1.0 PURPOSE

1.1 To establish procedures of responding to the emergencies this can minimize loss of life, property and environmental impacts.

1.2 To define the role of emergency response team members to act properly when emergency occur.

1.3 To out line the communication and co-ordination with in the company and the external agencies in the emergency.

2.0 RESPONSIBILITIES

2.1 AMD provides approval on this procedure.

2.2 All employee are obligated to participate in training and rehearsal of the emergency response plan including; fire fighting training, evacuation and emergency drill. The safety committee shall prepare the rehearsal schedule.

2.3 All employees are obligated to alert the other people or report the emergency he/she discovering to the Emergency Control Center that will be called “ECC”.

2.4 All non-emergency response team- employee who are not assigned and trained to be a member of Emergency Response Teams, obligated to strictly follow the instruction given by the “ECC”.

3.0 SPECIAL INSTRUCTIONS AND PRECAUTIONS

3.1 Emergency is a sudden, generally unexpected occurrence or set of circumstances demanding immediate action that could lead on a worse effect to life, injury, property or environment. There are classified into 3 levels as below ;

3.1.1 Level 1: An unexpected occurrence which the company can control the situation themselves.

3.1.2 Level 2: An unexpected occurrence which the company cannot control the situation themselves and need cooperation with neighboring or industrial estate fire brigade.

3.1.3 Level 3: An unexpected occurrence which the company and neighboring or industrial estate fire brigade themselves cannot control the situation and need cooperation from Provincial or Other Provinces.

3.2 Scope of this emergency plan are ;

3.2.1 Fire and/or Explosion.

3.2.2 Chemical Spill and Leakage.

3.2.3 Natural Gas releases, Toxic Gas releases.

3.2.4 Major accident involving fatal injury or severe injury.

3.2.5 Flood and Other Natural Disaster

3.2.6 Terrorism, Riot

3.2.7 Epidemic , pandemic such as Bird Flu ,SARS

3.3 The support document may be need in case of emergency are following ;

3.3.1 Pre-Fire Plans.





3.3.2 Chemical Spill and Leakage Pre-Plans.

3.3.3 Plant layout.

3.3.4 Fire Protection Systems Data.

3.3.5 Material Safety Data Sheets.

4.0 PROCEDURE

4.1 Safety Committee shall establish Emergency Plan which including;

4.1.1 Emergency Preparedness Plan

a) Training Program b) Inspection Program c) Authorize and Responsibility

4.1.2 Emergency Response Plan

a) Fire Fighting Plan b) Spill Control Plan c) Evacuation Plan d) Flood emergency response plan e) Other

4.1.3 Emergency Relief/Mitigation Plan

4.2 Discussion/General Consideration.

4.2.1 To minimize the risk of accident or incident that could lead to a catastrophic event of emergency, precaution ruled by the Hazardous Work Permit Procedure, Fire Protection Plan Procedures and General Fire Safety Procedure must be always strictly followed.

4.2.2 Operation department must insure that Operating Procedures, in themselves, are not potential causes. After startup, random checks should be made to note whether the operators are following established procedures.

4.2.3 Safety Committee should conduct hazard analyses to visualize the chain of incident that can cause emergencies and make them hard to control.

4.2.4 It is necessary to prepare some of items listed below ready for any case of emergency. a) Telephone which have enough number to contact or being contacted with external

agencies. b) Radios which have enough frequency or channel to contact or being contacted with

external agencies. c) Plant Layout showing. d) Hazardous Area. e) Safety Equipment. f) Fire Protection System/ Water distribution Drawing. g) Traffic Plan. h) Assembly Point. i) Nearby location/ Neighborhood area. j) Nearby Tank Farm, Storage area of Flammable Material, etc. k) White boards, pen, and papers. l) List of on shift employees. m) Telephone directory composed with at least police station, hospital, fire brigade and etc.





4.3 Training of Emergency Response Teams.

4.3.1 All Emergency Response Teams are required to be trained in ; a) Technical Fire Fighting 1 time/year. b) First Aids 1 time/year. c) Hazard Communication 1 time/year. d) Spill Prevention and Control Plan 1 time/year. e) Emergency Drill 1 time/year.

4.4 Safety Equipment and Inspection

4.4.1 To ensure readiness of equipment and tools to be operated in emergency, the following list shall be inventoried and inspected/ maintenance regularly. Any defective of this equipment has to be rectified as soon as possible ; a) SCBA shall be inspected on monthly basis b) Fire hoses, nozzles, and accessories 1 time/month. c) Fire Pump 1 time/week d) Deluge systems 1 time/month. e) Fire Detection/Alarm systems 1 time/month. f) Fires fighting Gear including fire-fighting suits, helmets face shield, boots, and gloves 1

time/month. g) Portables fire extinguishers monthly. h) Foam/ Portable foam carts monthly. i) Fire Blankets 1 time/month. j) Smoke Detectors biannually. k) Spill Kits 1 time/month. l) First Aids Kits 1 time/month. m) Stretcher 1 time/month. n) Resuscitation Kits 1 time/month.

