    Cefaclor l khng sinh cephalosporin th h th hai, c nhiu u im

    ni bt nh tc dng mnh trn vi khun Gram (-) v bn vi cc men -

    lactamase ca vi khun, do vy c th trnh c s khng

    thuc...Cefaclor c s dng trong cc trng hp nhim vi khun nhy cm

    vi thuc, iu tr vim nhim trng ng h hp trn, nhim khun tai mi

    hng, c bit l vim tai gia, vim xoang [5]. Ngy nay cefaclor c

    sn xut trong nc di dng bo ch vin nang nh vin nang cefaclor

    Stada 250 mg ca cng ty STADA Vit Nam, vin nang Mekocefaclor 250

    cng ty C phn Ha-Dc Mekophar....Cefaclor cng l thuc c tn trong

    Danh mc cc dc cht yu cu bo co s liu th tng ng sinh hc

    khi ng k thuc do B y t ban hnh [6].

    Ti Vit Nam cng b quy trnh x l mu v nh lng cefaclor

    trong huyt tng ngi tnh nguyn bng HPLC [8]. Tuy nhin, vic p

    dng ti quy m phng th nghim va v nh, c s vt cht cn thiu thn

    ti cc Trung tm Kim nghim Dc M phm tuyn tnh v cc cng ty sn

    xut thuc trong nc l cn kh khn. Hng ti mc tiu nng cao cht

    lng v hiu qu iu tr ca thuc cefaclor sn xut trong nc th cc n

    v kim nghim u mun c ch ng phn tch, gim st nng

    cefaclor trong huyt tng ngi.

    Xut pht t yu cu thc t , chng ti tin hnh thc hin ti:

    Xy dng phng php nh lng cefaclor trong huyt tng bng sc

    k lng hiu nng cao vi 2 mc tiu chnh:

    1. Xy dng phng php x l mu, kho st iu kin sc k v nh lng

    cefaclor trong huyt tng bng sc k lng hiu nng cao.

    CHNG I: TNG QUAN

    1.1. Tng quan v Cefaclor

    1.1.1. Cng thc ha hc

    - Cng thc phn t: C15H14ClN3O4S.H2O [9],[11]

    - Khi lng phn t: 367,8g/mol [ 9]

    - Cng thc cu to:[9],[13]

    - Tn khoa hc: (6R,7R)-7-[[(2R)-2-amino-2-phenylacetyl]amino]-3-

    chloro-8-oxo-5-thia-1-azabicyclo[4.2.0]oct-2-ene-2-carboxylic monohydrat

    [16]

    1.1.2. Tnh cht ha l

    - Bt trng hoc vng nh.

    - Kh tan trong nc (10g/L), thc t khng tan trong methanol v

    methylene chlorid, chloroform, benzene (0,5g/L).

    - Nhit nng chy dng khan: 2200 C

    - Hng s phn ly: pKa1= 1,5; pKa2 = 7,17;

    - H s phn b du nc logP (octanol/nc) = 0,35 [12] nn vic chit

    cefaclor trong huyt tng bng phng php chit lng - lng l kh khn.

    - Cefaclor hp thu bc sng UV c max = 263 nm, do vy c th nh

    lng cefaclor bng HPLC vi detector t ngoi [3],[12],[20].

    - n nh pH 2,5 - 4,5 [7],[9],[12],[16].

    1.1.3. Dc l, dc ng hc

    *Dc ng hc:

    Cefaclor c hp thu tt khi ung lc i. Vi liu 250 mg v 500 mg

    ung lc i, Cmax t 13 g/mL sau 60 pht.

    - Thi gian bn thi ca cefaclor trong huyt tng t 30-60 pht. Chc

    nng thn mt hon ton th thi gin bn thi ko di thm 2, 3 n 22, 8 gi.

    - Cefaclor phn b rng ri khp c th, i qua nhau thai v bi tit trong

    sa m nng thp.

    - Cefaclor thi tr nhanh qua thn, 85% liu s dng c thi tr qua

    nc tiu dng khng i trong vng 8gi, phn ln thi tr trong 2 gi u.

    - Tng tc thuc: Probenecid lm chm bi tit cefaclor, mt s t

    cefaclor c o thi qua thm tch mu [4], [5], [12].

