Testing Strategy Paradigms
1. Reflex approach: Conduct existing KRAS testing and for those ~60% of patients
that are wild-type, reflex to test for any exons/codons not tested
a. Reflex option, CE-IVD: Transgenomic (TBIO) RAScan (requires confirmation of MT by
Sanger sequencing, expect this to occur on average in one exon per six patients)
b. Reflex option, RUO/Laboratory Developed Test: Sanger sequencing of all exons not
tested
2. Test for all KRAS and NRAS exons in parallel:
a. CE-IVD: Transgenomic (TBIO) RAScan (requires confirmation of MT by Sanger
sequencing, expect this to occur on average in one exon for 50% of patients)
i. On average, 1 exon to be subjected to Sanger sequencing for each two patients
b. RUO/Laboratory Developed Test: Sanger Sequencing of all exons (requires 7 amplicons
to be sequenced)
i. Seven sequencing runs per patient (unless staging of exons conducted)
c. RUO/Laboratory Developed Test: Next Generation Sequencing of all exons in parallel
i. Exons tested in parallel (together with additional oncogenes) (NB: Currently LifeTech panel
missing NRAS c59 and c117; Illumina missing NRAS exon 4)
