Sleep and circadian neurophysiology

- description: The Centre for Sleep was established in 1981 with the aim to diagnose and treat patients that usually fall into a “mixed” category, between psychiatry and neurology (Parkisonism, agitation as side-effect during antipsychotic treatment, psychiatric symptoms in patients with Mb Huntington, Chorea maior/minor, Multiple sclerosis, Wilson disease, epilepsy, frontotemprotal dementias etc.). Our special focus is diagnosis and 5 treatment of sleep disorders such as narcolepsy, sleep apnoea, insomnia, somnambulism, REM Behaviour Disorder, Pavor nocturnus. We use polysomnography, specific forms of clinical exploration and non-invasive EEG. The staff includes five psychiatrists and neurologists, seven EEG technicians and seven nurses.

Emergency psychiatry and intensive care

- description: Department was established in 2003 in a renovated hospital building with the aim to admit acutely disturbed patients who need acute diagnostics and intensive care. The department consists of: - Intensive care unit (10 beds, admission of agitated and aggressive patients, the average length of stay is 16 days and if a patient needs prolonged treatment, they would be transferred to one of the departments within the hospital), - Emergency unit (32 beds, adult patients from all diagnostic entities who are coming to psychiatric treatment for the first time, or those who are agitated and suicidal) and - First psychosis unit (32 beds, patients aged 18 – 40 years who are experiencing first episode of psychosis and those who are in treatment for less than 3 years. The average length of stay is 45 days. Activities within the department include diagnostics, pharmacological treatment, education, occupational therapy, socio therapy and sports. After being discharged from the hospital, patients continue the treatment in intensive Day care unit, and as outpatients.)

Psychiatric rehabilitation

- description: Department was established in 2007 in a newly renovated hospital building and consists of three departments - Department for Treatment of Patients with Schizophrenia and psychotic disorders (40 beds), Department for Treatment of Patients with Affective and Anxiety Disorders (40 beds) and Department for occupational therapy and rehabilitation. Patients treated at these departments come here after being treated (and stabilized) at other departments within the hospital. After being discharged from the department, they are expected to take positions they used to hold in their family and social networks. Some of our activities are group therapy, occupational therapy, morning gymnastics, psychoeducation, social and communication skills therapy, bibliotherapy, film therapy, wellness, breathing exercise etc. The staff consists of four psychiatrists, nurses, psychologist, social worker and an defectologist (educational specialist dealing with children/adults) with disabilities.

Brewing your own coffee (a latte is 4,70 €!) , cooking your own meals by far is the cheapest option (and partly why Danes invite friends over instead of going out… and bring lunch to work instead of buying at the canteen). Many museums are free on Wednesdays etc. Renting a bike will save you loads on public transport (a ticket is 3,30€!)

- description: Opportunities exist for a 2- 6 week placement in Learning Disability at St. Vincent’s. The Centre is now home to 96 residential service users ranging from 7 – 70 years of age, all living in purpose built bungalows. Intellectual Disability

- description: Brothers of Charity Galway is an NGO funded by the Health Services Executive (HSE) to deliver social, residential, rehabilitative and mental health care to individuals with Learnin Disability diagnosis in the West of Ireland. The mental health care involves providing outpatient clinics in a number of locations in COuntries Galwaz and Roscommon as well as regualr reviews for service users in community-based residential settings and group homes. Italian EFPT exchange programme – Trieste

- description: Target :adult and old age psychiatry. Our psychiatric service is community based, detached from the hospital. It is a 24 hours mental health centre, that hosts in and out -patients. The work is equipe-organized: physicians, social workers, psychiatric rehabilitation educators, nurses work in it. Each centre hold from 4 to 8 beds for acute patients. We practise as well brief psychotherapies, psychiatric consultations to the hospital and to the rest homes. The main aim of the service is to treat and create terapeutic bonds with people with mental disorders in the places where they live, through domiciliar consultations. We are focusing on early onset psychosis,

developing a peculiar pathway of care for the youth age patients. We coordinate the work together with the service of substance dependence. Our service is no-restraint and open-doors.

- description: The unit does ECT 3 times a week (Monday, Wednsday and Friday) in a mean of 4-6 patients each day, between 8:30 – 10:30, using a MECTA Spectrum 5000Q device. ECT is a treatment for severe psychiatric illnesses. Present ECT techniques use sedation, muscle paralysis, ventilation with oxygen, and brief-pulse electrical stimuli that virtually eliminate the past risk of fracture and minimize transient cognitive dysfunctions. The mortality rate (about 2 deaths per 100 000 treatments) is less than that reported for normal childbirth and is associated with the anesthesia risks. Although the effect of ECT on memory emerges in public discussions, previous research clearly demonstrates the circumscribed and mostly transient nature of the cognitive effects of ECT. Despite its well-documented efficacy and safety, ECT is widely stigmatized as a last-resort treatment. The high incidence of chronicity and recurrence in severe psychiatric illnesses should encourage greater attention to improvements in ECT practice and to studies of its mechanism of action. Unfortunately, the lack of experience with ECT during residency training and the failure to require such experience for specialty certification limits the ability of clinical psychiatrists to recognize patients for whom ECT may offer effective relief.