    *Ch nh

    - iu tr cc nhim khun ng h hp do cc vi khun nhy cm, c

    bit sau khi dng cc khng sinh thng thng m tht bi.

    - Vim tai gia cp, vim xoang cpm vim hng vim amidan ti pht.

    - Vim ph vim ph qun mn trong t din bin.

    - Nhim khun ng tit niu di khng bin chng (vim bng

    quang).

    - Nhim khun da v phn mm do Staphylococus aureus nhy cm v

    Streptococus pyogenes [4], [5].

    *Chng ch nh

    - Ngi c tin s d ng vi khng sinh nhm cephalosporin [4], [5].

    *Thn trng

    - Bnh nhn c tin s d ng vi penicillin.

    - Thn trng khi ch nh cho bnh nhn c tin s mn cm vi

    cephalosporin, c bit l vi cefaclor v penecilin.

    - Ngi bnh c tin s ng tiu ha, c bit l vim i trng.

    - Dng cefaclor di ngy c th gy vim i trng gi mc [4].

    *Liu iu tr v cc dng

    - Dng ng ung, vo lc i

    + Ngi ln:

    - Vim hng, vim amidan, vim ph qun, nhim khun da v m mm,

    nhim khun ng tit niu di: 500-1000mg/ln x ngy 2 ln hoc

    500mg/ln/ngy 3 ln

    - Nhim khun nng: 1g/ln/ngy 3 ln

    - Liu gii hn k n: 4g/ngy [4], [5].

    +Tr em:

    - Liu thng dng : 20-40mg/kg cn nng/2-3 ln/ngy

    - Vim tai gia: 40mg/kg/cn nng/2-3 ln/ngy, liu tng cng trong

    ngy khng qu 1g.

    - Liu t a: 1,5g/ngy [4], [5].

    1.2. Tng quan v sc k lng hiu nng cao (HPLC) [1]

    1.2.1. C s l thuyt [1]

    HPLC cn gi l sc k lng hiu nng cao, l k thut phn tch da

    trn c s ca s phn tch cc cht trn mt pha tnh cha trong ct, nh

    dng di chuyn ca pha ng lng di p sut cao.

    Tc di chuyn khc nhau lin quan n i lc tng i ca cc cht

    ny vi pha tnh v pha ng. Th t ra gii cc cht ra khi ct v vy ph

    thuc vo cc yu t nh bn cht ca pha tnh, phn cc ca pha ng,

    1.3.1. Khi nim:

    Thm nh mt phng php phn tch dc cht sinh hc l qu trnh

    ca phng php m bo phng php t yu cu vi cc ng dng

    phn tch thc t.

    Do qu trnh phn tch mu sinh hc b nh hng bi nhiu yu t nh

    lng mu t, nng thp, ln nhiu tp cht l cc cht ni sinh (cc mui

    v c, lipid, protein v cht chuyn ha) v s khc nhau gia cc c th, nn

    phng php phn tch phi c thit lp v thm nh m bo tin

    cy. [7]

    1.3.2. Ni dung thm nh phng php

    *Tnh chn lc - c hiu

    Tnh chn lc hay tnh c hiu l kh nng nh gi mt cch r rng cht

    phn tch khi c mt cc thnh phn khc, c th l tp cht, hoc cc thnh

    phn cn tr khc. Trong HPLC, tnh chn lc - c hiu c th hin khi

    gc cefaclor nng chnh xc khong 1000 g/mL trong MeOH. T dung

    dch chun gc chun b 6 dung dch chun lm vic WSS1, WSS2, ...WSS6

    trong MeOH 50% c nng chnh xc khong 5, 10, 30, 50, 100 v 300

    2.2.2. Xy dng quy trnh chit cefaclor trong huyt tng

    Mt iu quan trng trong vic phn tch dc cht trong mu mu

    huyt tng l loi b cc protein tan trong nc. Cc phng php thng

    dng kt ta protein; chit lng-lng v chit pha rn. Da trn tnh cht ha

    l ca cefaclor l mt cht rt phn cc v c tnh cht lng tnh nn rt kh

    chit khi huyt tng bng phng php chit lng-lng. Do vy chng ti

    tin hnh kho st chit tch cefaclor trong huyt tng bng phng php

    ta protein vi cc tc nhn gy ta nh MeOH, MeCN, axit Trichloroacetic,

    axit perchloric

    2.2.3. Xc nh iu kin sc k

    Kho st phng php phn tch trn h thng sc k Agilents s dng

    ct sc k pha o C18 (4,6 x 250 mm, 5 m, Zorbax Eclipse XDB-Agilents,

    USA) vi cc pha ng c thnh phn t l khc nhau. T kt qu kho st

    la chn nhng iu kin sc k ph hp c th tin hnh nh lng

    cefaclor trong huyt tng bng HPLC vi detector UV-Vis.

