An Update in Periodontal Medicine: Managing Oral Infections to Improve Overall Health

PeriodontalMedicinewasdefinedin1996astheeffectofperiodontalinfectionsonsystemichealth.Thisdisciplinewasinitiallyestablishedtostudytheobservationsthatlinkedperiodontaldiseasewithanincreasedriskforheartattack,strokeandpretermdeliveryamongpregnantwomen.Althoughthisisanemergingfield,currentfindingssuggestthatperiodontitisisassociatedwithmoressevereandprevalentcardiovasculardisease,kidneydisease,chronicobstructiveairwaydisease,diabetesandpregnancycomplications.Theseassociationsappeartoconferadditionalriskevenafteradjustmentsforrelevantconfounderslikesmoking,obesity,andothertraditionalriskfactors.Currentevidencesuggestthattheperiodontalorganismsgainaccesstothesystemiccirculationandthatperiodontalinfectionscreateanoralwoundthatservesasaportalforthesystemicdisseminationoforalpathogens.Onceintothebloodstreamtheseorganismselicithepaticinflammation,vasculardamage,andmanyappearcapableofcrossingtheplacentalbarrierinpregnantwomen.Thus,clinicalperiodontaldiseaserepresentsaninfectedwoundthatcanserveasachronicportalforthesystemicdisseminationoforalpathogensthathasthepotentialtomediateinflammatorydamagetomultipleorganssystems.Theevidencefromhumansandanimalstudieswouldsuggestthatonceperiodontalorganismsenterintothecirculatorysystem,theyinduceupstreamanddownstreammediatorsofthehepaticacutephaseresponseresultinginincreasesinserumbiomarkerssuchassICAM(solubleintracellularadhesionmolecule-1),IL-6(interleukin6),fibrinogenandCRP(C-ReactiveProtein).Manyoftheseinflammatorymarkersarepredictiveofcardiovascular,pregnancyanddiabetesrisk.Itappearsthat,periodontaldiseasemay,inpart,explaintheassociationbetweentheobservedincreasesininflammatorybiomarkersoftheacutephaseresponse,coagulationandvascularstressasassociatedwithincreasedriskforcertainsystemicconditions. Duringpregnancy,periodontalinfectionalsoappearstobeaspecialthreattotheplacentaandfetus.Case-controlandprospectivecohortstudieshavesuggestedthatthereisasignificantassociationbetweenmaternalperiodontaldiseaseandpregnancycomplicationsthatresultinpretermdelivery[gestationalage(GA)<37weeks]andlowbirthweight[LBW,birthweight<2500g].Severalstudiessuggestthatperiodontaldiseaseindependentlyenhancestheriskofobstetriccomplicationsincludingpretermbirth,growthrestriction[birthweightlessthan10thpercentileofweightforgestationalage,adjustingforbabygenderandrace],preeclampsiaandverypretermbirth[deliverylessthan32weeksgestation].Periodontaldiseaserepresentsariskfortheseadversepregnancyoutcomesevenafteradjustingforarangeofpotentialconfoundersincludingprevioushistoryofpretermbirth,parity,maternalweightgain,smoking,andotherinfections.Datasuggestthatmaternalperiodontalinfectionsmaypotentiallyrepresentabonafideriskfactorforpretermbirthandgrowthrestrictionbyservingasaninfectiousandinflammatoryexposuretotheplacentaandfetusduringpregnancy.Thisinfectiousandinflammatorystresswhichrepresentsthetoxicityoftheoralbiofilmappearstoincreaseneonatalmorbidityandmayhavelife-longeffectsonthehealthoftheoffspring. Estimatesofthepotentialmagnitudeoftheeffectofperiodontaldiseaseontheseconditionsaresignificant,increasingtheriskasmuchastraditionalriskfactorssuchassmoking.Furthermore,datafromanimalmodelsofdiseaseprovideacompellingdemonstrationofbiologicalfeasibility.However,datasupportingthereversibilityofthisassociationinhumansisequivocal.Althoughsomeearlytreatmentstudiesshowpromise,otherstudiesdonotshowanysystemicbenefitsfromtreatment.Clearlyadditionalstudiesexploringthepotentialbenefitsoftreatingperiodontaldiseasetoreducingtheriskforthesesystemicconditionsareneeded.However,sinceperiodontaldiseaseisbothapreventableandtreatablecondition,itrepresentsapotentiallymodifiableriskfactor.Thus,ifperiodontaldiseaseisproventobecausallyrelatedtotheseconditions,thefindingscouldhaveatremendoushealthcareimpact.

