NGUYÊN NHÂN TRONG DỊCH TỄ HỌC
Causality in Epidemiology
PGS,TS LÊ HOÀNG NINH
Định nghĩa nguyên nhânDefinition of causality
Nguyên nhân và hệ quảCause and effect
Chúng ta dễ dàng quan sát hệ quả thường là bệnh tật hay tử vong Nhưng dịch tễ học quan tâm đến nguyên nhân của các hệ quả Is there any single cause that results in
a disease or death inevitably? If yes, very few
Definition of cause
Dictionary defines cause as
a) The producer of an effect, result, orconsequence.
b) The one, such as a person, an eventor a condition, that is responsible foran action or a result.
Cause

Rothman pie
Cause: A cause of a disease is an event, condition, or characteristics that plays anessential role in producing an occurrenceof the disease.
Rothman points out that the cause ofany effect must consist of a constellationof components that act in concert.
Sufficient cause: A set of minimalconditions and events that inevitablyproduce disease.
Component cause: An individual event, condition, or characteristic required by agiven sufficient cause.
Necessary cause: A component causepresent in every sufficient cause.
Often there are many sufficient causes, which may produce a given effect.
A given component cause may play arole in any number of sufficient causes(i.e. different sufficient causes mayshare some component causes

How to establish causal inference
For infectious disease Koch’s postulate
For Chronic disease Hill’s criteria
Henle-Koch postulate (1884)
1. The parasite (the original term) must bepresent in all who have the disease.
2. The parasite can never occur in healthypersons.
3. The parasite can be isolated, cultured andcapable of passing the disease to healthyexperimental animal.
4. The organism must be reisolated from theexperimentally infected animal.
*Not suitable for all microbial diseases
Limitations of Koch postulate
Bradford Hill proposed the following criteriafor a association to be causal:
Hill’s criteria:
1. Strength of association2. Consistency3. Specificity4. Temporality
Hill’s Criteria of causal association

Biological gradient
Plausibility

Analogy
Strength of Association
The stronger an association, the morelikely to be causal in absence of knownbiases (selection, information, andconfounding).
May be misleading for unknownconfounding.
Strength of Association
Độ mạnh của sự liên quan(rule of thumb)
RR (Relative risk)1.1-1.31.4-1.71.8-3.03-88-1616-4040+
“Ý nghĩa)"Weak (yếu)Modest ( nhẹ)Moderate ( trung bình)Strong ( mạnh)Very strong ( rất mạnh)Dramatic ( cực mạnh)Overwhelming ( siêu mạnh)
Consistency: hằng định
Replication of the findings by differentinvestigators, at different times, in
different places, with different methodsand the ability to convincingly explaindifferent results.
Specificity: chuyên biệt
This means a cause lead to a single effect, not multiple effect
However, a single cause often leads tomultiple effect. Smoking is a perfectexample
Temporality: thời gian
It refers that the putative cause in factprecede in time the presumed effect.
First exposure, then disease.
This is essential to establish acausal relation
Biologic Gradient / Dose Responseincremental change in disease rates in conjunctionwith corresponding changes in exposure.
Risk
Exposure
Need to consider threshold and saturation effects, characteristics of the exposure.
Plausibility
Does the association make sensebiologically.
Coherence hợp lý
Does a causal interpretation fit withknown facts of the natural history andiology of the disease
Very similar to plausibility
Experimental Evidence
The demonstration that under controlled
conditions changing the exposure causes
a change in the outcome is of great value,
some would say indispensable, forinferring causality.
Analogy: tương tự
We are readier to accept arguments thatresemble others we accept
Have there been similar situations in thepast?
Hill’s criteria for causal inference
Except for temporality, none of the Hill’scriteria is absolute for establishing acausal relation
Hill himself recognized that and statedclearly
He argued that none of his criteria isessential
However, temporality is absolutelyessential to establish a causal relation
Implication of casual inference Presence of a cause leads to excessamount of disease compared toabsence of the cause An increase in the amount of cause
leads to an increase in disease. Reduction in the amount of cause leads
to a reduction in disease Challenge is to identify the most
important component cause for publichealth intervention