    2.2.4. Thm nh phng php phn tch

    Thm nh phng php phn tch theo hng dn ca US-FDA, tin

    hnh trn cc mu: Mu trng, mu chun v mu QC; nh gi cc ch tiu:

    2.2.4.1. Tnh c hiu chn lc

    X l mu theo quy trnh la chn, tin hnh chy sc k theo iu

    kin xc nh ca cc mu: Mu trng, mu chun, mu th. Ghi li cc

    sc k v p ng pc ti cc v tr tng ng vi thi gian lu ca hot

    cht.

    2.2.4.2. ng chun v khong tuyn tnh

    Chun b mt dy nng chun ca cefaclor trong huyt tng trng

    vi cc nng trong khong 0,5 30 g/mL. Tin hnh phn tch cc mu.

    T p ng pc ca cefaclor ti cc nng tng ng, xc nh s tng

    quan tuyn tnh gia p ng pc v cc nng pha.

    Xy dng phng trnh hi quy v xc nh h s tng quan tuyn

    tnh r. Tnh li nng theo phng trnh hi quy xy dng, xc nh

    ng vi tng nng .

    2.2.4.3. Gii hn nh lng di (LLOQ)

    Tin hnh tim sc k 6 mu trng v 6 mu chun c nng trong

    khong 0,5 g/ml. Ghi li p ng pc ca mu trng v mu chun. Tnh li

    nng ca cc mu kho st da vo ng chun xy dng trong cng

    iu kin. Tnh % lch gia cc kt qu ca 6 ln phn tch.

    2.2.4.4. ng, lp li

    a. ng

    Tin hnh chy sc k cc l mu bao gm LQC (1,5 g/mL), MQC

    (10,0 g/mL), HQC (25,0 g/mL). Mi l mu gm t nht 5 mu c lp c

    cng nng . Xc nh kt qu nh lng cc mu QC theo ng chun

    xy dng trong cng iu kin.

    So snh gi tr trung bnh cc ln nh lng ti mi nng vi gi tr thc.

    b. lp li

    - lp li trong ngy:

    Da theo ng chun pha trong huyt tng trng tin hnh trong

    cng iu kin, tnh li nng ca cc mu QC. Xc nh lp li trong

    ngy bng cch tnh ton RSD gia cc ln nh lng ca mi nng c

    phn tch trong cng mt ngy.

    - lp li khc ngy:

    Tin hnh tng t nh xc nh lp li trong ngy ca cc mu QC.

    Tnh gi tr RSD ca kt qu nh lng cho mi nng cho mi nng

    trong t nht 3 ngy phn tch khc nhau.

    2.2.4.5. Hiu sut chit (T l thu hi)

    Tin hnh sc k cc l mu QC bao gm LQC, MQC, HQC theo quy

    trnh xy dng, mi l mu gm t nht 5 mu c lp c cng nng .

    Song song tin hnh nh lng cc mu chun pha trong dung mi khng

    qua giai on chit tch, c nng tung ng vi cc mu QC.

    So snh kt qu p ng ca cefaclor trong mu QC pha trong huyt tng

    hnh phn tch ngay sau khi pha (nng ban u).

    - Nng cefaclor trong mu chun c xy dng trong mi ngy

    CHNG III: THC NGHIM V KT QU

    3.1. Kt qu xy dng phng php nh lng

    3.1.1. Kho st iu kin sc k

    Da vo nhng c tnh ha-l ca cefaclor v cc cng trnh nghin cu

    c cng b [3],[8],[15],[13],[18], chng ti tin hnh nghin cu

    phng php nh lng cefaclor trn h thng sc k HPLC-Agilents vi

    iu kin sc k thay i nh sau:

    - Ct RP C18 (4,6 x 250 mm, 5 m, ZORBAX Eclipse XDB-Agilents,

    USA).

