Nghiên cứu hiệu quả của stent tự tiêu đối với bệnh mạch máu vùng dưới gối:

Stent phủ thuốc được sử dụng là một thành tựu về mặt kỹ thuật và độ bền
trong điều trị bệnh lý mạch máu cẳng chân. Nhưng vấn đề đặt ra là sự tồn tại vĩnh
viễn của giá kim loại, điều này sẽ gây ra tổn thương những mạch máu lân cận.
Với giả thuyết đặt stent sinh học tự tiêu cho các tổn thương mạch máu vùng
cẳng chân là an toàn và hiệu quả về lâu dài, một nghiên cứu được thực hiện bởi
Ramon L. Varcoe, MBBS, MS, PhD với mục tiêu là khảo sát hiệu quả trung hạn
của stent sinh học phủ thuốc - tự tiêu trong của nghiên cứu trong điều trị các tổn
thương mạch máu vùng chày và kheo. Kết quả cho thấy với thời gian nghiên cứu
trung bình là 12.0 ±3.9 tháng, có 33 bệnh nhân vơi 38 chi được can thiệp, trong đó
68.4 % bị thiếu máu cấp tính và 31.6% bị đau cách hồi nặng. Có 43 đoạn mạch
được đặt 50 stent sinh học phủ thuốc tự tiêu với chiều dài trung bình của stent là
19.2± 11.6 mm. Trong quá trình theo dõi này có 5 bệnh nhân tử vong, nhưng toàn
bộ số bệnh nhân còn lại đều được theo dõi. Trong số 38 chi được can thiệp: thì số
lượng được cải thiện lâm sàng là 30/38( chiếm 79%). Số lượng tái hẹp có ý nghĩa
trong stent là 3/ 50( chiếm 6%). Sử dụng phương pháo phân tích Kaplan Meier
cho thấy tỷ lệ thông mạch tương ứng là 96% và 84.6% sau 12 và 24 tháng điều trị.
Tỉ lệ lành vết thương ở những bệnh nhân bị mất mô là 64% và không có bệnh
nhân nào bị đoạn chi với tỉ lệ 100% bảo tồn. Và chỉ có 6% bệnh nhân bị tái hẹp
trong stent khi theo dõi tiếp sau đó. Từ những số liệu này, các nhà nghiên cứu
chứng minh cho việc an toàn tuyệt vời và rõ ràng của việc giải thoát khỏi những
tổn thương khi tái thông mạch máu bằng cách sử dụng stent sinh học phủ thuốc –
tự tiêu đồng thời đây cũng là một tiềm năng để trở thành phương thức điều trị dành
cho những bệnh lý mạch máu vùng cẳng chân.

Study: Absorb Stent Works Below the Knee
'Excellent' results but only in unusually focal lesions by Nicole Lou

Implanting the Absorb bioresorbable vascular scaffold in focal below-the-knee
lesions was safe and effective in the long run, investigators suggested.
Technical success was achieved in all cases. And the majority of tibial and distal
popliteal lesions treated with the everolimus-eluting disappearing scaffold
improved at 1 year (79%), Ramon L. Varcoe, MBBS, MS, PhD, of Australia's
Prince of Wales Private Hospital, and colleagues reported online in JACC:
Cardiovascular Interventions.
On Kaplan-Meier analysis, primary patency was observed in 96% and 84.6% of
lesions at 12 and 24 months, respectively; freedom from clinically-driven target
lesion revascularization was 96% at both time points. There was a 6% rate of
binary restenosis over follow-up, the authors found.
They concluded that the numbers "demonstrated excellent safety, patency, and
freedom from target lesion revascularization using the Absorbbioresorbable
vascular scaffold below the knee."
"It has significant potential to become the favored class of therapy in this group of
patients," they wrote.
"We congratulate the authors for reporting the long-term clinical outcome of their
experience with coronary scaffolds in below-the-knee interventions; their
restenosis rate, together with the appropriate duration of clinical follow-up,
actually represent a proof of concept of the great potential of vascular scaffolds in
peripheral interventions," Antonio Micari, MD, PhD, and Roberto Nerla, MD, both
of Maria Cecilia Hospital in Italy, wrote in an accompanying editorial.
Varcoe's study included 33 patients with critical limb ischemia (68.4%) or severe
claudication (31.6%). Operators treated 43 lesions with 50 Absorb scaffolds in
total; five patients died over the follow-up period, leaving 38 treated limbs
available for analysis.
Importantly, the investigators selected highly focal lesions for their study. The
average lesion clot was just 19.2 mm long.
"This makes it impossible to derive reliable considerations about the usefulness
and the real impact (in clinical practice) of these devices on below-the-knee
procedures, given that infrapopliteal disease is known to be more diffuse and
complex in a real-world setting," according to Micari and Nerla, who suggested
that future studies include a broader population.
Varcoe declared relationships with Abbott Vascular, Medtronic, Boston Scientific,

Abstract
Objectives The aim of this study was to investigate the midterm performance of
an everolimus-eluting, bioresorbable vascular scaffold (Absorb, Abbott Vascular,
Santa Clara, California) for the treatment of focal tibial and distal popliteal lesions.
Background Drug-eluting stents are used below the knee to improve technical
success and durability, but the ongoing presence of a permanent metal scaffold
may have deleterious effects on the local vessel.
Methods Tibial and distal popliteal angioplasty with scaffold placement was
performed using an everolimus-eluting, bioresorbable scaffold (Absorb). Clinical
and ultrasound follow-up was performed at 1, 3, 6, 12, and 24 months to detect
binary restenosis and evaluate safety, restenosis, and clinical improvement.
Results Thirty-eight limbs in 33 patients were treated for critical limb ischemia
(68.4%) or severe claudication (31.6%). Fifty scaffolds were used to treat a total of
43 lesions, with a mean length of 19.2 ± 11.6 mm. During a mean follow-up period
of 12.0 ± 3.9 months, 5 patients died, and all others were available for follow-up.
Among the 38 treated limbs, clinical improvement was present in 30 (79%). Binary
restenosis was detected in 3 of 50 scaffolds (6%). Using the Kaplan-Meier method,
rates of primary patency were 96% and 84.6% at 12 and 24 months, respectively,
and rates of freedom from clinically driven target lesion revascularization were
96% and 96% at 12 and 24 months, respectively. Complete wound healing
occurred in 64% of those treated for tissue loss, with no major amputation and a
limb-salvage rate of 100%.
Conclusions Twelve-month follow-up demonstrated excellent safety, patency,
and freedom from target lesion revascularization using the Absorb bioresorbable
vascular scaffold below the knee.
Perspectives
WHAT IS KNOWN? DES are superior to both angioplasty and bare-metal stents
in the interventional treatment of short tibial artery lesions, but the permanent
metallic implant has several disadvantages.
WHAT IS NEW? Our early results using a drug-eluting, bioresorbable scaffold
for these lesions suggest that they have patency rates at least equivalent to metal
DES, with inherent advantages related to their resorption.
WHAT IS NEXT? Further trials are required with larger numbers, the
involvement of multiple centers, and control groups to compare bioresorbable
scaffolds with conventional therapy
