Infection : Host & microorganism

Diagnosis : 1. ผปวยมอาการส าคญไดแก ไข รวมกบ อาการผดปกตของอวยวะทตดเชอ เชนทผวหนง จะมอาการปวด บวม แดง รอน ทปอดจะมอาการไอ หายใจไมสะดวก เปนตน 2. ผลการตรวจทางหองปฏบตการ clinic พบ wbc count สง organ function test ผดปกต 3. ผลการตรวจทางหองปฏบตการ microbiology พบหลกฐานวามเชอ หรอสาร พษของเชอตนเหตกอโรค ตลอดจนพบ specific antibody

1. Community acquired infection (CAI)

เปนการตดเชอ (มกไมดอยา) ขณะอยในชมชน หรอนอกโรงพยาบาล

2. Nosocomial (hospital) acquired infection (NI)

เปนการตดเชอ (มกดอยา) หลงเขาอยในโรงพยาบาลแลวเกน 48 – 72 ชวโมง หรอหลงออกจากโรงพยาบาลภายใน 3 วน

Virulence proteins & genes & drug resistant (R) gene Enzymatic toxins : collagenase, .. Etc Toxins : endotoxin, exotoxins (enterotoxin,…) ผลทเกดขน host cells / organ(s) dysfunction เกดอาการปวย

Morphology (รปราง) (cocci / rods / spiral or spirochetes) 3. O2 requirement for growth (aerobic / facultative/ microaerophilic / anaerobic)

การวนจฉยทางหองปฏบตการ

ประโยชนของการใชหองปฏบตการ 1. มการตดเชอหรอไม ชออะไร ผลการทดสอบยาเปนอยางไร 2. การรกษาการตดเชอไดผลหรอไม 3. ม endemic strain หรอ epidemic/outbreak spread หรอไม จากแหลงไหน 4. ขอมลส าหรบวางแผนควบคมการระบาดของโรคตดเชอในโรงพยาบาล 5. ตดตามการปองกนการแพรกระจายของเชอ / R-genes ไดผลหรอไม

การตรวจทางหองปฏบตการ (ปฏบตตามคมอมาตรฐาน) Specimen :

Selection : เลอกถกต าแหนงและเวลา หลกเลยง contamination

Collection : เกบปรมาณพอเหมาะ ใสในภาชนะถกตอง หลกเลยง contamination

Transportation : time น าสงโดยรวดเรว temp เหมาะสม เลอกใช transport medium ทถกตอง (ถาจ าเปน) สงทควรท า * 1. กรอกขอมลทมผลตอการตรวจลงในใบ request ใหครบถวน เชน ชอผปวย ชนดตวอยาง วนท และ เวลาทเกบ ยาตานจลชพ , ผลการตรวจ ครงกอนทเกยวของกน …… 2. ประสานและรวมมอกบหองปฏบตการตรวจวนจฉยลวงหนา (ถาจ าเปน)

การตรวจวนจฉยทางหองปฏบตการ

Macroscopic & Microscopic examination

ม bacteria หรอไม รปรางและการตดสเปนอยางไร (~ 30-60 นาท)

Isolation & Identification

genus & species (biochemical, antigenic, nucleic acid)

ทราบชอเชอทเปนตนเหต (~ 2 -10 วน)

Antimicrobial susceptibility ทราบยาทใช จะใช ไดหรอไมได (~ 2 วน)

Detection : Ag & Ab, nucleic acid

ทราบชอเชอทเปนตนเหต (~ 1-2 วน)

Endemic / Epidemic Study ทราบวาเปนสายพนธทก าลงระบาดอยหรอไม (~ 2 -10 วน)
