QUI EST CONCERNÉ?
LE JEÛNE
Lors de l’évolution, l’espèce humaine a été soumise à de fréquentes fluctuations d’apport
de nourriture (printemps, été, automne, hiver).
La capacité de l’homme à survivre à ce manque de nourriture survie de l’espèce.
Le corps humain a donc développé une réponse physiologique au manque de nourriture.
Il s’adapte en réduisant et en changeant le métabolisme permettant ainsi de maintenir les
fonctions vitales le plus longtemps possible à partir des réserves propres du corps.
LE JEÛNE
La recherche scientifique s’intéresse aux effets de
privation de nourriture à l’aide de différents modèles
in vitro et in vivo.
Depuis 40 ans beaucoup d’études ont été réalisées
sur le manchot empereur (40Kg), qui jeûne jusqu’à 100
jours d’affilée.
LE JEÛNE
In vivo (bactéries, levures, nématodes, rat, souris) le manque de nourriture périodique en alternance avec les périodes d’abondance montre une amélioration de la réponse au stress et une augmentation de la durée de vie.

Influence de l'heure de prise des médicaments antihypertenseurs sur
• Cette revue a examiné les effets liés à la prise quotidienne en une fois de médicaments le soir plutôt
que le matin sur le risque de décès, les conséquences cardiovasculaires et la réduction de la
pression artérielle.
• 21 études impliquant 1993 patients souffrant d'hypertension primaire ont été identifiées.
• Conclu: « la prise de médicaments antihypertenseurs le soir permettait un contrôle légèrement
supérieur de la pression artérielle que la prise le matin, au niveau de la pression artérielle sur 24
heures. Mais l'effet de ceci sur le risque de décès et sur les conséquences cardiovasculaires
indésirables n'est pas connu ».

Initiation of drug treatment
Initiation of antihypertensive therapy with a two-drug
combination may be considered in patients
with markedly high baseline BP or at high CV risk.

It is recommended to initiate an antihypertensive
treatment with a two-drug combination, preferably
in a SPC. The exceptions are frail older patients and
those at low risk and with grade 1 hypertension
(particularly if SBP is <150 mmHg).

1. Une prise en charge médicale individualisée pour chaque patient est recommandée
2. Les patients hypertendus doivent consulter avant le mois de Ramadan afin d’être examinés et
d’ajuster les prises médicamenteuses
3. Une éducation thérapeutique doit être mise en place pour assurer l’observance thérapeutique et le
respect des règles hygiéno-diététiques
4. Les diurétiques doivent être évités surtout lors du jeûne en période estivale dans les climats chauds
5. Une monothérapie à libération prolongée est recommandée

6. Le patient doit être conscient des différents effets secondaires que peut lui provoquer le jeûne et être en contact permanent avec son médecin traitant.
• Les symptômes seront essentiellement neurologiques et associent des convulsions, des maux de tête très violents, des vomissements. A l’état extrême, une perte de connaissance ou un coma peut survenir, de même qu’une hémorragie massive dans le cerveau qui est alors responsable d’un accident vasculaire cérébral hémorragique.
• Ces symptômes ne sont pas obligatoirement en relation causale avec l’hypertension artérielle. Ils peuvent être liés directement au jeûne.
• Le patient doit signaler ces signes à son médecin traitant qui lui seul doit trancher.

7. Les patients dont l’HTA n’est pas contrôlée, ne peuvent réaliser le jeûne du
mois de Ramadan avant l’équilibration de leur pression artérielle
8. Les patients qui présentent une urgence hypertensive doivent être traités
d’urgence par des médicaments par voie intra-veineuse et rompre le jeûne.

• Baseline and follow-up measurements after 5.8±0.1yr.
• Participants consumed a 300mL drink containing 75g glucose
and 150mg C 13-acetate.
• BP and heart rate were measured at 5-min intervals for
120min after the drink.
• Exhaled breath was collected to calculate the gastric 50% emptying time.
The prevalence of postprandial hypotension (PPH) doubled from
9.1% to 18.2%.
Gastric emptying was slower at follow-up (P=0.04). The fall in
systolic BP (SBP) was related directly to the rate of gastric
emptying at both the initial study (r=0.54, P=0.005) and at follow-
up (r=0.41, P=0.04).
The change in the maximum fall in SBP was related to the increase
in baseline SBP (r=-0.63, P<0.001).

RATIONALE:
It has been suggested that caffeine is most likely to benefit mood and performance when alertness is low.
OBJECTIVES:
To measure the effects of caffeine on psychomotor and cognitive performance, mood, blood pressure and heart rate in sleep-restricted
participants. To do this in a group of participants who had also been previously deprived of caffeine for 3 weeks, thereby potentially removing the
confounding effects of acute caffeine withdrawal.
METHODS:
Participants were moderate to moderate-high caffeine consumers who were provided with either decaffeinated tea and/or coffee for 3 weeks
(LTW) or regular tea and/or coffee for 3 weeks (overnight caffeine-withdrawn participants, ONW). Then, following overnight caffeine abstinence,
they were tested on a battery of tasks assessing mood, cognitive performance, etc. before and after receiving caffeine(1.2 mg/kg) or on another
day after receiving placebo.
RESULTS:
Final analyses were based on 17 long-term caffeine-withdrawn participants (LTW) and 17 ONW participants whose salivary caffeine levels on
each test day confirmed probable compliance with the instructions concerning restrictions on consumption of caffeine-containing drinks.
Acute caffeine withdrawal (ONW) had a number of negative effects, including impairment of cognitive performance, increased headache, and
reduced alertness and clear-headedness. Caffeine (versus placebo) did not significantly improve cognitive performance in LTW participants,
although it prevented further deterioration of performance in ONW participants. Caffeine increased tapping speed (but tended to impair hand
steadiness), increased blood pressure, and had some effects on mood in both groups.
CONCLUSIONS:
The findings provide strong support for the withdrawal reversal hypothesis. In particular, cognitive performance was found to be affected
adversely by acute caffeine withdrawal and, even in the context of alertness lowered by sleep restriction, cognitive performance was not improved
by caffeine in the absence of these withdrawal effects. Different patterns of effects (or lack of effects) of caffeine and caffeine withdrawal were
found for other variables, but overall these results also suggest that there is little benefit to be gained from caffeineconsumption.
