Akut ITP
• 4-6 barn (0-15 år) /100 000 /årca 75 barn/år i hela landet
• Akut debut 75% – ofta symtom bara 1-2 d• >50% föregående infektion (1-4v före)• Säsongsvariation: vinter - vår• Topp vid 2-5 års ålder• I småbarnsåldern något oftare pojkar.
Från 7 års ålder ingen könsskillnad

• Blödning i hud och slemhinnor• Palpation av lever, mjälte, lymfkörtlar• Infektionstecken• Tecken på annan bakomliggande medfödd
sjukdom, speciellt:-Fanconis syndrom-Wiscott-Aldrich syndrom-TAR syndrom (Thromhocytopenia with absent radius)
Utredning - prover
• Fullständigt blodstatus• Perifert blodutstryk • Serologi:
småbarn - CMV äldre - Mykoplasma, EBVSpec grupp - HBV, HIV Inför IVIG: HBV, HCV

Kronisk ITP
• Det finns vid debuten inget säkert sätt att skilja akut från kronisk ITP. Men Kronisk ITP: - vanligare hos äldre barn, särskilt flickor (3:1)- smygande debut med ofta ngt högre trc-tal- kan vara första symtom på autoimmun sjukdom
• De flesta patienter håller Trc >30 x 109/L utan beh• Endast ett fåtal drabbas av allvarligare blödningar • 60-80 % tillfrisknar och når Trc-nivåer på 100-150 x 109/L
spontant• Tillfrisknande förefaller vara oberoende av givna
behandlingar

Behandling – medicinsk 2• Vid låga trc-tal och blödningsbenägenhet ev IVIG
0,5g/kg var 3-4v
• Splenektomi normaliserar trc-talet hos 60-90% av barnen. OBS: spontan remission kan ske även många år efter debut.
• Före splenektomi vaccination mot:– Pneumokocker– meningokocker– Hemofilus influenzae– följas av kontinuerlig antibiotikaprofylax under minst 2 år
Uppföljning
• Följas med tanke på ev debut av autoimmun sjukdom
• Flickor inf om risk få barn med övergående neonatal autoimmun ITP

Lindrig purpura (fåtal blåmärken och petekier)
• Vid TPK
Behandling 2
Omfattande purpura (med rikliga petekier och echymoser med eller utan slentinneblödning)
• Behandlas med IVIg eller kortikosteroider• I svåra fall kan dessa kombineras.
IVIg oftast snabbare effekt än perorala kortikosteroider- förstahandsmedel vid mer uttalade symptom. - lämpligare att ge vid ev pågående infektion - eller om man inte gjort BM-us
Behandling 3
• Vid signifikant slemhinneblödning och TPK
Behandling Livshotande blödning
• Vid livshotande (intrakraniell) blödning,oavsett TPK-värde behandlas initialt med IVIgeventuellt i kombination med högdosPrednisolon po. eller Metylprednisolon, alternativt Dexamethason parenteralt.
• Trombocyttransfusion ges. • Plasmaferes kan prövas om iv. kortikosteroider,
IVIg och trombocvter ej har effekt. • I sista hand kan även splenektomi övervägas.
Inläggning på sjukhus• Livshotande blödning, oavsett TPK-värde:
Inläggning rekommenderas absolut• TPK 30 x 109/L utan symtom eller endast
lindrig purpura: Inläggning rekommenderas ej.• För övriga patienter kunde man ej enas om
någon rekommendation. I det enskilda fallet måste hänsyn givetvis tas till barnets följsamhet till behandling, avstånd till sjukhus samt till föräldrarnas önskan.

• Information!! Avoid NSAID, traumatic procedures. Hospitalization and helmet of no use.
• Wait and see in many (most?) patients• Drug treatment: Which patients?
• clinical bleeding tendency most important• Patients with ongoing wet bleeding usually treated• take platelet count into account?• If severe bleeding in ITP:
– consider other diagnoses!– anticipate PROBLEMS, be active !!
Drug treatment of acute ITP
• Option 1: Intravenous immunoglobulin• Dose 0.8 – 1.0 g/kg x 1 – 2 days• Effective. Expensive. Potentially serious side effects. No BM
examination required.
• Option 2: Steroids• Prednisolone 2 – 4 mg/kg/d 1 – 2 weeks, tapering week 3
» Alternative Prednisolone 4 mg/kg/d i 3 days.» Alternative Methylprednisolone i.v., Dexamethasone i.v./po
• Nearly as effective. BM required before start of therapy.
Treating severe bleedings
• Problem: patients with major hemorrhage respond less effectively to treatment
• (Medeiros MD, Buchanan GR. J Pediatr 1998; 133: 334-9)
• Life threatening hemorrhage: • Combination of i.v. steroids – IVIG – platelet
transfusions - cyclocaprone

• Isolated, thrombocytopenia (PLC < 30 in 90%)
• ”Healthy” child, minor purpura• Normal HB and WBC• Physical examination OK apart from
bleedings• If treated: Good respons to steroids or IVIG• Bone marrow: Normal or increased

ITP: Exclusion diagnosis
• … the child is sick: Fever, pallor, bone- or joint pain
• … you find enlarged lymph nodes, liver or spleen• … there are major bleedings• … the PLC is > 30 x 109/l• … other cell lines are affected• … treatment with immunoglobulin or steroids is
ineffective
Consider other diagnoses if…
…in that case: (try to) think, do more investigation!
• if ITP strongly suspected• Full blood count, peripheral blood smear,
reticulocytes• CRP, transaminases
• if doubt about diagnosis ITP:• Exclude viral infections: HIV, CMV, EBV• Platelet antibodies ?? Not as routine screening. Low
specificity and sensitivity• Coagulation tests • Coombs test, ANA, DNA antibodies• Bone marrow!

• Chronic course (PLC < 150 after 6 months) in 25%

Bone marrow in ITP• Advantages:
• safe diagnosis• Acute leucemia and aplastic anemia excluded
• Disadvantages:• general anesthesia, possible bleeding

• if atypical features• before steroid treatment• if no remission after 2-3 weeks• in chronic ITP

• Choice of drug in 283 children:• 217 (76.7%) IVIG• 17 (6.0%) corticosteroids• 49 (17.3%) IVIG + steroids
Discussion between ”activists” and ”non-interventionists” :
• How dangerous is thrombocytopenia in ITP?
Much less bleeding tendency in ITP than in aplastic diseases
• Risk of ICH 0.1-1 %• But: Simple and effective (for a certain
time…) drug treatment