    - Pha ng cha 10 - 100 mM Natridihydro phosphat (NaH2PO4) pH 3,1

    v MeOH hay MeCN vi cc t l khc nhau.

    - Tc dng khong 1,0 mL/pht.

    - Detector: UV-Vis vi bc sng ghi sc k 263 nm gn vi bc

    sng max ca cefaclor [20].

    - Th tch tim: 100 L.

    Nhn xt:

    Kt qu kho st trn cc mu cefaclor chun pha trong pha ng tng

    ng cho thy: c 4 nng , thi gian lu v cc thng s khc (chiu cao,

    din tch) ca pc khng khc nhau nhiu. C th la chn nng ca h

    m trong pha ng l 10 mM.

    Tuy nhin, khi kho st nh lng cefaclor trong huyt tng vi cc

    nng NaH2PO4 10 mM; 30 mM; 50 mM v 100 mM th ch c nng 50

    mM v 100 mM cho thi gian lu n nh. V vy, nng NaH2PO4 50 mM

    c la chn s dng cho kho st bc tip theo.

    b.La chn cht to cp ion:

    Theo phng php nh lng cefaclor trong USP 32, pha ng la chn

    hai cht to cp ion l 1-pentansulfonat natri v triethylamin. Trong s cc ti

    liu tham kho cng b [8], [19], [12], [15] pha ng u b sung cht to cp

    ion. Tuy nhin la chn cht no ph hp nht cng nh nng ca cht to

    Thay i cc cht to cp ion trong thnh phn pha ng: tetrabutyl

    Qua hnh 3.3 cho thy trn SK pc ca cefaclor b b vai khi khng c

    cht to cp ion. iu ny c khc phc khi c thm cht to cp ion trong

    pha ng. Cefaclor p ng pc tt hn khi thm cht to cp ion tetrabutyl

    ammonium hydroxgen sulfat so vi triethanol amin trn SK (hnh 3.4, bng

    3.1). V vy, chng ti la chn tetrabutyl ammonium hydrogen sulfat l cht

    to cp ion cho bc pht trin phng php tip theo.

    c. T l cc thnh phn trong pha ng:

    Cc ti liu nghin cu c cng b u ghi nhn thnh phn pha ng

    gm h m v dung mi MeOH hoc MeCN. Tuy nhin, trong qu trnh

    kho st chng ti nhn thy cn c s phi hp ng thi hai dung mi

    MeOH v MeCN trong thnh phn pha ng p ng cc yu cu phn

    tch dc cht trong huyt tng. Kt qu ny cng ph hp vi cc nghin

    cu c cng b [8], [16]. Thay i t l cc thnh phn chng ti la

    chn t l thnh phn nh sau: m phosphate-MeOH-MeCN (75:16:9, v/v/v).

    phng php x l mu nh lng cefaclor trong huyt tng c tnh kh

    thi cao.

    Phng php x l mu c la chn l ta protein bng axt

    chloroacetic. Phng php ny c u im n gin, d dng thc hin, t c

    hi v c bit tnh kinh t cao v khng phi dng dung mi, ha cht thit b

    v cc vt t tiu hao t tin nh cc phng php khc.

    Vi thit b phn tch hin c l h thng HPLC, detector UV-Vis. Chng

    ti tin hnh kho st v a ra iu kin sc k ph hp, la chn pha

    ng thng dng c b sung thm cht to cp ion tetrabutyl ammonium

    hydrogen sulfat v iu chnh pH bng Natridihydro phosphat. Vi iu kin

    sc k khng qu phc tp, cefaclor tch hon ton khi cc tp cht c

    trong huyt tng vi thi gian trin khai sc k khong 5pht. V vy c th

    p dng nh lng c nhiu mu trong mt ngy, tit kim chi ph v nhn

    lc.

    Phng php nh lng cefaclor trong huyt tng trong kha lun c

    thm nh theo cc ch tiu ca US-FDA gm: tnh chn lc-c hiu, khong

    tuyn tnh-ng chun, ng, chnh xc, gii hn nh lng di

    LLOQ, mt s ch tiu v n nh trong huyt tng.
