Have they responded with functional or dysfunctional patterns of behavior? ( maladaptie rsponses ,such as rejection of the sick person, or overprotectiion )

Frequent attendance with minor illnesses
with the same symptomes or with multiple complaints
with a symptom that has been present for a long time
with a chronic disease that dose not appear to have change
by a parent with a child with minor problems (the child as a presenting symptom of illness in the parent)
Incongruity between the patient’s distress and the comparatively minor nature of the symptoms
Failure to recover in the expected time from an illness , injury , or operation
Failure of reassureance to statisfy the patient for more than a short peroid
An adult patient with an accompanying relative
What is a “family”?
A group of people – often but not necessary
related by blood or marriage – with a commitment to live with and care for one
another over time

ระยะที่ 1 เร่ิมสร้างครอบครัว วัยผู้ใหญ่ตอนต้น 1.สร้างกฎระเบียบใหม่ร่วมกัน 2.ปรับบทบาทใหม่ของแต่ละคน 3.สร้างความสัมพันธ์ระหว่างคนสองคน
ระยะที่ 2 ครอบครัว เร่ิมมีบุตร
วัยผู้ใหญ่ตอนต้น 1.ปรับตัวให้เข้ากับสมาชกิใหม่ 2.ปรับบาทบาทของตนใหม่ 3.สานความสัมพันธ์ระหว่าง2 คน
ระยะที่ 3 ครอบครัว ที่มีบุตรเล็ก
วัยผู้ใหญ่ตอนต้น 1.เตรียมพืน้ที่เป็นสัดส่วนให้ลูกที่เร่ิมโต 2.เตรียมพืน้ที่ที่เป็นสัดส่วนให้คู่ของตน 3.ปรับตัวอยู่ระหว่างความเป็นส่วนตัวของชีวติคู่และความเป็นครอบครัวเดียวกับลูก

ระยะที่ 5 ครอบครัวที่มีบุตรวัยรุ่น
เร่ิมเข้าสู่วัยกลางคน
1.ก าหนดขอบเขตและภาระหน้าที่ที่วัยรุ่นควรมีในบ้าน 2.เตรียมใจกับการแยกตัวของวัยรุ่นรวมถงึการแยกออกจากบ้านในอนาคต
ระยะที่ 6 ครอบครัวที่บุตรแยกออกจากบ้าน
วัยกลางคน 1.ปรับตัวกับการหมดบทบาทความรับผิดชอบของ “พ่อ-แม่” 2.ลูกแยกออกจากบ้าน รับผิดชอบตนเอง มีแฟน มีคู่ครอง 3.พ่อแม่พี่น้องของตนเองเร่ิมเข้าสุ่วัยชรา และเร่ิมเจบ็ป่วย 4.อาชีพม่ันคงและประสบความส าเร็จ 5.ปรับตัวกับความสัมพันธ์ของคู่ครองที่มีมายาวนานและเร่ิมชนิชา

1.ปรับตัวกับความเปล่ียนแปลงของร่างกายตามสังขาร ความเจบ็ป่วยของตนและคู่ครอง 2.ปรับตัวกับการสูญเสียความเป็นตัวของตัวเองโดยต้องพึ่งพาลูกหลานมากขึน้ 3.ปรับตัวกับการสูญเสียคู่ชีวติ เพื่อน และสังคมที่ตนเองเคยคุ้นเคยมาตลอดชีวติ เหมือนถูกทิง้ให้อยู่ในโลกใหม่

Don’t ignor patient’s expectations
Patients may be unwilling to listen to their doctors unless they believe they first have been listened to themselves.
Timing is important. (Asks for a patient’s expectations too early in the interview, the patient may think the doctor is evading making a diagnosis and there for may be reluctant to say much.)
Non compliance : patient’s expression of disagreement about treatment goals
Four basic tactics make up this approach
Separate the people from the problem
Focus on interests, not positions
Generate a variety of possibilities before deciding what to do
Use objective criteria to judge the solution ,rather than pit one personal opinion against another

I.Primary prevention : ป้องกันในระยะที่ยังไม่มีโรคเกิดขึน้ 1. Health promotion : ประกอบด้วยงาน การให้สุขศึกษา การโภชนาการ สุขวิทยาส่วนบุคคล การอนามัยสิ่งแวดล้อม การสุขาภบิาล การวางแผนครอบครัว 2. การควบคุมกันเฉพาะโรค : vaccine, ควบคุมพาหะน าโรค II. Secondary prevention: ป้องกันเม่ือมีโรคเกิดขึน้แล้ว (Early diagnosis & Px) , Screening of diseaseเช่นวัดความดัน ตรวจระดับน า้ตาล เอก็ซเรย์ดูวัณโรค ช่วยลดความรุนแรง ,การกระจายโรค 3. Tertiary prevention : ลดภาวะแทรกซ้อน ความพกิาร ได้แก่ การก าจัดความพกิาร และ ฟ้ืนฟูสุขภาพ

การแก้ปัญหาสุขภาพองค์รวม
เมื่อมององค์รวมหลายคนจะกลุ้มใจว่าจะแก้ปัญหาได้อย่างไรเพราะแพทย์ถูกสอนให้เป็นผู้แก้ปัญหา โดยไม่ทราบว่าปัญหาต่างๆของมนุษย์อาจมีทางออกโดยไม่ต้องการแก้ไข หรือไม่ต้องมีคนช่วย(ที่ไม่ใช่เจ้าของปัญหา) มาช่วยแก้ให้
แพทย์มีหน้าที่ช่วยสรุป ปัญหาคืออะไร เป็นปัญหาจริงหรือไม่ เป็นปัญหาจากมุมมองของใคร ต้องแก้ไขหรือไม่ หรือต้องการให้แพทย์ประสานงานให้ หรือเพียงต้องการปรับทุกข์ สิ่งที่แพทย์จะช่วยได้ก็ต้องมองอย่างองค์รวมให้เป็นก่อน