Oneofthekeyfeaturesofperiodontaldiseasesistheintimatecontactbetweenthebiofilm

ofthemicrobialplaqueandtheulceratedepitheliumoftheperiodontalpocket.Thisallows

thesubgingivalbacteria,someofwhichhavetissue-invadingproperties,togaindirectaccess

totheunderlyingconnectivetissueandgingivalmicrovasculaturethroughthedisrupted

epithelialbarrier.Asaresult,bacteremiasarefairlycommoninpatientswithperiodontitis

andaretriggeredbymechanicalstimulationofthegingivaltissues.Inaddition,inflammatory

mediatorsthatareabundantlyproducedlocallyintheinflamedgingivathroughcellularinnate

andadaptiveimmunitypathwayscanenterthebloodstream,reachdistantorgansandalso

‘excite’thevascularendothelium.Thesephenomenahavebeenshowntobeimportantinthe

contextofatherogenesis.Thepresentationwillgiveanoverviewofthepotentialbiologically

plausiblemechanismsthroughwhichperiodontalinfection/inflammationmayleadtoextra-oral

pathology,andsummarizetheavailableepidemiologicevidencefromcross-sectional,cohortand

interventionstudiesthathaveinvestigatedtheassociationofperiodontitisandcardiovascular/

cerebrovasculardisease.Thepublichealthimplicationsofthesefindingswillbediscussed.

The Role of Host Response Variations in Oral and Systemic Disease Interactions

Variationsinhostresponseamongsthumansarecriticalindeterminingwhogetsdiseaseandwhoremainshealthygiventhesameenvironmentalchallenges.Clearlyinitiatingandenvironmentalchallengesarenecessaryinanydisease,andinperiodontaldiseasethesubgingivalbiofilmandchallengessuchassmokingcanfulfilltheseroles.Thislecturewillexploretherelationshipbetweenoraldisease,specificallyperiodontaldisease,andotherchronicinflammatorydiseasessuchasdiabetes,heartdiseaseandpre-termpregnancies.Thecentralroleofthechronicinflammatoryburdenasalinkingfeaturebetweenthesediseasesandthespecificpotentialeffectoftheperiodontalbiofilmasthemajoretiologicalagentwillbeaddressed.Mechanisticlinks,particularlyforcardiovasculardisease,andtheconceptofsharedriskfactorswillbecoveredalso.Thegatekeeperroleofoursurfaceepitheliallayeranditsvariationswillbeexploredinbothitsmolecularvariationsandfunctions.StimulationofTLRsbyperiodontalbacteriatypicallyproduceseitherahyper-inflammatoryresponseorananti-microbialpeptideresponse.Theseresponsesareunderstoodtoberegulatedbyepigeneticmodifications.LikewisethechangesincellularreceptorexpressionaremediatedbymicroRNAswhichmayalsohavearoleinapoptoticresponses.Featuressuchasthelipidraftcellularreceptors,inparticulartheTLRfamilyaswellasGPCRswillbereviewedaswillthepotentialforsignalingthroughthesereceptorsinproducingcytokine,andothercellularinflammatoryandanti-inflammatoryresponses.Theimportanceofgenetics,specificallybySNPcarriage,andtheregulationofinflammationbybothmicroRNAaffectsandbychromatinremodelingandotherepigeneticchangeswillbementioned.Thecentralroleofchronicinflammationandhowitcontributesandisworsenedinavarietyofstateswillbeconsidered.Theaudienceshouldachieveanunderstandingandarealisticperspectiveoftheinteractionsofthesechronicdiseasesandknowledgeontheirprevention.

