Children with anterior mediastinal masses are at risk for life-threatening airway compromise during anesthesia, and can present a diagnostic and management challenge for pediatric surgeons. There were 26 patients with anterior mediastinal masses over a 15-year period. The mean age was 11.3 years, and there were no gender differences. The diagnoses were lymphoma (62%, 16/26), leukemia (15%, 4/26), and other (23%, 6/26). Diagnosis was made by CBC/peripheral smear in 2/4 patients with leukemia, by bone marrow biopsy in 2/4 patients with leukemia, by thoracentesis in 3/16 patients with lymphoma, by lymph node biopsies in 6/16 patients with lymphoma, and by biopsy of a mediastinal mass in 7/16 patients with lymphoma and in 6/6 patients with other diagnoses. Radiographic evidence of airway compression was seen in 62% (16/26) of children. Only 12% (3/26) had a tracheal cross-sectional area (TCA) <50%. Correlation of symptoms with anatomical airway obstruction or complications was poor. Pulmonary function studies were obtained in 38%, 10/26 children. Only 2 children had a PEFR (peak expiratory flow rate) <50% predicted. This data also correlated poorly with anatomical airway obstruction or complications. 3 patients had anesthesia-related complications: one desaturation during induction prior to median sternotomy, one with significant desaturation and bradycardia during biopsy under local anesthesia with minimal sedation, and one with prolonged (5 days) mechanical ventilation after general anesthesia. 2 of these patients had a TCA <50%, and 2 had SVC obstructions. There were no anesthesia-related deaths, and the overall survival was 85% (22/26). CONCLUSION: Anterior mediastinal masses in children should be approached in a step-wise fashion with multi-disciplinary involvement, starting with the least invasive techniques and progressing cautiously. The surgeon should have a well-defined and preoperatively established contingency plan if these children require general anesthesia for diagnosis.

BACKGROUND: The presence of a mediastinal mass in a child poses significant anesthesia-related risks including death. To optimize outcome clinicians must be able to predict which patients are at highest risk of anesthetic complications. METHODS: We conducted a retrospective review of 118 pediatric patients who presented with mediastinal masses. We investigated their medical records for clinical symptoms and signs at presentation and reviewed their chest radiographs, computed tomography scans, and echocardiograms and electrocardiograms when available. We then conducted analyses to identify clinical and diagnostic imaging features associated with anesthesia-related complications. RESULTS: Eleven of 117 [9.4%, 95% confidence interval (CI) 4.1-14.7%] patients experienced an anesthesia-related complication. Four preoperative features were significantly associated with anesthetic complications: orthopnea (P = 0.033, odds ratio (OR) 5.31, 95% CI, 1.15-24.56), upper body edema (P = 0.035, OR 8.00, 95% CI, 1.16-55.07), great vessel compression (P = 0.037, OR 5.41, 95% CI, 1.11-26.49), and main-stem bronchus compression (P = 0.044, OR 5.11, 95% CI, 1.05-24.92). The presence of pleural effusion (P = 0.060, OR 4.53, 95% CI, 0.94-21.96) or tracheal compression (P = 0.061, OR 5.09, 95% CI, 0.93-27.81) also appeared to be risk factors. Although the rate of anesthesia-related complications detected in our cohort was comparable with that found in earlier studies, the events were less severe. CONCLUSIONS: Patients who present with orthopnea, upper body edema, great vessel compression and main stem bronchus compression are at risk of anesthesia-related complications. The low severity of complications in our series may reflect a combination of factors: use of the least invasive method such as interventional radiology to obtain tissue for diagnosis, completion of a thorough preoperative assessment and minimal anesthesia intervention.

• L'anestesista e il chirurgo devono pianificare una strategia comune di intervento.
• ll paziente asintomatico da sveglio può andare incontro brutalmente a ostruzione delle vie aeree durante anestesia.
• Nei casi di massa mediastinica anteriore la compressione delle strutture è massimale in posizione supina perciò se possibile il paziente va posto in decubito laterale o perfino in posizione prona.
• Nei pazienti gravemente compromessi può essere necessario condurre una biopsia diagnostica o manipolazioni sulle vie aeree soltanto in anestesia locale poichè i rischi di un'anestesia generale potrebbero essere maggiori di qualsiasi beneficio

L’anestesia è necessaria per: biopsie diagnostiche e stadiazione (elezione) ostruzione acuta delle vie aeree (urgenza/emergenza)
VALUTAZIONE PREOPERATORIA
• Anamnesi dettagliata
• Esame fisico mirato ad evidenziare :
Segni e sintomi che indicano compressione delle vie aeree (stridor, ortopnea, dispnea, cianosi, tosse, riduzione del murmure e sibili).
Segni e sintomi che indicano una compressione critica sulle strutture cardiovascolari (affaticabilità, edema del collo e/o della faccia, astenia, turgore giugulare, cefalea, edema della papilla, dispnea, polso paradosso, ortopnea, modificazioni posturali della PA
• ECG, Ecocardiogramma
• Rx di collo e torace in 2 P, TAC con contrasto del torace
• Fibroscopia per valutare l'ostruzione dinamica delle vie aeree

URGENZA-EMERGENZA

• Attenzione ai segni clinici denunciati in posizione supina.Se durante anestesia si perde il controllo delle vie aeree, posizionare il paziente in decubito laterale o prono.
• Utilizzare il fibrobroncoscopio a paziente sveglio per l'intubazione, ponendo attenzione al collasso dinamico delle vie aeree,
• Mantenere la ventilazione spontanea ( i curari possono condurre alla "perdita" delle vie aeree).
• Se emergenza da compressione delle vie aeree procedere con il broncoscopio rigido per identificare l'ostruzione e determinarne l'estensione della compressione sulle vie aeree
CONCLUSIONI
• La biopsia chirurgica può essere intervento non sempre semplice (coinvolgimento strutture vitali e vie aeree)
• Valutazione preoperatoria e planning accurato
• Scelta della procedura efficace e meno invasiva
• Richiede anestesista e chirurgo esperto
• Campionamento del materiale in modo competente
• Confronto tra Anatomo-patologo e Chirurgo
• Anatomo-patologo esperto e dedicato
