สาเหตุที่ทำให้เกิดภาวะแทรกซ้อนทางหัวใจในผู้ป่วยผ่าตัดกระดูกและข้อ

1. ผู้ป่วยสูงอายุและมักมีโรคประจำตัวหลายโรค

2. มี functional capacity ไม่ดี

3. การผ่าตัดกระดูก บางอย่างทำให้เกิดภาวะ systemic inflammatory response

4. มีการสูญเสียเลือดในการผ่าตัด

5. ภาวะเจ็บปวดหลังผ่าตัด

ทำตามแนวทาง ของการประเมินคนไข้ก่อนผ่าตัด, ไม่ควรหยุดยาบางชนิดถ้าไม่จำเป็นเช่น aspirin,statin,beta blocker

เฝ้าระวังภาวะแทรกซ้อนทางระบบหัวใจหลังผ่าตัดโดยเฉพาะโรคหัวใจขาดเลือด

Respiratory Complications

• ผู้สูงอายุมักมีความเสื่อมของอวัยวะต่างๆหลายระบบรวมถึงระบบหายใจ

• ผู้ป่วยสูงอายุการทำงานของปอดที่เสื่อมลงเกิดขึ้นจากมีการเพิ่มขึ้นของ closing volume และ FEV1 ลดลง 10% ทุกๆ 10 ปี

• ในผู้ป่วยที่ได้รับการผ่าตัดกระดูกอาจมีเศษกระดูกหลุดเข้าไปในกระแสเลือดไป อุดตันเส้นเลือดที่ปอดและทำให้ระดับออกซิเจนในเลือดต่ำลงจนเกิดภาวะแทรกซ้อนได้

ผู้ป่วยบางรายอาจมีภาวะหยุดหายใจขณะหลับร่วมด้วย ซึ่งมักพบร่วมกับภาวะนอนกรนและผู้ป่วยน้ำหนักมาก ทำให้การผ่าตัดที่ต้อง

Neurological Complications

• early mobilization

• adequate pain control

• maintenance of normal sleep cycles

• avoidance of psychotropic medications

FAT EMBOLISM SYNDROME

• neuraxial techniques do not play a role in the development of back pain after delivery

NAUSEA AND VOMITING

• direct exposure of the chemoreceptive trigger zone in the brain to emetogenic drugs (e.g., opioids)

• hypotension associated with generalized vasodilation

• gastrointestinal hyperperistalsis secondary to unopposed parasympathetic activity

COMPLICATIONS

URINARY RETENTION

• one third of patients after neuraxial anesthesia

• blockade of the S2, S3, and S4 nerve roots inhibits urinary function as the detrusor muscle is weakened

• administration of intrathecal morphine is strongly associated with this complication

COMPLICATIONS

PRURITUS

• most common side effect related to the intrathecal administration of opioids, with rates between 30% and 100%
