Inleiding • Momenteel zijn er twee kanker-inducerende-
infectieziekten waarvoor op grote schaal vaccinatie programma’s zijn ingevoerd, nl. hepatitis B virus infectie (HBV) en infectie met humaan papilloma virus (HPV).
• Ontwikkeling van een veilig en doeltreffend HBV vaccin was een belangrijke medische verwezenlijking van de vorige eeuw. Begin deze eeuw is er ook een vaccin dat beschermt tegen bepaalde HPV infecties.
Inleiding • Het toedienen van een nieuw vaccin aan de
juiste doelgroep is even belangrijk als de ontdekking, ontwikkeling, evaluatie en goedkeuring. Misschien is bereiken van de juiste doelgroep wel de moeilijkste etappe.
• Tijdens de voordracht zullen we dieper ingaan op overeenkomsten en verschillen van HBV en HPV betreffende infecties, vaccinatieprogramma’s, en uitdagingen voor de toekomst.
Ziektelast HBV • Wereldwijd is 1 op 3 mensen
gedurende zijn leven blootgesteld aan HBV
• 6% van de wereldbevolking chronische drager
• 10-15% van HBV chronische geïnfecteerde ontwikkeld HCC.
• In 2010 verantwoordelijk voor 650000 doden, waaronder 340000 te wijten aan HCC.
• Transmissie: seksueel, parenteraal, perinatal, horizontaal
HPV • 80% van vrouwen komt in
contact met HPV • 11-12% van de vrouwelijke
wereldbevolking heeft een HPV infectie.
• Jaarlijks 610000 kankers, waarvan 530000 baarmoederhals kankers te wijten aan chronische HPV infectie.
• 266000 doden door cervicale kanker
• Transmissie: huid-huid, seksueel

• Eerste vaccin beschikbaar in 1982 – Initieel risicogroepen – Extensie naar universele
vaccinatieprogramma’s • Meer uitgebreide implementatie
vanaf 1990 • Het duurde 23 jaar voor 181
landen universele zuigelingen/kindervaccinatie hadden ingevoerd.
• Screening voor chronische HBV – Niet systematisch – Laat gestart
HPV • Eerste vaccin beschikbaar in
2006-2007 – Geen risicogroep benadering – Dadelijk universele vaccinatie -
programma’s – Sterke vraag vanuit private sector – Snelle formulering van
aanbevelingen. • Begin 2012 was HPV vaccinatie
geïntroduceerd in 40 landen • Cervicale kanker kan
vroegtijdig worden opgespoord door screening.

HBV versus HPV vaccins verschillen - 1 HBV
• HBV vaccins (zowel de vroeger plasma afgeleide als de recombinante) beschermen tegen alle HBV genotypes, d.i. 100% van de infecties en complicaties
• Goede bescherming zelfs na verdwijnen van antilichamen. Afwezigheid van chronische infectie 20-30 jaar na vaccinatie is gedocumenteerd.
• Werking van immuun geheugen bewezen, anamnestisch antwoord bij herhaalvaccinatie.
HPV • Huidige HPV vaccins
beschermen tegen twee hoog risico genotypes 16 en 18, goed voor 70% van de BMHkankers. En belangrijkste genotypes in ander HPV cancers.
• Primaire bescherming door antilichamen; hoge titers worden bekomen na vaccinatie. Lange termijn bescherming moet nog verder bestudeerd worden.
• Werking immuun geheugen aangetoond maar nog geen bewijs of een contact met HPV virus ook zal leiden tot snelle productie van antilichamen.

strategieën • Inspanningen om universele zuigelingen-
vaccinatie door te voeren gecombineerd met vaccinatie van groepen met hoog risico voor infectie en transmissie zijn gepland.
Eliminatie van HBV transmissie is een haalbaar doel (2)
• Uitdagingen – Afspreken van nationale en regionale targets – Tijdig toedienen van eerste dosis bij geboorte, ook
voor kinderen geboren buiten een hospitaal of gezondheidscentrum
– Vaccinatie van alle personen met hoog risico op besmetting, ook moeilijk bereikbare of “underserved” groepen
– Uitbreiden en onderhouden van huidige strategieën en programma’s.

• Tao Bian of the Chinese Centers for Disease Control and Prevention et al. show that breakthrough mutants capable of infecting vaccinated people are emerging, threatening the program’s long-term success. The prevalence in children less than 18 years old of such mutants, which occur in the surface protein of the virus, rose from 6.5% in 1992, before universal vaccination, to 14.8% in 2005, says Bian. 0.49% (9/1,825) in 1992 to 0.33% (15/4,596) in 2005 (a decrease of 33%, i.e., about one-third). Majority of variants were no vaccine escape variants. [2]
• The immunization program decreased the prevalence of hepatitis in children under 5 by an order of magnitude, from 9.67% in 1992 to 0.96% in 2006, resulting in an estimated 80 million more children free from HBV infection.

• Voorkomen van HBV en HPV geïnduceerde kanker door vaccinatie is mogelijk.
• Het blijft een uitdaging om duurzame programma’s met een goede couverture op te zetten.
• Globale en aangehoudende inspanning is nodig.
