GA• Drowsy, febrile, mild dry lips, no
sunken eyeballs
Skin• Mild pallor, no jaundice, (+)
scars from insect bite both legs, no petechiae
• Good skin turgor
Physical Examination
HEENT
• Mildly pale conjunctiva, sclera not icteric or injected
• No ear or nasal secretion, no tender sinuses
• Mildly injected pharynx, enlarged tonsils (2+), no exudate
Neck• No stiffness• No thyroid enlargement• No lymph node enlargement
Chest/ Lungs• No lesions, no retractions• Equal breath sounds, no stridor, no
crepitation, no wheezes
Physical Examination
Heart• Tachycardia, regular rhythm,
normal S1 S2, no murmur
Abdomen• No lesions, normoactive bowel sounds,
tympanitic• Soft, no tenderness, no spleen or liver
enlargement
Extremities• No gross deformities, no cyanosis, no
clubbing, no edema• Capillary refill of < 2 sec
Physical Examination• Neurologic Exam
– Conscious; oriented to time, place, person; GCS=15
– Nystagmus – negative– CN: II,III-normal visual acuity,
pupils equally reactive to light; III,IV,VI-intact EOM; V-intact masseter muscle contraction; VII- no facial asymmetry; VIII- can hear, bilateral; IX,X-intact gag reflex; XI-can shrug shoulders, bilateral; XII-tongue midline on protrusion
– Babinski absent; no nuchal rigidity; no Brudzinski; no Kernig’s sign
– Motor: normal tone, power grade 5/5, intact coordination
– Sensory: intact– DTRs: 2+, all
Problem List
• High-grade fever (Day 3 on admission)• Vomiting 1 day with decreased appetite• Mild epigastric pain• Mild injected and enlarged tonsils

• BUN 9, Crea 0.35• Dengue Ns1Ag, IgG,
IgM = negative• Influenza A Ag, B Ag =
negative• RSV = negative• UA
– WBC 1-2, RBC 0-1
After Initial Investigation..
Influenza and UTI may be ruled out.
Dengue is unlikely (to be confirmed after the 4th day).

Management
• Record V/S, I/O• Laboratory:
– CBC, E’lyte, BUN, Crea
– Dengue Ns1Ag, IgG, IgM
– Influenza A, B– RSV
• UA
• IV maintenance + 3% deficit (15 kg)– 5% D/N/3 IV drip @
70cc/hr
• Med:– Paracetamol syrup 1
½ tsp PO prn for fever q 4-6 hr
– Ceftriaxone 800mg IV drip OD
