu ngi b thng phi ngh vic do vt thng (khng tnh thi gian khm bnh) th phi xp tai nn ny vo loi LTA. S hn ch ch c chnh thc tho thun gia cc chuyn gia v sc kho ngh nghip v trng phng lin quan v c ghi nhn h s. / Any accident that doesnt required to take day away from next day/shift. But have to restrict work at least next day/shift depend on

the accident level. If the victim can not do his/her daily work or restricted working time of next day/shift Its RWC. After that, the victim has to take day away from work due to his/her injury (dont count the medical examination time) Its transferred to LTA. RWC just is determined officially when having an agreement between by a physician or other licensed health care professional and related manager and

therefore to be recordable.

Medical Treatment Case

Any accident that doesnt required to take day away from work or restricted work. But the victim has to take medical treatment or physical treatment thats required more than first aid treatment.

7 S cp cu (FAC) First Aid Case

L TNL khng dn n vic phi ngh vic hoc hn ch cng vic v nn nhn cng khng cn phi iu tr y t. Tai nn c s cu bi s cu vin c o to. Any accident that doesnt required to take day away from work or restricted work and the victim isnt required medical treatment. The accidents victim shall be take a first aid by the trained emergency assistant.