The Periodontal Host Response with Type 2 Diabetes

Inthepastdecade,periodontaldiseasehasbeenrecognizedasnotmerelyalocalinfectiousdisease,butaschronic,subclinical,inflammatorydiseaseforthehost.Thesubjectswithtype2diabetesappeartorespondtobacterialchallengeinanexaggeratedmannerascomparedwiththesubjectswithoutdiabetesthroughseveralpossiblemechanisms,anddevelopsevererformsofinflammatoryperiodontaldisease.Severeperiodontaldiseaseinsuchsubjects,inturn,actstoreduceinsulinsensitivityknownasinsulinresistance,therebycontributingtotheinductionofhyperglycemiaaswellashyperinsulinemia,importantriskfactorsforvascularcomplications.Additionally,recentstudiessuggestedthatsuchsubclinicalinflammatorystatealsopromotesrenaldysfunction,allofwhichareimportantriskfactorsforatherosclerosis.Finally,allsuchconditionsacttoincreasetheriskforcoronaryheartdiseases,oneoftheleadingcausesofmortalityandmorbidityinsubjectswithdiabetes.Thus,itisveryimportanttoelucidatemolecularmechanismsastowhylocalperiodontalinflammationisamplifiedtothelevelofinfluencingouroverallsystemichealth. Recently,macrophageshavebeensuggestedtoinfiltrateintoadiposetissues,andtointeractwithadipocytes,therebyexacerbatingadiposetissueinflammation.Furthermore,bothcelltypesappeartoexpresstoll-likereceptor-4(TLR4),andfreefattyacidshavebeenfoundtoactasendogenousligandforTLR4.Basedonthesefindings,wehypothesizedthat,incasesofinfectiousdiseasessuchassevereperiodontaldiseaseandgutinfection,classicalexogenousligandforTLR4mayfurtherexacerbateinflammatoryresponsesinadiposetissue,therebycontributingtotheinductionofmanyunwantedsideeffectssuchasinsulinresistance.Toprovethis,weestablishedco-culturesystembetweenadipocytesandmacrophagesandstimulatedthesecellswithbacteriallipopolysaccharide(LPS).WefoundthatstimulationofthecellswithLPSmarkedlyup-regulatedinflammatorygeneexpressioninadipocytesaswellasproteinproductionsfromco-cultures.Someoftheseeffectswereconfirmedinvivomodelbyusingbothgeneticallyinducedandenvironmentallyinducedobesemodelmiceaswell.Hence,ourresultssuggestthat,althoughtheprevalenceofextremelyobesesubjectsasseeninwesternsocietiesisverylowineasternAsiaincludingJapan,oncetheydevelopsevereperiodontaldiseases,thediseasemayup-regulateadiposetissueinflammation,especiallyinsubjectswithnewlydiagnosedtype2diabeteswhoareslightlymoreobesethanthesubjectswithoutdiabetes.Currentunderstandingonthesemechanismswillbediscussedinthecontextofdevelopingnew

“order-made”diagnosticstrategiestoavoidsuchunwantedsideeffects.

Epidemiologicalstudieshavesuggestedtherelationsamongmanysystemicdiseasesandperiodontitis.Inthosestudies,clinicalindiceshavebeenusedasparametertoshowseverityofperiodontitis.Whatisreallyinvolvedinthepathogenicityofthesystemicdiseasesthatperiodontitisrelates?Directpathogenicityofperiodontalbacteriaseemstobeasmallportionofthepathogenicitybetweenthesediseases.Hostresponsestothebacteriaordenaturedhostproteinsmustbealargeportionofpathogenicitysuchasimmuneresponsestomoleculesmimickedtothebacterialantigensorinvolvedininflammation. Forfurtherunderstandingtheseverityofperiodontitis,wehavebeenusedbothbacterialDNAtoidentifyspecificperiodontalbacteriaandimmunoglobulinG(IgG)todeterminethedegreeofsensitizationbybacterialantigens.IncreasedIgGantibodylevelmeansactivatedacquiredimmuneresponseandfurtherhostresponsethatmaycausefailureofhomeostasis,resultingtissueororgandamages.Realpathogenicityhasnotbeenwelldocumented. However,usingthesetwoparameters,wecanunderstandthehostresponsesofperiodontitispatients.Therearethreegroups;1)majorgroupwithregularIgGresponsereflectingtheseverityofperiodontalinfection,2)minorgroupwithhypo-response(lowIgGlevel)tosevereperiodontalinfection,3)minorgroupwithhyper-response(highIgGlevel)tolessperiodontalinfection. Inthefirstgroup,IgGantibodylevelchangesaccordingtotheamountofbacterialDNAdetectedfromperiodontallesion.Thus,itiseasytoinferaboutthesensitizationofhostfromtheselaboratoryexaminations.Furthermore,thisgroupmaybeclosetothosewhoseperiodontalconditionsrelatedtothesomesystemicdiseases.Studiesusingthisassumptionhavebeenconducted,suggestingrelationsbetweenperiodontitisandsystemicdiseasesomehow.Inthesecondgroup,IgGantibodylevelstayslowerleveleventhoughalotofamountofbacterialDNAdetectedfromperiodontallesion.Thehostmaybeinimmuno-compromisedcondition.Bacteremiaoraspirationpneumoniamustbeconsidered.Inthethirdgroup,IgGv\levelstayshigherleveleventhoughverylowamountofbacterialDNAdetectedfromperiodontallesion.Hyper-immunoglobulinemiathatmayrelatetoinjuryofglomerulusmustbeconsidered.Inaddition,excessiveimmunoreactionmayexistandinjuretissuesororgans.Becausefewstudiesusingthisassumptionhavebeenconducted,furtherstudiesmustbeperformedforthesesystemicdiseases. Inthislecture,Iwillshowourrecentstudiesasexamplesfortheideamentionedabove.ThedefinitionofthresholdofIgGtiteragainstPorphyromonasgingivalis ,utilizationoftheIgGtitertesttoexplainchronicobstructivepulmonarydisease,preliminarystudytorevealtherelationoftheIgGtiterandatherosclerosisrelatedtodiabetes.

