The Committee for Tactical Emergency Casualty Care (C-TECC) was convened to speed the transition of military medical lessons learned from the battlefield to civilian crisis response in order to reduce preventable causes of death in both our first responders and civilian population.

The Tactical Emergency Casualty Care (TECC) guidelines are a set of best practice recommendations for casualty management during civilian tactical

and rescue operations. Based upon the principles of Tactical Combat Casualty Care (TCCC), TECC accounts for the differences in the civilian environment, resources, patient population, and scope of practice from the military combat environment of TCCC.

Taking into account that wounding patterns and mechanisms of injury may be similar in civilian incidents involving ballistic and explosive wounding, TECC recommends treatment modalities

based on the situation and available assets. The primary TCCC tenants of placing far forward timely medical care, and doing the right thing at the right time, are also paramount in the TECC guidelines. As with TCCC, TECC is divided into three phases of care based on the relationship of the provider, the casualty, and the threat.

Considerable expertise and effort has gone into the development of a civilian operational medical standard

in parallel to the successful guidelines of Tactical Combat Casualty Care. Known as Tactical Emergency Casualty Care, this new standard utilizes data and experiences from the military and accounts for the differences of operating in the civilian sector. The TECC guidelines will continue to be updated using evidence-based medical best practices and will remain under the custodianship of the Committee for Tactical Emergency Casualty Care..

Active Shooter & Mass Casualty

Incidents require extraordinary efforts on the part of local

Fire-Rescue/EMS & Law Enforcement agencies Tactical Emergency Casualty Care (TECC) guidelines account for the unique operational considerations and limitations of medical services in high-risk conditions, while prioritizing and focusing medical efforts to first address what must be done to affect survival of injured casualties. TECC is based on the rapid application of point-of-wounding care in order to reduce potentially preventable deaths. Equipment must therefore be forward deployed to enable the immediate implementation of care, requiring TECC treatment devices to be valuable parts of the standardized equipment for Fire-Rescue/EMS and Law Enforcement assets.

Considerations, planning and inter-agency training should occur around the concept of properly trained, armor-protected medical personnel who are escorted into areas of mitigated risk (which are clear but not secure areas), to perform point-of-wounding care, medical stabilization and provide for casualty evacuation to conventional EMS triage/treatment/transport points. These caregivers may be are referred to as, “Rescue Task Forces” (RTFs). The deployment of RTFs should be considered when there is the potential for an ongoing Active Shooter or Mass Casualty Incident involving a potential threat to Fire-Rescue/EMS teams rendering care while under the protection of Law Enforcement Officers. Due to the heightened risks of these conditions, RTFs should be properly equipped with ballistic protective equipment and forward deployed medical gear suitable for performing point-of wounding treatment, stabilization and rapid evacuation of injured casualties.

Trauma to the high thigh and groin region (commonly referred to as “junctional injuries”) can be extremely difficult to address in the prehospital setting – particularly when the wound involves the femoral artery. Inguinal hemorrhage is a life threatening situation, with the casualty potentially bleeding out within three to five minutes of wounding. Penetrating gunshot wounds, traumatic amputations due to explosives or vehicle crashes and accidents involving power tools and heavy machinery are a few instances where paramedics would be called upon to stop the massive blood loss resulting from this type of injury.

In the military, high extremity wounds in the thigh/groin account for up to 20% of preventable combat deaths found on the modern battlefield. Increased foot patrols during the counterinsurgency phase of military operations in Afghanistan resulted in a significant rise in severe IED blast injuries characterized by multiple limb amputations and damage to the groin and pelvic region. Historically, injuries of this nature were considered non-survivable since there were no solutions available at the point of wounding, exposing a major gap in currently fielded medical therapies.

Hypothermia Prevention

Hypothermia is a common finding among combat and civilian casualties, often complicating the management of those with severe trauma. Although permissive hypothermia may be protective in anoxic brain injury following resuscitation from non-traumatic prehospital cardiac arrest 1, secondary hypothermia associated with traumatic injuries and hemorrhagic shock is often overlooked and can be fatal.

Hypothermia is defined as a core temperature of less than 95°F (35°C) 2 and is referred to as secondary hypothermia in the trauma casualty. It is caused by an underlying pathology that prevents the body from generating enough core heat. Body heat is generated as a result of oxygen consumption, which is reduced during conditions of shock and results in a decrease in core temperature.3 Hypothermia is the third most serious condition in a trauma casualty, ranking close to hypoxia and hypovolemia.4 The “lethal triad” of hypothermia, acidosis and coagulopathy is a significant contributing factor to the mortality of trauma casualties and has been noted as a major reason for resistance to resuscitation after trauma. 5, 6

A core body temperature less than 89.6°F (32°C) in trauma casualties is associated with a 100% mortality rate and any decrease in temperature below 95°F (35°C) is a poor prognostic sign.3 Secondary hypothermia

in traumatic injured casualties remains a common problem that occurs early during casualty care and is not limited to cold environments, occurring in warm, ambient temperatures as well.

As many as 66% of civilian trauma casualties arrive in emergency departments with core temperatures of less than 96.8°F (36°C), and approximately 80% of casualties who subsequently die from trauma were noted to have a body temperature less than 93.2°F (34°C). 7 Hypothermia has been found in recent years to be more prevalent in combat trauma than was previously realized and was found to independently contribute to overall mortality. 8

Current trauma care guidelines call for active efforts to prevent hypothermia.9, 10 These recommendations are based on studies that document the extraordinarily high association between hypothermia and mortality in trauma casualties. Basic, effective interventions should be performed early in casualty care to reduce further heat loss in the patient. Since the prevention of hypothermia is much easier than its treatment, significant attention should be paid to preventing hypothermia and should start as soon after wounding as is tactically possible.

NitroStrike™ Nitrogen Foam System is a new, patented CAFS (Compressed Air Foam System) technology that brings increased capabilities and performance in a cost effective solution. Our unique blending system generates a more durable bubble matrix, allowing better adhesion to surfaces and more effective fire suppression. Additionally, NitroStrike™ maximizes foam expansion rates and bubble durations for optimized fire fighting, meeting NFPA standards. Our systems allow the use of all foams, including decontamination solutions, without the need to modify the base system.

A. NitroStrike™ 2.5 Gallon portable Fire Suppression Systems allows you 1 min. 45 sec. of sustained fire fighting/decontamination at a 25’ distance from your target. Tested by UL for 2A and 10B fires. NitroStrike™ 2.5 is also simple to refill, requiring less support and allowing you to rapidly reuse the system when time is limited.