3. Programme information 3.1 Turkish Psychiatric Inpatient and Outpatient units - description:

The clinic is composed of an inpatient and an outpatient unit. The inpatient unit with 20 beds and has a turnover rate of 180-200 per year. The inpatient unit also provides daily care for an additional five patients whenever needed. It serves as a half-open psychiatry clinic where, under the supervision of nurses, interns and their relatives; patients are allowed to leave the clinic during the day hours. However, in case of need, the outdoors/indoors can be temporarily locked. In the inpatient unit, the staff members and the patients organise their own entertainment meetings one day each week. There is also a lounge where the patients watch TV, play games, drink their beverages and attend good morning meetings aimed to increase the interaction among them. There is also an activity room where the patients can paint, knit or perform other kinds of handwork activities under the supervision of an expert.The mean duration of hospitalisation in the inpatient unit of the department of psychiatry is around 3-5 weeks This reflects a diagnosis and acute treatment-oriented approach of the caregivers. In the inpatient unit there is an electroconvulsive treatment (ECT) application room. The ECT device being used is a MECTA system. However, the Thymotron IV system will soon be

available for use. The number of patients receiving ECT treatment per year is around 30 and the average of ECT courses per patient is between 8-14. Bilateral application of ECT is carried out under anaesthesia. The outpatient unit offers special services for bipolar disorder, schizophrenia, alcohol/substance abuse and a consultation liaison program. The faculty members apply a regular training program for residents. The Department also holds regular meetings for brainstorming, reading recently published articles and seminars on several important issues.

English 3.2 Turkish Children’s Psychiatric In-patient Unit - description:

a) Child and Adolescent Mental Health Service (CAMHS) In-patient Unit of Dokuz Eylul University Hospital of Medical Faculty is one of the three institutions which give inpatient mental health service in the child and adolescent psychiatry discipline in Turkey. In our unit, patient admission has started since November 2005. We accept patients between 0 to 18 years old. However, most of the patients which are admitted to the unit are consisted of adolescent

age group. The unit has 10 beds and plus 5 daily care patients capacity. The patients are mostly from Aegean region and the patients referred from many parts of the country can also be accepted to the unit. Admission criteria are mainly, diagnostic clarification, treatment planning for severe psychiatric states, crises intervention, and forensic mental health assessment. On the other hand, the psychopathologies that need highly secure accommodation (such as substance abuse-dependency, severe disruptive behavioral problems, high risk of self harm) and organic medical conditions that need extra nursing care and medical assistance are not accepted to the unit. Our CAMHS in-patient unit provides a multidisciplinary and multi-model treatment service based on psychodynamic, cognitive-behavioral and family psychotherapies, pharmacotherapy and educational therapies. These treatment services are presented in a therapeutic community setting based on peer group and peer relations. The treatment team is consisted of psychiatrists, psychologists, psychotherapists, teachers and nurses. The young people in the unit are treated with multi-model treatment via the scheduled programs for weekdays and weekends. The average duration of stay in the unit is determined as 8-10 weeks. This duration can change according to the need of the child and the adolescent. The team consists of two consultant child and adolescent psychiatrists, three child and adolescent psychiatry trainees, one or two psychologist, six nurses, one educational specialist and one social worker. 3.3 Turkish Children’s Psychiatric In-patient Unit - description:

Child and Adolescent Mental Health Service (CAMHS) Inpatient Unit of Ege University Hospital of Medical Faculty is one of the three institutions which give inpatient mental health service in the child and adolescent psychiatry discipline in Turkey. We accept patients between 0 to 18 years old. Commonly seen disorders in unit are psychotic disorders and bipolar disorder. Most of the patients which are admitted to the unit are consisted of adolescent age group. The unit has 12 beds capacity. The patients are mostly from Aegean region but patients from other regions are being accepted. Admission criteria are mainly diagnostic clarification, intervention for severe psychiatric conditions and forensic mental health assessment. The treatment team is consisted of consultant psychiatrists, trainees, psychologists, psychotherapists and nurses. The average duration of stay in the unit is determined as 2-6 weeks. This duration can change according to the

need of the child and the adolescent. The team consists of two consultant child and adolescent psychiatrists, three child and adolescent psychiatry trainees, two psychologist, six nurses, one educational specialist and one social worker. Due to being one of three hospital which are giving mental health service in our country, our clinic application and admission number is high. We are very happy if you join our team.

- description: Mental Health Services across Bristol are based in a wide variety of sites including specialist mental health hospitals, community outpatient centres, integrated departments of the main physical health

hospitals and stand-alone specialist tertiary care centres. The city is small enough to be able to travel between these sites within the programme. Visiting trainees will be able to get involved in a variety of the areas listed above, ranging from clinical to academic. There are local trainee training programmes that the visiting trainee should be able to get involved in also. - introduction: There are a wide variety of opportunities available to visiting trainees and these can be tailored to trainee’s interests. The main focus would be on shadowing clinical work, being based in a few specialist areas of choosing (blocks of 1-3 weeks). In addition there are academic programmes that the trainee would be able to attend, Balint groups and the Core Trainee MRCPsych course (if applicable). The city is known for its friendliness, being small enough to travel easily but large enough to have a lively cosmopolitan atmosphere. - description: Visiting trainees can get involved in a variety of clinical activities. We will try our best to accommodate their wishes and interests, though not all may be possible.