Association between Periodontal Disease andAdverse Pregnancy Outcome

Periodontaldiseaseisaninfectiousdiseasewithacertainextentofinflammatorylesionintheperiodontiumwherenumbersofmicroorganismsandinflammatorycellsareincontactandvarietiesofinflammatorymediatorsareproducedand/orinfiltratedin.Chronicperiodontitisbecomesprominentatlateragesofadulthood,butgingivitisproceedsattheiryoungerages.Adversepregnancyoutcomeincludespre-termbirth(PB:deliveryearlierthan37weeksofgestation)andlowbirth-weight(LBW:deliveryweightlessthan2,500g).IncidencerateofPLBW(PBand/orLBW)arereportedtobe4~15%andvariesamongdifferentcountriesanddistricts.PLBWisthemaincauseofmortalityandmorbidityofthenew-bornandbeanineligibleburdennotonlyfromfamilialbutalsofromsocioeconomicaspects. Bothperiodontaldiseaseandadversepregnancyoutcomearemulti-factorialinitspathogenesisandpossesscommonriskfactorssuchassmoking.Sincethemiddleof1990`s,anumberofepidemiologicalandexperimentalstudieshavebeenperformedonthetopicoftheassociationbetweenperiodontaldiseaseandPLBWandtheresultswerereportedintheliterature.Suchstudiescanbecategorizedintothreegroupsaccordingtotheaimsofthestudies,i.e.analyzingtheassociationbetweenthebothevents,analyzingthemechanismsbehindtheassociation,andevaluatingtheeffectsofperiodontalinterventionontheassociation.Althoughtherehavebeensomewhatlargediscrepanciesintheresultsoftheassociationstudies,significantpositiveassociationshavebeenreportedinmorethanhalfofthelargescaledepidemiologicalstudies.Mechanismsoftheassociationhavebeenanalyzedwithemployingmicrobiological,immune-pathological,andgenomicmethods.Sofar,therehavebeenseveralstudiesshowinginterestingfindingswhichmayinpartexplainthemechanismsoftheassociation.ItcouldbeconcludedthatthebiggestcontroversieswerefoundintheperiodontalinterventionstudiesandthatperiodontaltreatmentswouldnotberecommendedduringthepregnancyinordertoreducetheincidencesofPLBWaccordingtotheresultsofthelargescaledRCTs.However,ithasalsobeenclaimedthatperiodontaltreatmentduringthepregnancyinitselfisbeneficialinordertotreatperiodontaldiseaseandmaintainhealthierperiodontiumofthepregnantwomen.ItwasalsodemonstratedthatunsuccessfulperiodontaltreatmentsresultedinhigherrisksofPLBWthansuccessfulperiodontaltreatmentsinastudy.Inthispresentation,associationbetweenperiodontaldiseaseandadversepregnancyoutcomewillbediscussedfromthethreeaspectsmentionedabove.

Interrelationships between Systemic Osteoporosis and Periodontal Disease : Association and Mechanisms

Manystudieshaveattemptedtodefinetherelationshipbetweenpostmenopausalosteoporosis

andperiodontaldisease.Moststudiessupportapositiveassociationbetweenthesecommon

diseases;however,manyarecross-sectionalinnature,includerelativelysmallsamplesizes,and

haveinadequatecontrolofpotentialconfoundingfactors,suchasage,gender,hormoneintake,

race,andsmoking,limitingourunderstandingofthenatureoftherelationshipbetweenthese

diseases.Clinicalconditionscausinglowestrogenenvironmentsinpostmenopausalwomen

allowT-andB-cellabnormalities,increasedlocalproductionofthebone-activecytokines(i.e.,

