โรงพยาบาลขนาด A S ที่มีศักยภาพ พัฒนา intermediate bed/ ward โดยสหวิชาชีพ และ/หรือสร้างเครือข่ายระบบรับส่งต่อฟื้นฟูภายในเขต/จังหวัด

โรงพยาบาลขนาด M1 M2 พัฒนาศักยภาพเพื่อเป็นintermediate bed/ ward ในฐานะเครือข่ายการ

ฟื้นฟูกับโรงพยาบาล A S ในพื้นที่ และประสานข้อมูลถึง โรงพยาบาลระดับ F

โรงพยาบาลระดับ F มีบุคลากรที่มีศักยภาพ สามารถเชื่อมโยงงาน intermediate care สู่ชุมชน และคัดกรองผู้ป่วยที่มีความจ าเป็นกลับเข้ารบัการรักษาฟ้ืนฟู

กรณีจ าเป็น เช่น เกิด complication เป็นต้น

• Accessibility for all (bedside area for W/C user, PWD toilet, ramp, PWD parking, sign)

• Multi-displinary team (at least 2 branches)

• > 5 days/ week