ALARM LEVEL

Alarm Level Meaning Things to do

EM1 General Alarm Operational partially disruption, incipient stage-fire, no explosion or serious consequent. Loss severity is MINOR. Can be controlled internally by team.

All Emergency Response Team member must report to the CCR in where the Emergency Control Center will be consequently formed.

Non-emergency members have to stop what they are doing. Prepare themselves for the next command or other alarm.

Evacuation Team check with the ECC and prepare for evacuation, except the building on fire, shall be evacuated immediately.

Onscene Commander goes to the signaling area immediately and assesses the risk.

Report to All Managers, Supervisors via pagers. Sizing-Up, Onscene Commander has to

communicate with emergency response team member for the next strategy.

Plant/Process partially shut down

EM2 Evacuation Alarm Severe disruption to operation unit, problem seems increasing to damage customer. Loss severity is SERIOUS Call back the CCR immediately and come in for standing by Emergency Control Center.

MC call for mutual aid an or external help Non-ERT Immediately Evacuate the plant Security Guard prepare route for fire trucks Plant shut down if necessary Prepare for mutual aid coordination Prepare for media, public interested parties. All senior management have to be at the emergency

control center Emergency Control Center took over by the

Government Agency Emergency Response Team, stand by to support. Plant Totally Shut down.

All Clear Alarm Situation is under controlled. Emergency Response Operation is abort.

Emergency Response Team report to ECC for investigation and salvage plan meeting

Resume to normal situation.

ภาคผนวก ข-43

ระบบคอมพวเตอรควบคมอตโนมต

การควบคมและบนทกผลการท างานของหมอน า จะท าการควบคมและบนทกผลดวย

ระบบการควบคมอตโนมต (DCS: Distribution Control System) ซงการท างานของอปกรณ

ตางๆ ทใชในการเดนเครองของหมอน า จะถกควบคมตรวจสอบโดยระบบคอมพวเตอร และ

แสดงผลผานจอมอนเตอรทงหมด เชน ความดนน าปอน/ไอน า, อตราการไหลของน าปอน/ไอน า,

อณหภมน า/ไอน า,ระดบน าในหมอน า, เครองสบน าปอน หรอ วาลว เปนตน รวมถง ระบบปองกน

อนตรายอตโนมต (Interlock System) โดยอปกรณดงกลาวขางตนจะถกโปรแกรมใหท างานกน

อยางสอดคลอง รวมถงระบบปองกนอนตรายอตโนมต หากเกดเหตผดปกต และโดยขอมลการ

ท างานตางๆ จะสามารถเกบรกษาและแสดงผลยอนหลงไดมากกวา 5 ป

ตวอยางเชน

การควบคมระดบน าในหมอน า

การควบคมระดบน าในหมอน านน ระดบน าจะถกตรวจสอบโดยอปกรณวดระดบน า 3 ชด โดยคาท

ไดจะน าไปควบคมการท างานของอตราการเปดวาลวน าปอน เพอควบคมระดบน าใหไดตามทตงไว

หากเกดการท างานผดปกตไมสามารถควบคมระดบน าไดโดยมคาสงและต ากวาทตงไว ระบบ

DCS จะสงใหหยดการท างานของหมอน าโดยอตโนมต โดยหยดการท างานของเครองกงหนกาซ (Gas Turbine)

ระบบการปองกนความดนหมอน าเกน

ระบบจะมอปกรณตรวจวดความดน 2 ตวและสงคามายงระบบ DCS โดย DCS จะตงคาลมตของ

ความดนไว หากความดนมสงถงคาทตงไว ระบบ DCS จะสงใหหยดการท างานของหมอน าโดย

อตโนมต โดยหยดการท างานของเครองกงหนกาซ (Gas Turbine) ซงจะเปนการปองกนความดน

เกนหากอปกรณลนนรภยไมท างาน ดวยอกทาง

จดทารายงานผลการปฏบตงานในการควบคมหมอไอนา โดยมวศวกรควบคมและอานวยการใชหมอไอนารบรอง สงใหกรมโรงงานฯ

จดสงเอกสารรายงานผลการตรวจทดสอบความปลอดภยหมอไอนา และเอกสารรบรองความปลอดภยในการใชหมอนาหรอหมอตม ใหกรมโรงงานฯ ภายใน 30 วน นบแตวนทเสรจสนการตรวจสอบ

ตรวจตามเอกสารรบรองความปลอดภยในการใชหมอไอนา ถายนขอตออาย เปน 3 ปหรอ 5ป ใหเอาขอกาหนดทตองทาใสไวในแผน

สงใบแจงขอเทจจรงของผผลต ผนาเขา ผสงออกหรอผมไวในครอบครองซงวตถอนตรายในความรบผดชอบของกรมโรงงานอตสาหกรรม (วอ/อก.7, 7.1, 7.2)