Interleukin-1，-6and-8,tumornecrosisfactor-(TNF-)andariseinprostaglandinE2,resulting

intheprogressionofperiodontitis.Inpost-menopausalosteoporosis,lackofestrogenwillaffect

theremodelingofthebonetissueinsuchawaythat,inmostpatientswithperiodontitis,the

amountofboneresorbedexceedsthatbeingformed,resultinginnetboneloss.Osteoporosis

canbetreatedbyavarietyofmethods,thehormonereplacementtherapy(HRT),theselective

estrogenreceptormodulators(SERM)andthebisphosphonates.TheHRTorbisphosphonates

treatmentsimprovetheclinicaloutcomeofperiodontaldiseaseandmaybeanadjunctive

treatmenttopreserveperiodontalbonemass.Thispresentationreviewsthecurrentevidence

onthemechanismofperiodontalbreakdownaftermenopausewithlong-termfollow-upcases

andthebenefittooralhealthbytreatmentsforosteoporosis.

Is There a Role for Stem Cells in Periodontal Regeneration?

Althoughstemcellshavereceivedconsiderableattentioninrecentyears,clonogenicbonemarrowstromalstemcells(alsoknowasmesenchymalstemcells)werefirstisolatedandcharacterizedalmost40yearsago.Sincethenconsiderableefforthasbeenmadeindeterminingthetherapeuticapplicationsofmesenchymalstemcells.Mesenchymalstemcellpropertiessuchasmultipotencyandimmunomodulationmakethesecellsidealcandidatesfortissueregenerationandtissueengineering.Mostofusarefamiliarwiththeconceptofembryonicstemcellsandtheirpotentialuseintissueregeneration.Overthepast10years,reportshaveappearedcharacterizingmesenchymalstemcellsisolatedfromtheperiodontaltissues(periodontalligamentandgingiva)andtheirpotentialuseinperiodontalregeneration.Morerecentlyreportsofisolationofinducedpluripotentstem(iPS)cellsfromgingivaandperiodontalligamenthaveappeared.BothmesenchymalstemcellsandiPScellsfromperiodontaltissuesprovideanattractivesourceofcellsforperiodontaltissueengineering.Itisnowclearthatinorderfortissueengineeringtoreachitsfullregenerativepotentialanappropriatescaffoldforcelldeliverytogetherwiththeincorporationofsupplementalcytokinesandgrowthfactorswillbenecessaryfortheregenerativeprocesstoproceedbothspatiallyandtemporally.Thepreciserolesofmesenchymalstemcellsintissueregenerationstillneedtobeelucidated.Forexample,itisstillunclearwhetherthesecellsactviadirectdifferentiationinsituorthroughparacrinemediatedprocessesleadingtorecruitmentoflocalprogenitorcellstoenabletissueregeneration.Inthispresentationthebiologyandpotentialforclinicaluseofperiodontalstemcellsforperiodontalregenerationwillbeexplored.

Ithasbeendemonstratedthatmesenchymalstemcellsandprogenitorcellsofosteoblastsorcementoblastscanbeidentifiedwithinperiodontalligament(PDL).Thus,improvingthebiologicalpotentialofthesecellsandstimulatingtheperiodontalregenerationarerecognizedasbeingclinicallypossible.Oneofthemostphysiologicallyefficientmethodstostimulatethesecellsistheuseofcytokines. BasisFibroblastGrowthFactor(FGF-2)isknowntobedeeplyinvolvedintheproliferation,migrationanddifferentiationofavarietyofcellsandtostronglyinduceangiogenesis.Utilizingbeagledogsandnon-humanprimates,werevealedthattopicalapplicationofrecombinantFGF-2inducedstatisticallysignificantperiodontaltissueregenerationintheexperimentally-preparedintraosseousbonedefects.Recently,ahumanclinicaltrialwasconductedusingFGF-2inJapan.Thiswasarandomizedcontrolleddouble-blindedclinicaltrialofdoseresponsesincludingplacebocomparison.Asaresult,asignificantdifferencein%increaseinalveolarboneheightat2-or3-walledintrabonydefectsofthepatientswasdemonstratedbystandardisedradiographsbetweenPlaceboGroupand0.3% -FGF-2Groupat9monthsafterthetreatment.ThissuggeststhattopicalapplicationofFGF-2canbeefficaciousinregenerationofperiodontaltissueofperiodontitispatients. Basedontheresultsofaseriesofinvitro analyses,wehavesuggestedthefollowingmodeofactionofFGF-2toinduceperiodontalregeneration.Duringtheearlystagesofperiodontaltissueregeneration,FGF-2stimulatestheproliferationandmigrationofPDLcellswhilemaintainingtheirmultipotentnature,inducingdifferentiationintohardtissue-formingcellssuchasosteoblastsandcementoblasts.Furthermore,FGF-2inducesangiogenesisandincreasesextracellular-matrixproductionsuchasosteopontin,hyaluronanfromPDLcells,thusleadingtoalocalenvironmentsuitablefortheperiodontalregeneration.TheseresultsintheenhancedperiodontaltissueregenerationareshownattheFGF-2-appliedsites. Foridealperiodontalregeneration,itiscrucialtofullyintroducetheconceptof“tissueengineering”.IfweneedtotreatseverebonydefectsorhorizontalbonedestructionwithFGF-2,itisessentialtointroducetheconceptofa“scaffold”intothecarrierofFGF-2drug.AnFGF-2carrierthatcouldprovideaformableandosteoconductivescaffoldforundifferentiatedprogenitorcellswithinPDLwoulddramaticallyincreaseboththedentalandcraniofacialapplicationsofFGF-2drug. Furthermore,researchers,includingourgroup,havefoundthatmesenchymalstemcellscanbeobtainedfromvarioustissuessuchasbonemarrowandadiposetissues.Byusingbeagledogmodels,werecentlyrevealedthattransplantationofadipose-tissuederivedstemcellsenhancesperiodontalregenerationatappliedsites.Thecombinedeffectsof‘celltherapy’and‘cytokinetherapy’needtobeassessedinthefuture,toallowtheestablishmentof‘periodontaltissueengineering’.

