Slogan and Poster Design Competition 2017

EligibilityParticipant should be an eligible PolyU student or staff on the date of prize presentation, i.e. 13 October 2017 (Friday).

Entry Requirements1. Slogan and poster must be consistent with the theme of promoting flu vaccination.2. Slogans should be written in both English and Chinese. 3. The poster should be designed in A2 size with PDF format not exceed 25MB.

PrizesChampion: Smart Wristband智能運動手錶1st runner-up: Body Composition Monitors體重脂肪測量噐2nd runner-up: Personal-sized blending jar 便攜果汁機(Prizes may be changed without prior notice)

Judging PanelThe judging panel will be composed of members of the Health Education Team of UHS. (The composition of the judging panel may change without prior notice.)

Judging CriteriaEntries will be assessed according to the following elements: 1. Creativity and originality2. Effectiveness on promoting the message 3. Relevance to the theme

Announcement and Presentation of Awards1. Winners will be informed before 9 October 2017 (Monday).2. Those participants do not receive such notification shall be considered their entries

unsuccessful.

SubmissionThe soft copy of design works along with the entry form must be submitted to UHS: 1. By email: cyjlam@polyu.edu.hk;2. Submission deadline: 5:00 pm, 29 September 2017 (Friday). 3. Late submission will not be considered.

Copyright1. Entries should be the original work of one person and must not have been used on

other occasions or have won a prize in any other competitions.2. It is the sole responsibility of each participant to ensure that entries do not infringe

copyright, privacy rights, regulations, orders or directions of any third party. 3. The participants shall bear full legal and related responsibilities arising from any

possible breach of intellectual property rights in respect of their entries, and shall indemnify UHS and other parties concerned against any claims and liabilities arising from any such breach.

4. UHS will own the copyright to the winning entries.5. Winners should provide the original soft copy to UHS in higher revolution for

further publications. 6. UHS acquires the right to publish, exhibit and publicize the winning entries,

including, but not limited to, media such as the internet, printed or electronic marketing materials, etc.

Use of Personal InformationAny information supplied by the participants for the purpose of the competition will be treated as confidential, and will not be released by UHS and any third parties without the participant's permission.

ComplianceAny participant who does not abide by the terms and conditions as set out in this competition document may result in disqualification from the competition. Any such disqualification shall be at the absolute discretion of UHS. The decision of UHS shall be final.

DisclaimersUHS may at its discretion to cancel, modify or suspend the competition. Participants shall not be entitled to any compensation as a consequence to such cancellation, modification or suspension of the competition. All submitted entries will not be returned.