Clinical Application of Er:YAG Laser in Periodontal and Peri-implant Therapy

Recently,variouslasershavebeenusedforawiderangeoforalconditions.Lasershavenumerousadvantageousphysicalpropertiessuchasablation,hemostasis,bactericidaleffectaswellasphoto-bio-modulation,makingitsuitablefortreatinginflammatoryandinfectiousconditions.Thus,theuseoflasersisconsideredsafeandeffectivefortreatingoraldiseasessuchasperiodontitisandperi-implantitis.

Amongvariouslasersystems,inparticular,therecentdevelopmentoftheEr:YAGlaserhasexpandedtheapplicationoflaserstohardtissuetreatmentinperiodonticssincethe2.94µmwavelengthcanbeappliedonbothdentalsoftandhardtissues.TheEr:YAGlaserhasvariouscharacteristicsadvantageousforsoftandhardtissuemanagement,suchaseasyandpreciseablation,cutting,debridementandrecontouringwithminimalthermaldamagetothesurroundingtissuesunderwaterirrigation.Nowadays,theEr:YAGlaseriseffectivelybeingusedforvarioustissuemanagementssuchasgingivaltissueablation,dentalrootandimplantfixturesurfacedebridement,andosseoussurgery.

Inperiodontalandperi-implantapplications,theEr:YAGlaseriscapableoftreatingalveolarbonetissueeitherdirectlyorindirectlyduringosseousmanagement.AnimalstudiesreportedfasterwoundhealinginbonetissuefollowingEr:YAGlaserablationcomparedtoburcutting,aswellassignificantlyenhancedandfavorablenewboneformationfollowingsurgicaltreatmentofperiodontitisorperi-implantitisusinganEr:YAGlasercomparedtomechanicalinstrumentation.ThesepositiveresultsareconsideredtobeduetothehighdecontaminationanddetoxificationeffectsoftheEr:YAGlaseronthediseasedsite,thepronouncedbleedingfromthelaser-treatedbonesurface,aswellastheproductionofcharacteristicmicrostructuresontheablatedboneandrootsurfaces,whichpossiblyenhancesfibrinandbloodclotretention.Inaddition,potentialphoto-bio-modulationeffectsoflow-levellasereffectwhichinducesavarietyofbiologicalresponsesmaybepartlyinvolvedintheincreasedboneformation.Giventheseobservationsandevidences,theEr:YAGlaserhasbeenrecentlyestablishedasoneofthemostpromisinglasersystemsforperiodontalandperi-implanttherapy.

Inthispresentation,thecurrentclinicalapplicationsofEr:YAGlaserforvarioustreatmentssuchassofttissuemanagement,non-surgicalandsurgicalperiodontaltreatment,osseoussurgeryandperi-implanttherapywillbediscussed,basedonscientificevidencefromcurrentlyavailablebasicandclinicalstudiesaswellasclinicalcases.
